John Caffey and the birth of pediatric radiology: from accidental radiologist to architect of a discipline.

BACKGROUND: John Patrick Caffey (1895-1978) is widely regarded as among the foundational figures of pediatric radiology. The translational scope of his contributions, spanning textbook authorship, forensic radiology, and the evidentiary basis for child protection legislation, has rarely been examined as a coherent whole. OBJECTIVE: To examine Caffey's contributions across historical, technical, clinical, and medicolegal dimensions, including the controversies surrounding shaken infant syndrome that complicate his later legacy, and to assess the continued relevance of his methodological princi

Hemispherotomy for Pediatric Post-Traumatic Epilepsy.

Objective: Hemispherotomy is an effective treatment for select forms of drug-resistant hemispheric epilepsy, including perinatal stroke, Rasmussen's encephalitis, and Sturge-Weber syndrome. Post-traumatic epilepsy (PTE) has been reported to occur in ~10% of children following traumatic brain injury (TBI). TBI has not been extensively evaluated as an indication for hemispherotomy, as its effects are rarely unilateral. Here, we report the results from five pediatric cases of hemispherotomy for drug-resistant hemispheric PTE. Methods: A retrospective review was performed of all pediatric patients

Automated scoring of the Ambiguous Intentions Hostility Questionnaire with fine-tuned large language models.

Hostile attribution bias is the tendency to interpret social interactions as intentionally hostile. The Ambiguous Intentions Hostility Questionnaire (AIHQ) is a commonly used instrument to measure hostile attribution bias and includes open-ended questions where participants describe the perceived intentions behind a negative social situation and how they would respond. While these questions provide insights into the contents of hostile attributions, they require time-intensive scoring by human raters. In this study, we assessed whether large language models can automate the scoring of AIHQ ope

Rapid-onset respiratory failure following diesel aspiration at 4,200 m: a case report.

BACKGROUND: Oral siphoning of diesel fuel carries substantial risks of chemical inhalation injury; however, documented cases of diesel aspiration pneumonia, especially at altitudes >2,500 m, remain rare. The synergistic effects of hydrocarbon toxicity and hypobaric hypoxia in such environments are inadequately characterized. CASE PRESENTATION: A 23-year-old man accidentally aspirated approximately 10 mL of diesel while siphoning a fuel line at 4,200 m altitude. Within 5 h, he developed nausea, vomiting, and dry cough, progressing to right-sided pleuritic chest pain, fever (38.2 °C), and hypox

N-acetylcysteine attenuates oxidative-stress-associated apoptosis and collagen deposition after rat hindlimb ischemia-reperfusion injury.

Proper use of fingers and limbs is crucial for people to carry out normal activities of daily living. Therefore, when fingers or limbs are severed accidentally, replantation is attempted whenever possible. However, even when replantation is successful and fingers or limbs are preserved, functional limitations often arise due to contractures in reattached fingers or limbs, although mechanisms underlying contractures or countermeasures to ameliorate them are not well understood. Here, using a rat femoral artery ischemia-reperfusion (I/R) model, we show that oxidative stress caused by accumulatio

Intraoperative neurophysiological monitoring and spinal cord stimulator implantation.

INTRODUCTION: Spinal cord injury (SCI) is one of the most dreaded complications after spinal cord stimulation (SCS) implantation surgery. As a result, intraoperative neurophysiological monitoring (IONM) has been proposed to avoid accidental damage to nervous structures under anesthesia and confirm positioning for optimal stimulation. Our study uses a large administrative claims database to determine the 30-day risk of SCI after SCS implantation. METHODS: This retrospective cohort study used the IBM MarketScan Commercial and Medicare Supplemental Databases from 2016 to 2019. Adult patients und

A Novel and Low-cost Approach for Intravitreal Injection in an Experimental Model of Endophthalmitis.

PURPOSE: Animal models are necessary in understanding the pathogenesis of endophthalmitis and are also necessary to assist the development of new therapeutics for this sight-threatening ocular inflammation. Hamilton syringes are usually preferred to inject pathogens when performing experiments on test subjects, however, this method has technical and financial disadvantages. In this study, we report the findings and assess the related benefits of applying a novel low-cost intravitreal injection technique to initiate endophthalmitis in a mouse model while using the Eppendorf tip and a 26G needle

Pulmonary pathogenesis in a murine model of inhaled arsenical exposure.

Arsenic trioxide (ATO), an inorganic arsenical, is a toxic environmental contaminant. It is also a widely used chemical with industrial and medicinal uses. Significant public health risk exists from its intentional or accidental exposure. The pulmonary pathology of acute high dose exposure is not well defined. We developed and characterized a murine model of a single inhaled exposure to ATO, which was evaluated 24 h post-exposure. ATO caused hypoxemia as demonstrated by arterial blood-gas measurements. ATO administration caused disruption of alveolar-capillary membrane as shown by increase in

Health in Men Study: is frailty a predictor of medication-related hospitalization?

