The problem of inadequate hip stability prevents many patients with head trauma, stroke, or spinal injury from balancing on one limb in order to take a step. There is no adequate orthotic substitute for hip instability, and electrical stimulation with surface electrodes cannot effectively activate the deep hip muscles. This report describes a clinical program in which electrical stimulation via surgically placed electrodes is combined with routine tendon lengthening and transfers and physical therapy. The electrodes are fixed to the epimysium and the leads directed subcutaneously to exit the s

INTRODUCTION: Labyrinthitis ossificans is a known complication of bacterial meningitis which results in severe to profound hearing loss. Historically, labyrinthitis ossificans was considered a contraindication for cochlear implantation. However, with advances in technology, cochlear implantation is an accepted treatment for hearing loss in labyrinthitis ossificans.
MATERIALS AND METHODS: 17 cases with varying degrees of ossification in the cochlea were evaluated and treated at our tertiary referral hospital and cochlear implantation center out of a total of 832 profoundly deaf patients evalua

BACKGROUND: Currently, the implantation of permanent cardiac pacemakers entails mostly subclavian vein puncture, which is relatively simpler and easier to master. However, due to individual differences, some patients carry a narrow space between the clavicle and the first rib. If the range of activity of the upper limb is increased, the friction between the electrode wire and the bone gap leads to the breakage of the electrode wire, which is manifested by poor pacemaker perception and pacing.
CASE SUMMARY: A 68-year-old woman underwent permanent pacemaker implantation in our hospital because

OBJECTIVES: Sacral nerve stimulation (SNS) is a surgical treatment of urinary and fecal incontinence. Despite its clinical efficacy, the mechanisms of action of SNS are still poorly known. This may be related to the use of acute stimulation models. Up to date, no rodent model of chronic SNS implants has been developed. Therefore, the aim of this study was to create a fully implantable and remotely controllable stimulating device to establish an animal model of chronic SNS.
MATERIALS AND METHODS: The stimulating device consisted of an implantable pulse generator linked to a platinum electrode.

BACKGROUND: The increasing number of patients with cardiac implantable electronic devices (CIEDs) causes a rise in the absolute percentage of individuals qualifying for a transvenous lead extraction (TLE) due to infectious, vascular or lead failure related indications. As the survival time prolongs, TLE procedures more and more often concern the electrodes of long- -term functioning. Authors provide a retrospective analysis of the effectiveness and safety of TLE performed on leads implanted at least 10 years before the extraction.
METHODS: Between 2008 and 2012 we performed TLE of 364 electro

Within the first 14 days after implantation, thresholds were measured at three transvenous pacemaker electrodes with different surface areas. It should be verified, 1. to what extent a correlation existed between maximal threshold increase and electrode surface, 2. how electrode impedance and 3. how the amplitudes of the R-wave voltages developed. The results were: At initial implantation, thresholds were all the lower the smaller the electrode-surface area was. On overage, 9-10 days after implantation the highest thresholds were reached. Thereby, in small surface area electrodes the threshold

The availability of thin-film multichannel electrodes provides new possibilities for implantation and direct stimulation of the modiolar portion of the auditory nerve. Electrodes in direct contact with the auditory nerve should be functional at lower thresholds and might require fewer remaining neurons for stimulation compared to electrodes in the scala tympani. This strategy would also provide close contact with neural elements subserving a greater frequency range. Implantation of the eighth nerve may also be advantageous in profoundly deaf subjects who lack an implantable scala tympani. In t

A cochlear implant (CI) is a neural prosthetic device that restores hearing by directly stimulating the auditory nerve using an electrode array that is implanted in the cochlea. In CI surgery, the surgeon accesses the cochlea and makes an opening where he/she inserts the electrode array blind to internal structures of the cochlea. Because of this, the final position of the electrode array relative to intra-cochlear anatomy is generally unknown. We have recently developed an approach for determining electrode array position relative to intra-cochlear anatomy using a pre- and a post-implantation

