BACKGROUND: Acellular human dermal matrix grafts (Graftjacket; Wright Medical Technology, Arlington, TN, USA) are used clinically for rotator cuff augmentation without a detailed understanding of their biomechanical effects. The purpose of this study was to evaluate the effect of augmentation with dermal grafts on the biomechanical effects of rotator cuff repairs.
METHODS: Nine matched pairs of human cadaveric shoulders were used. A single-row rotator cuff repair combined with an augmentation graft was performed on one shoulder, and a single-row repair was performed on the contralateral shoul

BACKGROUND: Rotator cuff repair is a commonly performed procedure, but many of these repairs fail in the postoperative term. Despite advances in surgical methods to optimize the repair, failure rates still persist clinically, thereby suggesting the need for novel mechanical or biological augmentation strategies. Nonresorbable implants provide an appealing approach because patch materials may confer acute mechanical stability and act as a conductive scaffold for tissue ingrowth at the site of the tendon insertion.
HYPOTHESIS: The polyurethane scaffold mesh will confer greater biomechanical fun

PURPOSE: This describes a child whose neonatal brachial plexus injury was treated with kinesiotape and exercise.
DESCRIPTION: The subject was a two-year-old female whose X-rays demonstrated severe inferior subluxation of the humeral head and winging of the scapula on the left. She was fitted with a shoulder brace with surgery scheduled in six months. The initial PT exam noted 80 degrees of shoulder abduction (trumpet sign), significant asymmetry, and nonuse. Mallet score was 15/25. Treatment consisted of d/c of the brace and E-stimulation, parent education on exercise and taping, and kinesiot

PURPOSE OF THE STUDY: The goal of this retrospective study is to describe the different anatomic lesions of the subscapularis and to precise the diagnostic value of the clinical and imaging tests.
MATERIAL: Twenty-one cases of isolated tear of the subscapularis were operated on between 1989 and 1992. Affecting both sex, this lesion happens in younger patients than the rotator cuff tear concerning supra or infra-spinatus. The onset was most often traumatic but not specific. Four patients had no traumatic history.
RESULTS: In 16 cases, the complaint was an unspecific chronic painful shoulder.

BACKGROUND: ABO blood groups are associated with various diseases. A relationship between Achilles tendon ruptures and blood type O has been reported, although its pathogenesis was not clear. To the best of our knowledge, there is no published study describing the relationship between blood type and rotator cuff tendon tears.
OBJECTIVE: To determine whether patients with rotator cuff tear had a greater prevalence of blood type O than those without rotator cuff tear.
DESIGN: A cross-sectional study.
SETTING: Research hospital outpatient evaluation.

BACKGROUND: It is unknown at which stage of rotator cuff tear the biomechanical environment is altered. The purpose of this study was to determine if a critical rotator cuff tear stage exists that alters glenohumeral joint biomechanics throughout the rotational range of shoulder motion, and to evaluate the biomechanical effect of parascapular muscle-loading.
METHODS: Eight cadaver shoulders were used with a custom testing system. Four progressive rotator cuff tear stages were investigated on the basis of footprint anatomy. Three muscle-loading conditions were examined: rotator cuff only; rota

BACKGROUND: Previous studies have demonstrated varying correlations between Workers' Compensation status and the outcome of rotator cuff repair. However, none of those studies have formally accounted for potential confounding factors with multivariable analysis. We hypothesized that patients with Workers' Compensation claims who undergo rotator cuff repair have worse outcomes, even after controlling for confounding factors.
METHODS: One hundred and twenty-five patients (including thirty-nine with Workers' Compensation claims) who underwent unilateral primary repair of a chronic rotator cuff t

BACKGROUND: Graft bridging (GB) and long head of the biceps tendon (LHBT)-based reconstructive techniques have been used to manage massive rotator cuff tears (MRCTs); however, their clinical outcomes remain variable. In this study, we explored a combined strategy of autogenous fascia lata graft bridging with LHBT transposition. The aim was to compare the effectiveness of this combined approach with partial repair in the treatment of MRCTs.
METHODS: This retrospective comparative study evaluated two surgical treatments for MRCTs (> 5 cm or ≥ 2 tendons involved, Hamada grade 1-3): partial repai

BACKGROUND: Corticosteroid injections and physiotherapy exercise programmes are commonly used to treat rotator cuff disorders but the treatments' effectiveness is uncertain. We aimed to compare the clinical effectiveness and cost-effectiveness of a progressive exercise programme with a single session of best practice physiotherapy advice, with or without corticosteroid injection, in adults with a rotator cuff disorder.
METHODS: In this pragmatic, multicentre, superiority, randomised controlled trial (2 × 2 factorial), we recruited patients from 20 UK National Health Service trusts. We include

BACKGROUND: Rotator cuff arthropathy with loss of active arm elevation can be successfully treated with nonanatomic reverse total shoulder arthroplasty to restore active elevation. Shoulder kinematics in this context predominantly focus on glenohumeral motion, neglecting scapular motion, although both substantially contribute to global shoulder motion. Because scapular kinematics are difficult to assess clinically and in the laboratory, they are not well understood and therefore are often reduced to glenohumeral models with a static scapula.

BACKGROUND: Repair of rotator cuff tears has yielded excellent functional outcomes in recent decades; however, poor outcomes and dissatisfaction have been noted in specific groups. Spontaneous tendon rupture has been reported in patients receiving long-term hemodialysis owing to alteration of tendon structure, which might impede functional recovery after rotator cuff repair. The purpose of the present study was to compare the clinical outcomes between hemodialysis and non-hemodialysis patients after rotator cuff repair.
METHODS: We retrospectively reviewed patients who underwent mini-open rot

PURPOSE: To identify any dose-dependent association between the use of subacromial corticosteroid injections within a year before rotator cuff repair (RCR) and subsequent need for revision rotator cuff surgery.
METHODS: Two large administrative databases were queried for patients undergoing arthroscopic RCR. A minimum of 1 year of preoperative database exposure and 2 years of postoperative database follow-up were required for inclusion. Patients were stratified into groups that received 0 (control), 1, 2, or 3 or more ipsilateral corticosteroid shoulder injections within the year prior to RCR

Aims This study aimed to compare the clinical outcomes of rotator cuff repair in patients with rheumatoid arthritis with those of patients who have no known history of the disease. We hypothesised that the functional outcomes are comparable between patients and without rheumatoid arthritis and may be affected by the level of disease activity, as assessed from C-reactive protein (CRP) level and history of systemic steroid intake. Patients and methods We conducted a retrospective review of the institutional surgical database from May 1995 to April 2012. Twenty-nine patients with rheumatoid arthr

We investigated the function of biceps in 18 patients (19 shoulders) with lesions of the rotator cuff. Their mean age was 59 years. Another series of 18 patients (19 shoulders) with normal rotator cuffs as seen on MRI acted as a control group. Their mean age was 55 years. A brace was used to maintain contraction of biceps during elevation. Anteroposterior radiographs were obtained with the arm elevated at 0 degrees , 45 degrees and 90 degrees with and without contraction of biceps. The distance between the centre of the head of the humerus and the glenoid was compared in the two groups. We fou
