CHANGES IN ARTERIOLE REACTIVITY TO NORADRENALINE UNDER CONDITIONS OF HYPERHOMOCYSTEINEMIA.

The aim of the research was the experimental study of one of the possible mechanisms performing contractile activity of arterioles in hyperhomocysteinemic animals. Moderate hyperhomocysteinemia was induced in male rats (120-160 g., n=12) by adding L-methionine to the drinking water during 4 weeks. Control animals (n=12) administered the normal water without limitation. The experiments were performed on the isolated arterioles of first line of the rat's soft muscle (Gracilis). Reactions were studied on normal and deendothelized segments of arterioles. Increased homocysteine has been found to be

VMA21-X-linked myopathy in Peru: characterization of three families.

VMA21-X-Linked related myopathy is a rare neuromuscular disease characterized by a wide phenotypic spectrum ranging from neonatal forms with severe muscular weakness and respiratory failure, to mild childhood or adult-onset forms with slowly progressive muscular weakness and variably elevated serum creatine kinases. This condition is also called X-linked myopathy with excessive autophagy (XMEA) due to the presence of autophagic vacuoles with sarcolemmal proteins. Here we describe the clinical, muscle imaging, and genetic findings of six VMA21-X-Linked related myopathy patients belonging to thr

Advances in the surgical management of anal incontinence.

Standard procedures for anal incontinence due to trauma (obstetric lesions, iatrogenic lesions in connection with anal surgery) have been overlapping suture of the external anal sphincter and, for idiopathic incontinence, postnatal repair according to Parks. In cases where these operations fail, or if a pronounced sphincter destruction is found, transposition of striated muscles (primarily the gracilis and the gluteus maximus) may be performed. In patients where the incontinence is due to a primary neurological disease, implantation of an artificial sphincter or a neurostimulator may be the on

Proposal of new category for congenital unilateral upper limb muscular hypertrophy.

According to congenital anomalies of the hand and forearm classifications, the common characteristic of overgrowth problems is the skeletal overgrowth of part or all of the hand. Congenital unilateral muscular hypertrophy of the upper extremity has been classified under the overgrowth (gigantism) problems as whole-limb hemihypertrophy. The common characteristic of overgrowth problems is the skeletal overgrowth of part or all of the hand, which is not prominent in these patients. Only 15 cases with this anomaly have been reported since 1962. These patients have abnormal muscles with hypertrophi

Foot and ankle soft-tissue deficiencies: who needs a flap?

The patient who needs a flap for a deficit of the foot or ankle soft tissues is any patient who has a break in the integument. While skin grafts often will suffice, if there is a full-thickness loss, particularly over the plantar bony prominence such as the heel or metatarsal, one should strongly consider flaps. In my practice, the flap procedure most commonly performed for the metatarsal region is the V-Y advancement; for the hindfoot, free-tissue transfer; and for the Achilles region, a sural flap. If a lesion is small and the vascular inflow is good, I use free-tissue transfer of a thin ski

Novel TNNT1 mutation and mild nemaline myopathy phenotype in an Italian patient.

Mutations in the TNNT1 gene cause an infantile, lethal form of myopathy named "Amish" Nemaline Myopathy. Adult patients are very rarely described. We report a 49-year-old patient who presented a slowly progressive phenotype characterized by myalgia, exercise intolerance and dyspnea since infancy. In adult life she lapsed into a coma as a result of acute respiratory failure, with the need of tracheostomy, subsequently removed once her respiratory condition improved. Afterwards, non-invasive ventilation was started. Short stature, contractures, a small size posterior cranial fossa and osteonecro

Reconstruction of a large pelvic defect by transfer of a quadruplet combination of pedicled flaps from the medial thigh using bilateral muscular gracilis flaps and bilateral vertical posteromedial thigh (vPMT) propeller flaps-A case report.

The reconstruction of defects of the perianal area and vagina places a high demand on a reconstructive surgeon. Different reconstructive methods include a skin graft, a local skin flap, a musculocutaneous flap, and a pedicled perforator flap. Here, we report the case of a 59-year-old female patient with a pelvic defect, who underwent reconstruction with a quadruplet combination of pedicled flaps from the medial thigh, due to an extensive resection of a recurrence of a squamous cell carcinoma of the anus, vulva, and partially the colon. The surgical oncologist performed a rectum amputation, a p

Incomplete Facial Paralysis: The Use of the Ipsilateral Residual Facial Nerve as a Donor Nerve for Facial Reanimation.

BACKGROUND: The reconstructive approach for incomplete facial paralysis is not yet determined. In this article, the authors present a new surgical approach for patients with incomplete facial paralysis in which residual, ineffective movement is detected preoperatively in the ipsilateral buccozygomatic territory of the paretic facial nerve. METHODS: Sixteen patients with incomplete facial paralysis were found eligible for the procedure and underwent one-stage facial reanimation performed by the senior author (E.G.). Reanimation was performed using free gracilis muscle transfer with neural coap

The Place of Transanal Endoscopic Surgery in the Treatment of Rectourethral Fistula.

OBJECTIVE: To assess the role of transanal endoscopic operation (TEO) or transanal endoscopic microsurgery (TEM) in rectourethral fistulas (RUF). RUF may appear after radical prostatectomy. Their treatment represents a challenge; many therapies have been proposed, from conservative to aggressive surgical approaches. Transanal endoscopic surgery (TEO or TEM) is a minimally invasive technique to access the site of the RUF to perform repair. MATERIALS AND METHODS: This is an observational study with prospective data collection, conducted between September 2006 and December 2015. All patients wer

Semitendinosus tendon regeneration after harvesting for ACL reconstruction. A prospective MRI study.

