Forecasting the Workforce of Uveitis Specialists.

OBJECTIVE: The uveitis subspeciality faces workforce inadequacy in the setting of a rising burden of uveitis coupled with a decline in the number of specialists. While workforce projections exist for other ophthalmologic sub-specialties, none have modeled the future supply of uveitis specialists. The objective of our study was to characterize the current workforce and project its future supply in the United States through 2055. DESIGN: Simulation projection study using retrospective trend data. SUBJECTS: AUS-Registered uveitis specialists in the United States. METHODS: We identified 224 spe…

[Biomarkers in tear fluid in uveitis].

The etiology of uveitis, choroid inflammation, is diverse, the disease is often recurrent, difficult to treat, and frequently results in disability at a young age. Studies investigating the tear fluid composition in uveitis have revealed promising biomarkers relevant for prognosis and treatment optimization. This review presents literature data on changes in the tear fluid content of proteins involved in local immune responses, intercellular interactions, proteolytic and free radical processes, nitric oxide metabolism, and other metabolic pathways in different forms of uveitis. The paper also …

Is there a predisposition to uveitis in Turner syndrome?

INTRODUCTION: Autoimmunity is prevalent in Turner syndrome (TS) though uveitis is rarely reported. A definite link between TS and uveitis is not yet established. METHODS: We report two cases of uveitis with a history of TS and review the literature regarding TS, uveitis and autoimmunity. RESULTS: TS-associated uveitis is acute (100%), non-hypertensive (100%) anterior uveitis (87.5%) that usually responds to topical therapy without unexpected long-term visual sequelae. Systemic treatment is uncommonly required as relapses are infrequent. CONCLUSION: Reported cases of uveitis in TS were acute…

Spondyloarthritis Associated with Uveitis: A Review.

Spondyloarthritis (SpA) is a group of chronic inflammatory rheumatism characterized by common clinical, radiological, and biological manifestations occurring on a predisposing genetic background dominated by the HLA-B27 antigen. Acute anterior uveitis is the most common extra-articular feature of SpA. The objective of this review is to describe the prevalence, demographic characteristics, factors favoring the occurrence of uveitis in patients with SpA, clinical manifestations, and their therapeutic management.

Primary pyomyositis and uveitis in a cat.

A 6-year-old neutered male Siamese cat was referred for investigation of hindlimb ataxia and blindness of 2 weeks' duration. A swollen right hind limb, with no history of trauma, and no evidence of an external wound, was observed on physical examination. Ophthalmic examination revealed bilateral absence of the menace response and changes consistent with uveitis. Blood tests identified changes consistent with inflammation including serum amyloid A elevation. Infectious disease testing was negative. Degenerate neutrophils and bacterial cocci were detected on fine needle aspiration cytology of th…

Pediatric uveitis: Role of the pediatrician.

The challenges of childhood uveitis lie in the varied spectrum of its clinical presentation, the often asymptomatic nature of disease, and the evolving nature of the phenotype alongside normal physiological development. These issues can lead to delayed diagnosis which can cause significant morbidity and severe visual impairment. The most common ocular complications include cataracts, band keratopathy, glaucoma, and macular oedema, and the various associated systemic disorders can also result in extra-ophthalmic morbidity. Pediatricians have an important role to play. Their awareness of the var…

Uveitis in Patients with Ankylosing Spondylitis.

BACKGROUND: There is a close correlation between ankylosing spondylitis and uveitis, other less common ocular manifestations being episcleritis and scleritis. Though the prognosis of uveitis is good, prompt treatment is required to prevent long-term complications. This study aimed to estimate the prevalence of uveitis in patients with ankylosing spondylitis who follow outpatient clinic of a tertiary care hospital. METHODS: A descriptive cross-sectional study was done in medicine outpatient department of a tertiary care hospital for 6 months. Ethical clearance was obtained from the institution…

Psoriasis and uveitis: links and risks.

Uveitis, an inflammatory disorder of the mid-portion of the eye, is considered a relatively rare but very serious ocular complication of psoriasis. Data on the specific characteristics of uveitis in the background of psoriasis are extremely limited. The presence of uveitis in the context of psoriasis has been estimated to occur in 7-20% of the psoriasis cases. This incidence tends to be higher in patients suffering from psoriasis and psoriatic arthritis (PsA) or PSA alone. Psoriatic uveitis is usually bilateral, chronic, and severe. In term of pathogenesis, both psoriasis and uveitis are consi…

Epidemiology of Macular Edema in Uveitis.

PURPOSE: To describe the epidemiology of macular edema (ME) in patients with uveitis. METHODS: Review of articles listed on PubMed pertaining to uveitic ME. RESULTS: Reported rates of uveitic ME ranging from 20% to 70%, depending on the ancillary tests used (fundus examination, fluorescein angiography, optical coherence tomography). Macular edema might develop due to uveitis itself, or occur as an adverse effect of drugs taken for different diseases. It is more frequently observed in adults than in children, in chronic uveitis, and in intermediate uveitis. Males with ankylosing spondylitis a…

A Rare Cause of Uveitis: Vemurafenib.

A 25-year-old female presented with a decrease of vision and redness in both eyes. She had a history of nodular melanoma in her right shoulder, which was excised surgically and she was under oral vemurafenib treatment. She was diagnosed with moderately severe bilateral panuveitis and hospitalized for systemic investigation and workup. The laboratory test results were unremarkable and systemic workup failed to reveal an etiology. The condition was considered vemurafenib-induced uveitis, as the drug is known to be associated with uveitis. After reevaluation with the oncology department, vemurafe…

Tofacitinib for refractory uveitis and scleritis.

PURPOSE: To report the successful use of tofacitinib in the treatment of refractory uveitis and scleritis. OBSERVATIONS: Two patients, one with scleritis and another with anterior and intermediate uveitis, presented with refractory disease after failure of multiple steroid-sparing therapies. Treatment with tofacitinib led to durable resolution of uveitis and scleritis. CONCLUSIONS AND IMPORTANCE: Tofacitinib is a potential novel treatment option for refractory, noninfectious inflammatory eye disease.