BACKGROUND: Older adults are at high risk of medication-related hospitalizations. Frailty is a phenotype commonly observed in older people due to declining physiological functions. AIM: To examine the association of frailty with medication-related hospitalization among community dwelling older men. METHODS: A prospective observational cohort study was conducted among community dwelling older men (mean age 75.6 years SD 5.9) from Western Australia (4324) who participated in the Health in Men Study. Participants were followed-up at 12 and 24 months to determine adverse drug event-related hospi

Amifostine analog, DRDE-30, alleviates radiation induced lung damage by attenuating inflammation and fibrosis.

BACKGROUND: Radiotherapy of thoracic neoplasms and accidental radiation exposure often results in pneumonitis and fibrosis of lungs. Here, we investigated the potential of amifostine analogs: DRDE-07, DRDE-30, and DRDE-35, in alleviating radiation-induced lung damage. METHODS: C57BL/6 mice were exposed to 13.5 Gy thoracic irradiation, 30 min after intraperitoneal administration of the analogs, and assessed for modulation of the pathological response at 12 and 24 weeks. KEY FINDINGS: DRDE-07, DRDE-30 and DRDE-35 increased the survival of irradiated mice from 20% to 30%, 80% and 70% respective

Anterior tibial artery pseudoaneurysm as a rare technical complication after corrective fibular osteotomy: a case report.

INTRODUCTION: Partial fibular osteotomy has been recognized as a surgical alternative to treat medial compartment osteoarthritis of the knee. Related peroneal neuropathies are of concern among the relatively few complications after this procedure. In our clinical practice, the osteotomy level has therefore been modified to avoid the above defects. However, a rare case of vascular injury was encountered. Herein we describe an accidental anterior tibial artery pseudoaneurysm as a rare technical complication after this corrective osteotomy. CASE PRESENTATION: A 55-year-old male visited our emerg

A case report: use of cerebral oximetry in the early detection of cerebral hypoperfusion in a post-cardiac arrest patient during targeted temperature management.

BACKGROUND: We present a patient who received cerebral oximetry monitoring during targeted temperature management (TTM) post-cardiac arrest and discuss its potential in the early detection of cerebral hypoperfusion and implications on haemodynamics and ventilatory management. CASE SUMMARY: A 60-year-old Chinese male was admitted for acute pulmonary oedema with Type 2 respiratory failure. He failed an initial trial of non-invasive ventilation and was planned for intubation and mechanical ventilation. However, the patient suffered a pulseless electrical activity cardiac arrest peri-intubation.

Feasibility of photoacoustic-guided teleoperated hysterectomies.

Hysterectomies (i.e., surgical removal of the uterus) are the prevailing solution to treat medical conditions such as uterine cancer, endometriosis, and uterine prolapse. One complication of hysterectomies is accidental injury to the ureters located within millimeters of the uterine arteries that are severed and cauterized to hinder blood flow and enable full uterus removal. This work explores the feasibility of using photoacoustic imaging to visualize the uterine arteries (and potentially the ureter) when this imaging method is uniquely combined with a da Vinci® surgical robot that enables te

Luteolin-mediated Kim-1/NF-kB/Nrf2 signaling pathways protects sodium fluoride-induced hypertension and cardiovascular complications.

The use of sodium fluoride (NaF) as a major ingredient for tooth paste, mouth wash, and mouth rinse has become inevitable in our day-to-day life. However, flavonoids such as Luteolin might be of great value in the prevention of toxicity associated with accidental or inevitable ingestion of NaF. In the study, 40 male Wistar albino rats were randomly divided into four groups with 10 rats in a group. Group A was the control group and received normal saline, Group B was exposed to NaF at 300 ppm (300 mg/L) in drinking water daily for a week, Groups C and D were exposed to 300 ppm (300 mg/L) of NaF

Transesophageal Echocardiography Complications in Adult Cardiac Surgery: A Retrospective Cohort Study.

BACKGROUND: Although there have been several large reviews documenting the complications following intraoperative transesophageal echocardiography (TEE), most of these prior reports are almost 2 decades old and may not reflect current practices. The purpose of this study was to determine the incidence and types of complications following TEE in a contemporary cardiac surgical population. METHODS: We conducted a retrospective analysis of all cardiac surgical patients having undergone an intraoperative TEE between April 1, 2004, and April 30, 2012. Patients with TEE-related complications were i

Intraoperative esophagoscopy provides accuracy and safety in video-assisted thoracoscopic enucleation of benign esophageal submucosal tumors.

Benign esophageal tumors are rare; complete surgical resection is essential for the management of the submucosal tumors. Larger, symptomatic, or non-diagnostic lesions should be resected for both diagnostic and therapeutic indications. Video-assisted thoracic surgery has become a popular treatment in the field of thoracic surgery; however, thoracoscopic esophageal surgery may lead to an increase in operative complications. The effect and safety of thoracoscopic surgery for esophageal submucosal lesions were evaluated. A retrospective study evaluated patients undergoing thoracoscopic treatment
