Internal impingement of the shoulder in overhead athletes: Retrospective multicentre study in 135 arthroscopically-treated patients.

BACKGROUND: Internal impingement of the shoulder (IIS) is the leading cause of chronic shoulder pain in overhead throwing athletes. No consensus exists about which techniques are optimal when surgery is in order. The available studies are limited by small sample sizes and short follow-ups. The primary objective of this study was to assess return-to-sports (RtS) outcomes after surgical treatment for IIS. A favourable RtS outcome (RtS+) was defined as returning to the previous sport at the same or a higher level. HYPOTHESIS: The main hypothesis was that surgical treatment resulted in an RtS+ ou

Arthroscopically-Assisted Latissimus Dorsi Transfer for Irreparable Rotator Cuff Insufficiency: Modes of Failure and Clinical Correlation.

PURPOSE: The main objective of this retrospective study was to analyze the rate and modes of failure of latissimus dorsi transfer (LDT). The secondary objective was to evaluate whether a rupture of the transfer was associated with a worse outcome. METHODS: During a 2-year period, we performed consecutive LDTs either for irreparable posterior-superior rotator cuff tears (RCTs) or for failed prior repair. All the LDTs were performed by a single surgeon. All transfers were arthroscopically assisted and fixed as a tubularized LD tendon in a bone tunnel inside the humeral head. Three metal clips w

The treatment of primary anterior shoulder dislocations.

The treatment of primary anterior shoulder dislocations is complex and challenging. The goal of treatment is to achieve a stable, pain-free shoulder with a full range of motion. The currently available evidence on the outcomes of nonsurgical treatment and immediate surgical stabilization conflicts; decision making must also take into account patient-specific variables, including age, occupation, functional demands, sports participation, physical characteristics, and compliance. Although recurrence rates after anterior shoulder dislocation are difficult to pinpoint, abundant data indicate that

Proximal Humerus Fracture/Dislocation: Look for the Greater Tuberosity.

The shoulder is the commonest major joint involved in dislocations. These are often associated with fractures of the surgical neck and/or of the greater tuberosity of the proximal humerus. A good functional recovery is associated with a successful union of the tuberosity fragment, as this carries the insertion of the superior and posterior rotator cuff tendons. A 29-year-old male patient presented to our Emergency Department (ED) after a fall off his motorbike, resulting in a left shoulder fracture dislocation and an axillary nerve injury. His shoulder was reduced under sedation in the ED, wit

Complications and Frequency of Surgical Treatment with AO-Type Hook Plate in Shoulder Trauma: A Retrospective Study.

We investigated the complications and frequency of hook plate fixation in patients with shoulder trauma. We reviewed 216 cases of hook plate fixation use at our hospital between January 2010 and May 2020. Finally, we included 76 cases of acute distal clavicle fracture (DCF) and 84 cases of acute acromioclavicular joint dislocation (ACD). We investigated all complications after hook plate use, bony union in the DCF group, and reduction loss in the ACD group. We defined painful shoulder stiffness (PSS) as aggravating resting pain with stiff shoulder, and pain on shoulder elevation (PSE) as conti

EVALUATION OF RESULTS AND COMPLICATIONS FROM ARTHROSCOPIC SUTURE OF SLAP LESIONS.

OBJECTIVE: To evaluate the results and complications from arthroscopic suturing of SLAP lesions. METHODS: Seventy-one patients who underwent arthroscopic suturing of SLAP lesions between July 1995 and May 2008 were evaluated. The procedures were performed by the Shoulder and Elbow Surgery Group of the Department of Orthopedics and Traumatology, Fernandinho Simonsen Wing, Santa Casa de São Paulo, Brazil. Associated lesions were seen in 68 of the 71 patients evaluated (96%), and the other three (4%) had SLAP lesions alone. RESULTS: The associated lesions most frequently found in the patients u

Combined serratus anterior plication and pectoralis minor release improves clinical outcomes in refractory scapular dyskinesis with neurological involvement.

BACKGROUND: Scapulopexy can effectively treat symptomatic scapulothoracic dyskinesis. In patients with preserved serratus anterior function, restoring scapular positioning without rib-based fixation may reduce surgical risks and rib osteolysis. This study introduces a modified technique-Serratus Anterior Plication and PEctoralis minor Release (SAPPER)-and reports its outcomes in patients with refractory scapular dyskinesis associated with neurologic symptoms. MATERIALS AND METHODS: Patients referred for treatment of symptomatic scapular dyskinesis were included if electromyography revealed lo

Prospective observational study of daily upper-limbs activities following arthroscopic rotator cuff repair using triaxial accelerometers: Recovery process and relationship with preoperative clinical parameters.

BACKGROUND: Following arthroscopic rotator cuff repair (ARCR), upper-limb movement must be restricted during the early postoperative phase. However, the postoperative recovery process of daily upper-limb use and its relationship with preoperative parameters remain unclear. We investigated the postoperative recovery process of the operated limb use following ARCR quantitatively using triaxial accelerometers and examined its relationship with preoperative parameters. METHODS: We evaluated bilateral upper-limb use in daily activities using triaxial accelerometers in 21 patients who underwent ARC

Is the graft position critical for functional outcomes following arthroscopy-assisted lower trapezius tendon transfer for posterosuperior irreparable rotator cuff tears? A comparison of anterior vs. posterior position of graft.

BACKGROUND: Arthroscopy-assisted lower trapezius tendon transfer (aLTT) has emerged as a reasonable treatment option for posterosuperior irreparable rotator cuff tears (PSIRCTs) due to its biomechanical advantages and favorable clinical outcomes. Although there are various surgical techniques for aLTT, the optimal positioning of the graft on the humeral head footprint is unknown in aLTT. This study aimed to evaluate clinical and radiological outcomes based on the interpositional graft position of aLTT graft in PSIRCTs. METHODS: Patients who underwent aLTT for PSIRCTs from 2017 to 2022 were re

Low skeletal muscle mass is associated with inferior preoperative and postoperative shoulder function in elderly rotator cuff tear patients.

BACKGROUND: The age-related loss of skeletal muscle mass is an important characteristic of sarcopenia, an increasingly recognized condition with systemic implications. However, its association with shoulder function in elderly patients with rotator cuff tears (RCT) remains unknown. This study aimed to investigate the relationship between low skeletal muscle mass and shoulder function in elderly RCT patients. METHODS: A retrospective analysis was conducted on RCT patients who underwent chest computed tomography (CT) scans for clinical evaluation. Preoperative CT scan images of the chest were u

Evaluating the content validity of the Single Assessment Numeric Evaluation (SANE) in shoulder pathologies.

BACKGROUND: The Single Assessment Numeric Evaluation (SANE) is a single item patient reported outcome measure (PROM) of function, in where patients rate how normal they feel in relation to a specific joint or problem. Although validated for some orthopedic conditions, it is not yet validated for a population with shoulder pathologies; nor have other studies explored the content validity. This study aims to understand how 1) patients with shoulder conditions interpret and calibrate responses to the SANE and 2) how they define normal. METHODS: This study uses cognitive interviewing, a qualitati
