Tunneled Hemodialysis Catheter Making its Way into the Spinal Canal: An Unusual Complication.

The procedure of tunnelled hemodialysis catheter insertion is generally considered safe. We report the case of a lady who had a malpositioned femoral tunneled hemodialysis catheter that entered into the spinal canal causing neurological weakness. The catheter was later safely removed leading to partial neurological recovery.

Deep learning-based detection of lumbar spinal canal stenosis using convolutional neural networks.

BACKGROUND CONTEXT: Lumbar spinal canal stenosis (LSCS) is the most common spinal degenerative disorder in elderly people and usually first seen by primary care physicians or orthopedic surgeons who are not spine surgery specialists. Magnetic resonance imaging (MRI) is useful in the diagnosis of LSCS, but the equipment is often not available or difficult to read. LSCS patients with progressive neurologic deficits have difficulty with recovery if surgical treatment is delayed. So, early diagnosis and determination of appropriate surgical indications are crucial in the treatment of LSCS. Convolu

[Biomechanical analysis of miniplate fixation systems in restorative laminoplasty for spinal canal reconstruction].

OBJECTIVE: To investigate the biomechanical properties of H-shaped and L-shaped miniplate fixation systems (H-MFS and L-MFS, respectively) in restorative laminoplasty for spinal canal reconstruction (RL-SCR). METHODS: Laminectomy was performed in a 3D printed L4 vertebral model followed by RL-SCR using H-MFS or L-MFS, and the biomechanical properties of the reconstructed models were evaluated using static and dynamic compression tests. Biomechanical analyses of RL-SCR were also conducted in finite element models of the L3-L5 vertebrae with normal assignment (NA), laminectomy, or fixation with

An unusual case of pediatric cervical myelopathy due to congenital spinal canal stenosis.

Cervical myelopathy is a condition that is rarely reported in pediatric patients who have movement or neuromuscular disorders. We, herein, present a rare case of cervical myelopathy observed in a 14-year-old patient, who was previously a healthy boy treated with cervical laminoplasty, which was caused by cervical spinal canal stenosis based on multiple level disc herniation. The patient presented to the clinic with spastic and ataxic gait with previous diagnostic challenges. Magnetic resonance imaging showed cervical degenerative changes mainly marked at the C3-C4 and C4-C5 levels, along with

Retroperitoneal alveolar rhabdomyosarcoma intruding into spinal canal: A case report and literature review.

BACKGROUND: Rhabdomyosarcoma (RMS) is the most frequent soft sarcoma in children and adolescents. Alveolar rhabdomyosarcoma (ARMS) is a relatively rare subtype that is characterized by aggressive behavior and an unsatisfactory prognosis. An ARMS can arise anywhere but most commonly occurs at extremity sites with a very small fraction in the retroperitoneum. The utility of 2-Deoxy-2-[fluorine-18]-fluoro-D-glucose (18F-FDG) positron emission tomography combined with computed tomography (PET/CT) remains to be established in ARMS. CASE REPORT: A 3-year-old female child was accidentally found with

Hereditary Transthyretin Cardiac Amyloidosis in Refractory Spinal Canal Stenosis: Genetic and Pathologic Analysis.

Transthyretin (TTR) cardiac amyloidosis is a systemic disease caused by amyloid deposits in several systemic organs. The thickening of the yellow ligament because of amyloid deposits results in spinal canal stenosis. This study reports a case of mutant-type TTR cardiac amyloidosis in a patient with recurrent spinal canal stenosis. To our knowledge, this is the first study to report pathologic amyloid involvement in both spinal and cardiac tissues of a patient with hereditary TTR amyloidosis. It is important to be aware of underlying amyloidosis as a cause of spinal canal stenosis, especially r

How I do it: bilateral lumbar spinal canal microsurgical decompression via unilateral approach.

BACKGROUND: Bilateral lumbar spinal canal decompression via unilateral approach is a surgical way to treat degenerative spinal canal stenosis. METHOD: We report the treatment of degenerative lumbar spinal canal stenosis by removing overgrown ligaments, bone, and other compromising tissue on both sides of the spinal canal, using one side approach, avoiding surgical trauma of the counter side of the spine. CONCLUSION: This technique allows to achieve perfect results using common microsurgical instruments and Caspar distractor for one or multilevel surgery.

Risk factors of degenerative lumbar scoliosis in patients with lumbar spinal canal stenosis.