Utilisation of the semitendinosus and gracilis tendons in reconstruction of the anterior cruciate ligament (ACL) has become more common during the last few years. In recent studies a regeneration potential in the harvested tendons has been observed. In this study, 11 consecutive patients who underwent ACL reconstruction with a quadruple semitendinosus graft were examined 6-12 months postoperatively by MRI. Another two patients were examined within 2 weeks after surgery. The median age of the patients was 24 years and there were 8 males and 3 females. The right knee was involved in six patients

Identification of prognostic biomarkers in a large cohort of patients with LGMD R2.

BACKGROUND: Limb-girdle muscular dystrophy R2-dysferlin related (LGMD-R2) is a progressive muscle condition with marked variability in disease course, making prognosis challenging. Quantitative MRI (qMRI) has emerged as a complementary tool that may detect progression earlier and more precisely. Integrating different data modalities is challenging with conventional approaches, and artificial intelligence (AI) can help overcome this. Our aim is to develop robust models capable of predicting clinical progression in LGMD-R2 by incorporating AI-based techniques into the analysis pipeline. METHODS

Transperineal approach to complex rectourinary fistulae.

INTRODUCTION: We sought to present our experience and outcomes in patients with complex rectourethral fistulae (RUF) treated using the transperineal approach with gracilis muscle flap interposition. Complex RUF was defined as having prior radiation, failed repair attempts, and large size (>2 cm). METHODS: A retrospective review identified 10 patients presenting with complex RUF between July 2009 and November 2013. Three were excluded due to large fistula defects managed with urinary diversion. Seven patients met inclusion criteria and underwent reconstruction. RESULTS: Six of 7 patients had

Obturator nerve anatomy and relevance to one-stage facial reanimation: limitations of a retroperitoneal approach.

BACKGROUND: Single-stage facial reanimation with a partial gracilis muscle coapted to the contralateral facial nerve seems an optimal surgical solution yet has not supplanted the two-stage approach. Insufficient obturator nerve length may limit reach to sizable contralateral facial nerve branches (possibly necessitating interposition nerve grafting), compromise optimal muscle positioning, or risk nerve coaptation under tension. This study evaluates whether retroperitoneal obturator nerve dissection would effectively lengthen the nerve, thus obviating the aforementioned limitations. METHODS: T

Management of radiotherapy induced rectourethral fistula.

PURPOSE: An increasing number of men are being treated with BT or a combination of external beam radiation therapy and BT for localized prostate cancer. Although uncommon, the most severe complication following these procedures is RUF. We reviewed our recent experience with RUF following radiotherapy for prostate cancer to clarify treatment in these patients. MATERIALS AND METHODS: We recently treated 22 men with RUF following primary radiotherapy for adenocarcinoma of the prostate in 21 and adjuvant external beam radiation therapy following radical prostatectomy in 1. Free functioning gracilis transfer for brachial plexus reconstruction using the internal mammary vessels as recipients: A case report.

Adult brachial plexus injuries are often associated with concomitant trauma to the axillary or subclavian vessels. In patients planned for free functioning gracilis transfer (FFGT) this poses a challenge to reconstructive surgeons where using the standard donor vessels can lead to endangering the circulation in the affected extremity or risk flap loss due to the poor perfusion pressures. This case report describes the use of a FFGT for upper limb reconstruction in a 22-year-old patient with a pan plexus injury and concomitant axillary artery injury following a high energy motorcycle accident.

Muscle MRI in neutral lipid storage disease (NLSD).

Neutral lipid storage disease (NLSD) is a rare inherited disorder of lipid metabolism resulting in lipid droplets accumulation in different tissues. Skeletal muscle could be affected in both two different form of disease: NLSD with myopathy (NLSD-M) and NLSD with ichthyosis (NLSD-I). We present the muscle imaging data of 12 patients from the Italian Network for NLSD: ten patients presenting NLSD-M and two patients with NLSD-I. In NLSD-M gluteus minimus, semimembranosus, soleus and gastrocnemius medialis in the lower limbs and infraspinatus in the upper limbs were the most affected muscles. Gra

Reconstruction of the Urethral Sphincter with Dynamic Graciloplasty in a Male Rabbit Model.

INTRODUCTION: The use of artificial urinary sphincters can improve urinary incontinence after radical prostatectomies; however, complications can arise. We hypothesized that dynamic graciloplasty improves urethral sphincter reconstruction. MATERIALS AND METHODS: Models of urethral sphincter muscle reconstruction were developed in 5 adult rabbits by wrapping the gracilis muscle flap around the urethra. Intra-urethral pressure was measured in each of the models before reconstruction (control), after reconstruction, and after electrical stimulation of the flap in reconstructed models (stimulated

An interdisciplinary approach to the prevention and treatment of groin wound complications after lower extremity revascularization.

BACKGROUND: If not effectively treated, groin wound infections following lower extremity revascularization (LER) may result in graft or limb loss. METHODS: A retrospective review was performed of all patients who underwent muscle flap transposition by a single surgeon after LER between 2006 and 2010. RESULTS: Twenty-nine muscle transposition flaps were performed in 24 patients (21 sartorius, 6 rectus femoris, and 2 gracilis). Nineteen were for treatment of groin wound infections, two for treatment of lymphocele, one for coverage of exposed graft in the setting of pyoderma gangrenosum, and se
