Effect of Intraoperative Low Tidal Volume vs Conventional Tidal Volume on Postoperative Pulmonary Complications in Patients Undergoing Major Surgery: A Randomized Clinical Trial.

IMPORTANCE: In patients who undergo mechanical ventilation during surgery, the ideal tidal volume is unclear. OBJECTIVE: To determine whether low-tidal-volume ventilation compared with conventional ventilation during major surgery decreases postoperative pulmonary complications. DESIGN, SETTING, AND PARTICIPANTS: Single-center, assessor-blinded, randomized clinical trial of 1236 patients older than 40 years undergoing major noncardiothoracic, nonintracranial surgery under general anesthesia lasting more than 2 hours in a tertiary hospital in Melbourne, Australia, from February 2015 to Februa

Association Between Intraoperative Blood Loss Adjusted for Body Size and Postoperative Complication Risk After Hepatectomy: A Multicenter Retrospective Study.

BACKGROUND: While increased intraoperative blood loss (IBL) during hepatectomy has been associated with poorer surgical outcomes, variations in patient body size can obscure the impact of IBL on complications. This study aimed to clarify the relationship between body size-adjusted IBL and complications and to identify clinically relevant thresholds for IBL. METHODS: A retrospective analysis was conducted using a multicenter database of 2,043 patients who underwent hepatectomy for hepatocellular carcinoma. IBL was adjusted by body size factors, including total blood volume (TBV). The IBL/TBV r

Meta-analysis reveals higher intraoperative urologic complication rates in minimally invasive radical hysterectomy compared to abdominal radical hysterectomy.

BACKGROUND: Minimally invasive radical hysterectomy (MIRH) has been increasingly adopted for its benefits of reduced blood loss and shorter hospital stays. This study aims to conduct a meta-analysis to compare the incidence of intraoperative urologic complications, including bladder and ureteral injuries, between MIRH and abdominal radical hysterectomy (ARH). METHOD: The authors systematically searched PubMed, EMBASE, and the Cochrane Library for studies published up to April 2024. A total of 35 studies were analyzed to compute odds ratios (ORs) for intraoperative urologic complications, blad

[Mid-uretral retropubic TVT sling procedure complicated by intraoperative cystotomy (bladder injury): Is it possible to avoid postoperative indwelling catheter?].

OBJECTIVE: Retropubic mid-uretral sling (MUS) procedure may be complicated by bladder injury (intraoperative cystotomy). There is no scientific consensus on the length of catheter drainage following bladder injury during MUS procedure: it varies from hours to days. We have made it our policy to immediately remove the catheter. The objective of the current study was to assess the results associated with immediate removal of catheter drainage following intraoperative bladder injury during retropubic MUS procedure. METHODS: Retrospective case-control study. Group 1 (cases): 8 women who have expe

Giant bullous emphysema successfully treated with percutaneous drainage followed by resection: A case complicated by lung cancer diagnosed by intraoperative biopsy.

We present a case of bilateral giant bullous emphysema (GBE) with rapidly progressive dyspnea. The dyspnea was thought to be due to tension bullae caused by the check valve mechanism in COVID-19 bronchitis. Multiple nodules were also detected on both sides of the lung. As the patient had poor pulmonary reserve for surgical bullectomy, we first performed percutaneous intracavitary drainage. Prior to this procedure, we placed a chest tube in the thoracic cavity to avoid tension pneumothorax. As a result, the patient's remaining lung expanded and respiratory status improved, allowing him to under

Effect of pressure-controlled ventilation-volume guaranteed mode on intraoperative respiratory mechanics, oxygenation and postoperative pulmonary complications: a systematic review and network meta-analysis.

BACKGROUND: Ventilation strategies during general anesthesia are crucial for optimizing respiratory mechanics, ensuring adequate oxygenation, and minimizing postoperative pulmonary complications (PPCs) in surgical patients. This systematic review and network meta-analysis (NMA) aim to evaluate the effect of pressure controlled volume guaranteed ventilation (PCV-VG) in patients undergoing non-cardiothoracic surgery. METHODS: We selected randomized controlled trials (RCTs) to explore the effect of PCV-VG mode on intraoperative respiratory mechanics, oxygenation and PPCs in surgical patients pub

Thrombotic and hemorrhagic complications during visceral transplantation: risk factors, and association with intraoperative disseminated intravascular coagulation-like thromboelastographic qualities: a single-center retrospective study.

This study describes the risk of thrombotic and hemorrhagic complications, both intraoperatively, and up to 1 month following visceral transplantation. Data from 48 adult visceral transplants performed between 2010 and 2017 were retrospectively studied [32 multivisceral (MVTx); 10 isolated intestine; six modified-MVTx]. Intraoperatively, intracardiac thrombosis (ICT)/pulmonary embolism (PE) occurred in 25%, 0% and 0% of MVTx, isolated intestine and modified MVTx, respectively, and was associated with 50% (4/8) mortality. Preoperative portal vein thrombosis (PVT) was a significant risk factor f

Impact of Staple Line Oversewing on the Rate and Severity of Postoperative Complications and Intraoperative Events After Sleeve Gastrectomy: A Retrospective Study of 2,391 Patients.

BACKGROUND: Staple line complications following laparoscopic sleeve gastrectomy (LSG) remain a significant source of perioperative morbidity. While reinforcement strategies have been proposed to reduce these events, evidence regarding their true benefit remains inconsistent. This study evaluates the impact of systematic staple line oversewing on intraoperative and postoperative outcomes using standardized classification systems. METHODS: Retrospective cohort study of 2,391 patients undergoing primary LSG at a single center (2014-2025), divided into non-reinforced (NSG, n = 583) and reinforced

Comparison of postoperative pulmonary complications and intraoperative safety in thoracoscopic surgery under non-intubated versus intubated anesthesia: a randomized, controlled, double-blind non-inferiority trial.

PURPOSE: Traditional anesthesia for video-assisted thoracoscopy (VATS) such as double-lumen tracheal intubation (DLT) and one-lung ventilation (OLV), may lead to post-operative pulmonary complications (PPCs). Non-intubation VATS (NIVATS) is an anesthetic technique that avoided DLT and OLV, maybe avoiding the PPCs. So we hypothesized that NIVATS would non-inferiority to intubation VATS (IVATS) in the risk of developing PPCs and some safety indicators. METHODS: This study is a randomised, controlled, double-blind, non-inferiority trial, 120 patients were randomly assigned to the NIVATS group an

Intraoperative protective ventilation with or without periodic lung recruitment manoeuvres on pulmonary complications after major abdominal surgery (REMAIN-1): protocol for a randomised controlled trial.

INTRODUCTION: Postoperative pulmonary complications (PPCs) are frequent after abdominal surgery and significantly affect postoperative outcomes. Intraoperative protective ventilation (IPV) has been demonstrated to mitigate PPCs. However, the comparative effectiveness of two common IPV regimens-IPV with or without periodic lung recruitment manoeuvres (PLRM)-in preventing PPCs is unclear. This study aims to compare the effects of these two IPV regimens on PPCs. METHODS AND ANALYSIS: This study is a prospective, double-blinded, randomised controlled trial. A total of 1060 patients at intermediat