Lumbar spinal canal stenosis (LSCS) associated with degenerative scoliosis has being increasingly aware by the public and studied by many researchers. Degenerative changes leading to spinal stenosis can precede a spinal deformity which will develop into the de novo scoliosis. There are few studies focusing on the risk factors contributing to the degenerative lumbar scoliosis (DLS) in lumbar spinal stenosis patients. From September, 2017 to December, 2017, 181 patients who were diagnosed with LSCS in the outpatient department of our hospital were enrolled in this retrospective investigation. The

Surgical treatment of multiple spinal canal stenoses associated with vitamin D-resistant rickets.

We describe a 44-year-old woman who was diagnosed in childhood with vitamin D-resistant rickets, and who had paraparesis due to multiple spinal canal stenoses between C5 and L1 with ossification of the posterior longitudinal ligament and the yellow ligament. She was treated surgically with laminoplasty of the C2 through C7 levels and laminectomy from T8 through T11. Four months later, she underwent anterior fusion using an ilium graft by thoracotomy from the T12 to L1 levels. Six months after surgery, her symptoms improved. After 5 years, and with oral vitamin D, no progression of symptoms has

Thoracoscopic vertebral body replacement with an expandable cage after ventral spinal canal decompression.

OBJECTIVE: Minimally invasive thoracic anterior surgery using a thoracoscopic approach has evolved to include spinal biopsy, debridement, discectomy, decompressive corpectomy, interbody fusions, and internal fixations. Minimal access techniques can potentially decrease surgical access morbidity and also reduce the time required for recovery and healing. The thoracoscopic approach for decompression, stabilization, and anterior vertebral reconstruction of thoracolumbar fractures is described. METHODS: In this article and video, we discuss patient selection, surgical positioning, port placement,

Cement leakage into the posterior spinal canal during balloon kyphoplasty: a case report.

We report a case of cement leakage into the posterior spinal canal due to inadvertent pedicle perforation during balloon kyphoplasty. The leakage was corrected immediately without any sequelae. Features seen on radiography and the minimally invasive procedure used for removal are described. The postoperative radiographs of 100 consecutive patients treated with balloon kyphoplasty were subsequently reviewed. Only one patient had a similar leakage but had no neurological complications.

Lumbar spinal canal morphometry from computed tomography scans: reproducibility, results and clinical implications.

The reproducibility of measurements of the lumbar spine taken from computed tomography (CT) scans was assessed. The overall variaton in 53 patients scanned on more than one occasion was only 2.8%. Factors which reduced the reproducibility were increasing angle of scan from the mid-discal plane, measurement of oblique structures and measurement of dimensions with soft-tissue boundaries. Measurements from 142 patients are presented and compared with both other published series and the results from 17 patients with spinal stenosis. Whereas the sagittal diameter of the bony canal and the interpedi

Redundant nerve roots of the cauda equina caused by lumbar spinal canal stenosis.

To investigate pathogenesis of redundant nerve roots of the cauda equina, which were concomitant with severe lumbar spinal canal stenosis, six cadavers were examined anatomically and histopathologically, and quantitative analysis of nerve fibers was performed. In this anatomic study, it was observed that all the redundant nerve roots passed through the constriction of the spinal canal. No significant pathologic change was detected in the spinal ganglia and in the spinal cords except for the posterior column, in which dorsal redundant roots were entering. Redundant nerve roots of unequal length

Myeloscopic study on lumbar spinal canal stenosis with special reference to intermittent claudication.

Twenty-five cases of lumbar spinal canal stenosis were investigated by dynamic myeloscopy to explain the pathophysiologic mechanism of intermittent claudication, one of the characteristic signs of the disorder. Myeloscopic examination revealed that, in accordance with postural alteration, the diameter of blood vessels on the cauda equinae showed significant changes in many patients in the stenosis group, but showed no changes in control group. In this series, it was suggested that the microcirculatory disturbance of vessels on cauda equinae might play an important role in the development of in

Relationship between traumatic spinal canal stenosis and neurologic deficits in thoracolumbar burst fractures.

Using CT scans of 112 consecutive patients with thoracolumbar burst fractures, we investigated the relationship between traumatic spinal canal stenosis and neurologic deficits. We calculated the stenotic ratios of the area occupied by the retropulsed bony fragments to the estimated area of the original spinal canal. We also examined the shape of the narrowed canal and the disruption of spinal elements. Burst fractures having the following ratios are at significant risk of neurologic involvement: at T11 to T12 with 35% more, at L1 with 45% or more, and at L2 and below with 55% or more.
