Assessment of the Factors That Affect the Anatomic and Functional Success of Cartilage Tympanoplasty in Children.

OBJECTIVES: Many factors may influence the surgical outcome of tympanoplasty in children, including age, the size and location of perforation, otorrhea, status of contralateral ear, surgical technique, and adenoid hypertrophy. This study aims to evaluate the outcomes of pediatric cartilage tympanoplasty and to assess the factors that affect the success of tympanoplasty in children. METHODS: Children with chronic otitis media who underwent tympanoplasty using cartilage as graft material were evaluated retrospectively. Patient age, gender, size and site of the perforation, status of the contral

[Cartilage vs. temporal fascia as graft material].

So far, comparative studies have shown no significant audiological difference between cartilage and fascia grafts. In this retrospective study, the Turkish scientists E E Callioglu et al. examined next audiological results and morphological results after surgical cartilage and fascial tympanoplasty in patients with comparable preoperative hearing and middle ear pathology.

Team approach to total auricular reconstruction.

The surgery of microtia is a procedure that demands surgical and artistic skills, as well as precise planning of interventions. In cases where a functioning inner ear is present, functional and esthetic reconstruction is performed jointly with ear, nose, and throat surgeons. The middle ear plasty is performed through a transmastoidial approach at the time of the second stage of the outer ear reconstruction. We demonstrate the special requirements of this team approach and also pay a tribute to Burt Brent, who has set the standards of modern ear reconstruction.

Navigation-assisted construction of an external ear canal using an autogenous foreskin graft.

Classic microtia is combined with external ear canal atresia and middle ear malformation. In order to evaluate whether an operation to improve hearing ability and the use of computer-assisted surgery are indicated, preoperative high-resolution navigation CT is mandatory. We combined atresia surgery and tympanoplasty with auricular reconstruction in the case of an 8-year-old boy with bilateral microtia, aural atresia and malformation of the middle ear. After creating an auricle framework with rib cartilage and transplanting it under the skin of the mastoid plane, we shifted it forward in the se

Ear replantation without microsurgery.

Reconstitution of the amputated ear remains a challenge to the plastic surgeon. Reattachment as a composite graft of the total or subtotal amputated ear is unreliable. Microsurgical replantation can be performed in a minority of cases; however, difficulty with adequate venous drainage complicates the technical complexity of these cases. To enhance survival of a reattached ear segment, Mladick et al. advocated use of the retroauricular pocket principle. This technique involves deepithelialization of the amputated part, followed by anatomic reattachment to the amputation stump and then burial in

[Reconstruction of the external ear].

The authors after a short morphological description of the microtia, expose the surgical evolution which had led to the actual auricular reconstruction technique, using a costal cartilage framework. This technique requiring several steps is exposed in the typical case of microtia. It is the preferential method. The difficult reconstructions due to the poor quality of the skin are exposed by degree of complexity: in these cases, tissue expansion, fascia temporalis superficialis can be useful and finally auricular prostheses, attached to bone-integrated implants, can be recommended. The function

Reconstruction of microtia. Pursuing a natural appearance.

We have designed a "contour-accentuated" auricular framework in which the depth and height of the relief of the contours have been exaggerated to improve the illusion of three-dimensional topography. In one technique, the "full framework," a large amount of supplemental skin coverage is needed, and this is provided by a superficial temporal artery flap surfaced with postauricular skin and groin skin. In the second type, the "low-profile framework," the supplemental skin graft requirement is less and is provided by local subcutaneous tissue flaps in the conchal-tragal area. These are surfaced w

Pseudocyst of the auricle associated with trauma.

A pseudocyst of the auricle (benign idiopathic cystic chondromalacia) is an intracartilaginous cystic swelling of the anterior auricle. The cause is uncertain, and most patients deny any history of inflammation or trauma. A patient had antecedent trauma and a fracture in the conchal cartilage; a simple surgical procedure was used to treat this patient.

