Genetic causal association between varicella-zoster virus infection and psychiatric disorders: A 2-sample Mendelian randomization study.

BACKGROUND: Psychiatric disorders, such as schizophrenia (SCZ), major depressive (MDD), and bipolar disorder (BD) have a profound impact on millions of individuals worldwide. The critical step toward developing effective preventive and treatment strategies lies in comprehending the causal mechanisms behind these diseases and identifying modifiable risk factors associated with them. METHODS: In this study, we conducted a 2-sample Mendelian randomization analysis to explore the potential links between chickenpox(varicella-zoster virus infection) and three major psychiatric disorders(SCZ, MDD, B…

Incidence and natural history of chemically defined varicella-zoster virus hepatitis in children and adolescents.

Of 786 children and adolescents enrolled in a multicenter trial of acyclovir for chickenpox, 27 (3.4%) met the case definition of varicella-zoster virus (VZV) hepatitis (serum aspartate aminotransferase level > or = 100 U/l). The clinical and cutaneous manifestations of chickenpox in the 15 placebo recipients with this complication did not differ significantly from those in 45 matched controls (p > 0.05), indicating that liver involvement by VZV is not a consequence of more extensive disease. Although acyclovir modified the course of chickenpox overall, it did not prevent VZV hepatitis; that i…

Invasive group A streptococcal disease in children and association with varicella-zoster virus infection. Ontario Group A Streptococcal Study Group.

OBJECTIVES: To describe the incidence and clinical features of invasive group A streptococcal (GAS) disease in children in Ontario and determine the risk of invasive GAS infection following chickenpox. METHODS: During 1992-1996, we conducted prospective, active, population-based surveillance for pediatric invasive GAS disease in Ontario, Canada (population: 11 million; 2.5 million children) and reviewed clinical and laboratory records. RESULTS: There were 1.9 cases of invasive GAS disease per 100,000 children per year. Streptococcal toxic shock syndrome (STSS) occurred in 7% of cases and nec…

Responding to suspected smallpox cases in the Los Angeles County from 2002 to 2006: identifying areas for education.

INTRODUCTION: Although smallpox has been eradicated, health care providers in emergency departments (EDs) need to remain vigilant to its recognition. Smallpox can be confused with chickenpox. We describe suspected smallpox cases reported in Los Angeles County from 2002 to 2006 and highlight areas for education. METHODS: We retrospectively reviewed suspected smallpox reports from 2002 to 2006. Laboratory testing was performed. Photographs of rashes were taken. RESULTS: Five suspected smallpox cases were reported. Two presented first to an ED. Smallpox was suspected based on rash features. Pre…

Rare complications of chicken pox in immunocompetent children: Acute respiratory distress syndrome.

Chickenpox is a highly contagious disease caused by varicella zoster virus. Fever and papulovesicular rash are hallmarks of this disease. These manifestations are self-liming and complete recovery is seen in most cases. We report two cases of chickenpox infection where the rare and atypical manifestations of acute respiratory distress syndrome with pleural effusion were seen.

Cost-effectiveness analysis of varicella vaccine as post-exposure prophylaxis in Hong Kong.

OBJECTIVES: The varicella vaccine is an effective post-exposure prophylaxis (PEP) for chickenpox. This study aimed to analyze the cost-effectiveness of PEP using varicella vaccine for pediatric patients from the perspective of the public healthcare provider in Hong Kong. METHODS: A decision tree was designed to compare cost and clinical outcomes of PEP with varicella vaccine versus no PEP in pediatric patients (aged 1-18 y) susceptible to chickenpox with household exposure. Two tiers of outcome were simulated: (1) total direct medical cost per subject exposed, and (2) the quality-adjusted lif…

[Varicella pneumonia associated with bilateral pleurisy in an immunocompetent adult].

Chickenpox is a viral infection usually benign and practically mandatory of the childhood. In the adult, it may know multivisceral complications as pneumonia in relationship with chickenpox which is the most frequent. One case of varicella pneumonia with a bilateral pleurisy has been noticed in a 38-year-old adult. The definite diagnosis has been based on clinical, biological and radiological arguments. Under an antiviral treatment, the evolution is favourable.

Varicella gangrenosa with toxic shock-like syndrome due to group A streptococcus infection in an adult: case report.

Varicella gangrenosa is a rare and serious complication of chickenpox that has been described in children only. We describe a case of an adult with varicella gangrenosa that presented as necrotizing fasciitis of a limb. This infection is caused by group A streptococcal superinfection of the skin lesions due to chickenpox. It can be misdiagnosed, with fatal consequences. Because of prompt recognition and aggressive surgical and medical treatment, the patient survived without loss of the affected limb.

[Interpretation of varicella serology in pregnancy].

Chickenpox during pregnancy may result in a severe, in some cases fatal, generalized infection of the newborn. Pregnant women with unknown Varicella-Zoster-virus (VZV) immunity, who have had contact with an individual infected by chickenpox, should be tested immediately for specific antibodies to VZV. The interpretation of test results may be difficult under certain circumstances. This problem is discussed using individual case studies.

[Varicella-pox virus infection: features of the course, clinical manifestations, complications, and possibilities for prevention].

Varicella zoster virus (VZV) is a pathogenic human herpes virus that causes chickenpox as a primary infection, after which it persists for a long time and latently in the peripheral ganglia. Decades later, the virus can reactivate spontaneously, or after exposure to a number of triggering factors, causing herpes zoster (shingles). The reasons for the long-term persistence of VZV are gradually being revealed, but some issues remain unknown at the moment. Chickenpox and its complications are especially difficult in immunocompromised patients, but they are often found in people without risk facto…

Reye syndrome associated with subclinical varicella zoster virus and influenza A infection.

An association is reported between Reye syndrome and varicella zoster virus (VZV) infection in a 10-year-old boy who had serologic evidence of coinfection with VZV and influenza A H3N2, and exposure to salicylates. He developed VZV reinfection without skin lesions after family exposure and influenza A was community-acquired. Recent chickenpox contact should initiate VZV serologic studies in Reye syndrome patients, regardless of the chickenpox history or evidence of infection with other viruses.

Hypopyon sign as an unusual complication of varicella infection in a girl with atopic dermatitis.

Varicella-zoster virus (VZV) infection, also known as chickenpox, is a common childhood affliction. Generalized small itchy single-standing vesicles on erythematous skin are typical. Both cutaneous and systemic complications of the VZV infection may commonly occur. A three-year-old girl with a previous history of mild atopic dermatitis presented in our Pediatric Dermatology Clinic in poor general condition, with a skin rash predominantly consisting of generalized large blisters with hypopyon sign and erosions. On a closer look, scattered erythematous papules and vesicles were also visible. A p…
