Obstetrical and epidemiological factors influence the severity of anal incontinence after obstetric anal sphincter injury.

BACKGROUND: Obstetric anal sphincter injury (OASI) is one of the most severe obstetrical complications. Although risk factors for OASI have been identified, little is known about various parameters that can influence symptoms' severity. The aim of this study is to explore whether obstetrical and epidemiological factors can have an effect on the severity of symptoms after OASI. METHODS: 11.483 deliveries between January 2010 and December 2014 were reviewed, and data from 88 women with OASI are presented. RESULTS: The only statistically significant differences between symptomatic and asymptoma

Systemic inflammation linking chronic periodontitis to cognitive decline.

Persistent inflammation in the systemic immune system can impose detrimental effects on the central nervous system (CNS). Neuroinflammation might be a result of this to accelerate the progressive deterioration of neuronal functions during aging. In this regard, controlling inflammation through delaying and/or preventing chronic inflammatory diseases may be a potential strategy to prevent or modify the progression of Alzheimer's Disease (AD). Periodontitis is a chronic inflammatory disease of the oral cavity that is common among the elderly, especially for those who have decline in cognitive fu

Quinacrine sterilization (QS): time for reconsideration.

Dr. Jaime Zipper, the Chilean inventor of the quinacrine method of nonsurgical permanent contraception, was aware that when chest surgeons injected quinacrine into the pleural cavity to treat and prevent reoccurrence of pleural effusion, it resulted in the formation of fibrous adhesions between the lung and costal pleura. Zipper thought that a similar scarring effect could occur in the fallopian tubes if quinacrine was instilled into the uterine cavity. A series of refinements of the methodology culminated in the use of a modified Copper T intrauterine device inserter tube as a delivery system

Sensitivity and specificity of serological tests, histopathology and immunohistochemistry for detection of Toxoplasma gondii infection in domestic chickens.

Since free-range chickens are important for the epidemiology of toxoplasmosis, this study evaluated the sensitivity and specificity of different laboratory techniques for the diagnosis of Toxoplasma gondii in these animals. Serum samples from 135 adult domestic chickens were tested for anti-T. gondii antibodies by the indirect fluorescent antibody test (IFAT), enzyme-linked immunosorbent assay (ELISA), modified agglutination test (MAT), and indirect hemagglutination test (IHAT). Tissue samples from all animals were analyzed by histopathology, immunohistochemistry and mouse bioassay (gold stand

Infections in children and adolescents with juvenile idiopathic arthritis and inflammatory bowel disease treated with tumor necrosis factor-α inhibitors: systematic review of the literature.

Tumor necrosis factor alpha (TNF-α) inhibitors are increasingly administered to children and adolescents with juvenile idiopathic arthritis (JIA) and pediatric inflammatory bowel disease (pIBD). Adult studies indicate that TNF-α inhibitors lead to an increased risk of serious infections compared to other disease-modifying antirheumatic drugs. We report herein a systematic literature review detailing the epidemiology and types of infections reported in children with JIA and pIBD treated with TNF-α inhibitors. The most frequently reported infections were mild and characterized as viral in etiolo

Carotid intima-media thickness: an early marker of cardiovascular disease.

Carotid artery intima-media thickness (IMT) measurements are now recognized as a surrogate measure of atherosclerosis. The measurement is performed on high-resolution ultrasound images of the carotid artery at the level of the carotid bifurcation. Increased IMT values indicate a higher likelihood of having had a stroke or myocardial infarction. In epidemiological studies, IMT measurements made in asymptomatic individuals also predict future cardiovascular events. Intima-media thickness measurements have also been used in clinical trials as a means of gauging the effects of interventions that m

Bacterial colitis increases susceptibility to oral prion disease.

Dietary exposure to prion-contaminated materials has caused kuru and variant Creutzfeldt-Jakob disease in humans and transmissible spongiform encephalopathies (TSEs) in cattle, mink, and felines. The epidemiology of dietary prion infections suggests that host genetic modifiers and possibly exogenous cofactors may play a decisive role in determining disease susceptibility. However, few cofactors influencing susceptibility to prion infection have been identified. In the present study, we investigated whether colitis might represent one such cofactor. We report that moderate colitis caused by an

Building the model: a review of input structures in extreme temperature-health.

BACKGROUND: The health impacts of extreme temperatures have been extensively studied through epidemiological models. However, limited attention has been paid to the specification of these models, particularly regarding input structure and model selection. Although exposure metrics and statistical techniques have evolved over time, a comprehensive synthesis of the variables included in these models, and the rationale behind their inclusion, is still lacking. This gap limits the comparability of studies and may compromise the robustness of temperature-health evidence. METHODS: We conducted a sy

The Global Syndemic of Modifiable Cardiovascular Risk Factors Projected From 2025 to 2050.

BACKGROUND: Cardiovascular diseases (CVDs) are the leading cause of global mortality. Understanding epidemiologic trends in the modifiable risk factors driving CVDs is essential in designing effective countermeasures. OBJECTIVES: The aim of this study was to forecast geospatial trends of modifiable cardiovascular risk factors, namely high systolic blood pressure (SBP), high fasting plasma glucose, high body mass index (BMI), high low-density lipoprotein cholesterol, and tobacco use from 2025 to 2050. METHODS: Historical data on disability-adjusted life years (DALYs) and mortality from the GB

Corticosteroid-sparing options in the treatment of childhood asthma.