The versatile cartilage autograft in reconstruction of the nose and face.

This paper deals with the long-term follow-up of cartilage autografts taken from various parts of the body to reconstruct areas of the nose, ear, trachea, eyelid, and other areas of the body which require augmentation, effacement, and long-term support. Our thesis will be that the cartilage autograft is the implant of choice in many of these areas, and that fate of autogenous cartilage is well known and should be given strong priority in facial grafting.

Advances in the understanding of chondrodermatitis nodularis chronica helices: the perichondrial vasculitis theory.

OBJECTIVES: Chondrodermatitis nodularis chronica helicis (CNCH) usually presents as a painful nodule affecting the pinna. The aetiology of the disease is unknown. Several theories have been suggested. We suggest a possible explanation based upon pathophysiological treatment correlations to new histopathological evidence. DESIGN: A detailed histopathological review of 16 confirmed cases of CNCH was undertaken by two pathologists, independently and together, using a qualitative grading of arteriolar narrowing. RESULTS: Review of cases revealed arteriolar narrowing in perichondrium region of pi

Butterfly cartilage graft inlay tympanoplasty for large perforations.

OBJECTIVE: Butterfly graft inlay tympanoplasty is a well-established technique for the repair of small perforations. However, the efficacy of the technique for medium and large tympanic membrane perforations remains unknown. STUDY DESIGN: Retrospective case series. METHODS: Postauricular tympanoplasty and tympanomastoidectomy using a large butterfly cartilage inlay graft (>4 mm diameter to total drum replacement) were analyzed in 90 pediatric patients (99 ears). RESULTS: Patient ages ranged from 2 to 20 years; mean follow-up duration was 27.6 months. Successful closure occurred in 92% of th

Reconstruction of microtia.

The reconstruction of microtia continues to represent one of the more challenging plastic surgery procedures. The limitations of the reconstruction are partly inherent in the soft-tissue deficiencies present in increasing degrees from large conchal remnant microtia, to lobular microtia, to the displaced remnant in auricular dystopia, and partly because of the high level of technical expertise required. After a brief review of the history of ear reconstruction in general and microtia specifically, issues related to the transition in the popularity from techniques involving three or more stages

Well-differentiated liposarcoma with chondroid metaplasia in the auricle of a dog.

A 13-year-old spayed female dog had a mass in the left auricle. Grossly, connection between the mass and original auricular cartilage was not recognized. The mass was unencapsulated and contained multiple islands of mature hyaline cartilage and neoplastic adipocytes. The neoplastic cells comprised predominant mature adipocytes, scattered lipoblasts and irregular round to spindle cells with moderate atypia. The atypical cells occasionally had lipid droplets. A diagnosis of well-differentiated liposarcoma (WDL) with chondroid metaplasia was made. This is the first report for liposarcoma with cho

Classification of the concha-type microtia and their new suitable treatment strategies without autogenous costal cartilage grafting.

OBJECTIVE: Numerous corrective methods have been successfully applied in concha-type microtia reconstruction over the past several decades, and autogenous rib cartilage grafting has become a routine technique in a two or three-stage operation. However, it still remains a challenge due to the effective use of the large volume of the remnant cartilage and skin involved. The objective of this study was to clarify how this remnant cartilage and skin could be manipulated for new suitable treatment strategies without autogenous costal cartilage grafting. METHODS: A total of 424 patients with concha

A translational approach to airway reconstruction leveraging decellularized meniscus and cartilage progenitor cells.

Severe subglottic stenosis develops in over 20,000 infants per year and requires laryngotracheal reconstruction to enlarge the airway by implanting autologous cartilage from a rib graft. However, young children often lack sufficiently sized costal cartilage for this procedure, resulting in increased donor site morbidity and operative time, as well as an elevated risk for airway restenosis, necessitating revision surgery. To overcome these limitations, we create a scaffold based on porcine meniscal cartilage decellularization - which we term MEND - by selectively digesting away the elastin and