During the last 30 years, a significant rise in wheezing illness has occurred in the child population. Despite its high prevalence there is no clear definition of the disease, which includes a heterogeneous group of syndromes ranging from transient wheezing in infancy to atopic asthma with persistence into adult life. Molecular advances and further epidemiological information from well characterised individuals and their families are likely to clarify the different subtypes of wheezing illness and inform therapeutic options. With the recognition that chronic airway inflammation is a feature of

Paths to Alzheimer's disease prevention: from modifiable risk factors to biomarker enrichment strategies.

Alzheimer's disease (AD) represents an increasing worldwide healthcare epidemic. Secondary preventive disease-modifying treatments under clinical development are considered most effective when initiated as early as possible in the pathophysiological course and progression of the disease. Major targets are to enhance clearance and to reduce cerebral accumulation of amyloid, decrease hyperphosphorylation of tau and the generation of neurofibrillary tangles, reduce inflammation, and finally progressive neurodegeneration. Comprehensive sets of biological markers are needed to characterize the path

Neurobiological mechanisms linking vitamin d signaling to cognitive decline and neurodegeneration: Untangling epidemiology, pathophysiology, and evidence.

Dementia is a leading cause of disability and dependence among older adults, and its prevalence is projected to increase sharply with age. While age and genetics remain the dominant risk factors, modifiable contributors are gaining attention, including vitamin D deficiency, which is highly prevalent worldwide. This integrated review synthesizes current evidence on the association between vitamin D status and dementia, spanning the epidemiological, mechanistic, and interventional domains. Epidemiological studies have consistently linked low serum 25-hydroxyvitamin D levels with poorer cognition

Risk factors for type 2 diabetes mellitus: An exposure-wide umbrella review of meta-analyses.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a global epidemic associated with increased health expenditure, and low quality of life. Many non-genetic risk factors have been suggested, but their overall epidemiological credibility has not been assessed. METHODS: We searched PubMed to capture all meta-analyses and Mendelian randomization studies for risk factors of T2DM. For each association, we estimated the summary effect size, its 95% confidence and prediction interval, and the I2 metric. We examined the presence of small-study effects and excess significance bias. We assessed the epidemi

The use of participatory modelling to inform management of endemic foot-and-mouth disease: A case study from Nusa Tenggara Barat, Indonesia.

Management of infectious diseases such as foot-and-mouth disease (FMD) in the endemic situation in smallholder farming systems brings significant challenges. While spatially explicit agent-based models (ABMs) are useful to inform disease response decision-making, complementary methods are needed to more fully capture the influences of context on disease control and management, such as human behaviour and ecological factors which are of particular importance for endemic diseases. We describe the development of an integrated Participatory Modelling approach (PM) applied to the control of FMD wit

Growth Modulation of Diabetic Factors and Antidiabetic Drugs on Prostate Cancer Cell Lines.

Risk factors for prostate cancer (PCa) include age, hormones, race, family history and diet. Recently, epidemiologic evidence has indicated that history of diabetes mellitus (DM) is inversely associated with risk of PCa. However, epidemiological investigations have yielded inconsistent results. Hence, the exact mechanism of DM-induced reduction in the incidence of PCa has yet to be fully elucidated. The aim of this study was to investigate the effects of DM factors, including glucose, insulin and insulin-like growth factor-1 (IGF-1), on the proliferation of PCa cell lines in vitro. Cell prolif

Lung cancer among silica-exposed workers: the quest for truth between chance and necessity.

BACKGROUND: In 1997, IARC upgraded crystalline silica to a Group 1 human carcinogen. However, the IARC report itself acknowledged variations in risk depending on inherent characteristics of the crystalline silica or external factors affecting its biological activity or distribution of its polymorphs. METHODS: We reviewed silica physical and physico-chemical properties and how such properties may affect its interaction with the target cells. Studies of silica, silicosis and lung cancer published from 1997 onwards are then reviewed in the search of any new advances in knowledge about silica car

Short-Term Low Dose Cannabidiol Does Not Influence Glucose Tolerance or the Gut Microbiome in Sedentary Adults with Overweight and Obesity: Pilot Study.

INTRODUCTION: Epidemiological data indicate that regular users of cannabis products may be protected from type 2 diabetes, although the mechanism is not understood. Observations from animal studies suggest that the cannabinoid, cannabidiol (CBD) may protect/improve glucose tolerance; an effect that may be partially mediated by favorable modifications to the gut microbiome. The aims of the current pilot project were to gain initial insight into the influence of short-term CBD ingestion on oral glucose tolerance, the gut microbiome, and inflammation in sedentary adults with overweight or obesity

Short-Term Association of Air Pollutant Levels and Hospital Admissions for Stroke and Effect Modification by Apparent Temperature: Evidence From Shanghai, China.

The epidemiological evidence on relationships between air pollution, temperature, and stroke remains inconclusive. Limited evidence is available for the effect modification by apparent temperature, an indicator reflecting reactions to the thermal environment, on short-term associations between air pollution and hospital admissions for stroke. We used a generalized additive model with Poisson regression to estimate the relative risk (RR) of stroke admissions in Shanghai, China, between 2014 and 2016 associated with air pollutants, with subgroup analyses by age, sex, apparent temperature, and se