BACKGROUND: Single unit recording in behaving nonhuman primates is widely used to study the primate central nervous system. However, certain questions cannot be addressed without recording large numbers of neurons simultaneously. Multiple 96-electrode probes can be implanted at one time, but certain problems must be overcome to make this approach practical.
NEW METHOD: We describe a series of innovations and practical guidance for implanting and recording from 8 arrays of 96 electrodes (768 electrodes) in the frontal cortex of Macaca mulatta. The methods include an individualized 3D-printed c

BACKGROUND/OBJECTIVES: Deep brain stimulation (DBS) is an established surgical treatment for essential tremor (ET). Both the thalamic ventral intermediate nucleus (VIM) and the posterior subthalamic area (PSA) are effective tremor targets can be approached through a single trajectory. This technical feasibility report describes a single-trajectory dual-target VIM-PSA DBS surgical protocol using octopolar electrodes and evaluates stereotactic accuracy, anatomical contact distribution, and preliminary surgical safety.
METHODS: Eleven patients with medication-refractory ET underwent bilateral DB

Intraspinal microstimulation (ISMS) may provide a means for improving motor function in people suffering from spinal cord injuries, head trauma, or stroke. The goal of this study was to determine whether microstimulation of the mammalian spinal cord could generate locomotor-like stepping and feedback-controlled movements of the hindlimbs. Under pentobarbital anesthesia, 24 insulated microwires were implanted in the lumbosacral cord of three adult cats. The cats were placed in a sling leaving all limbs pendent. Bilateral alternating stepping of the hindlimbs was achieved by stimulating through

Conducting polymer coatings on metal electrodes are an efficient solution to improve neural signal recording and stimulation, due to their mixed electronic-ionic conduction and biocompatibility. To date, only a few studies have been reported on conducting polymer coatings on metallic wire electrodes for muscle signal recording. Chronic muscle signal recording of freely moving animals can be challenging to acquire with coated electrodes, due to muscle movement around the electrode that can increase instances of coating delamination and device failure. The poor adhesion of conducting polymers to

UNLABELLED: Chronically implanted neural multi-electrode arrays (MEA) are an essential technology for recording electrical signals from neurons and/or modulating neural activity through stimulation. However, current MEAs, regardless of the type, elicit an inflammatory response that ultimately leads to device failure. Traditionally, rigid materials like tungsten and silicon have been employed to interface with the relatively soft neural tissue. The large stiffness mismatch is thought to exacerbate the inflammatory response. In order to minimize the disparity between the device and the brain, we

An inflammatory tissue reaction around the electrode array of a cochlear implant (CI) is common, in particular at the electrode insertion region (cochleostomy) where mechanical trauma often occurs. However, the factors determining the amount and causes of fibrous reaction surrounding the stimulating electrode, especially medially near the perimodiolar location, are unclear. Temporal bone (TB) specimens from patients who had undergone cochlear implantation during life with either Advanced Bionics (AB) Clarion ™ or HiRes90K™ (Sylmar, CA, USA) devices that have a half-band and a pre-curved electr

Vertigo control in cases of Ménière disease and deafness can be achieved by labyrinthectomy before or as a single-stage procedure during cochlear implantation. The aim was to describe a case in which a labyrinthectomy was performed after cochlear implantation. The scar tissue was removed from the electrode cable, and the receiver was removed from the periostal pocket and placed out without electrode dislocation. Labyrinthectomy was performed after securing the electrode at the external canal. The patient disclaimed after three months no disabling vertigo. Intraoperatively, the electrode was no

Implantable bioelectrodes have attracted significant attention for precise in vivo signal transduction with living systems. Conductive polymers, including polypyrrole (PPy), have been widely used as bioelectrodes due to their large surface areas, high charge injections, and versatilities for modification. Especially, several natural biopolymers, such as hyaluronic acid (HA), can be incorporated into conductive polymers to produce biomimetic electrodes with better biocompatibility. However, HA-incorporated PPy electrodes (PPy/HA) frequently lose their original performances after implantation in

A male patient with post lingual hearing loss was implanted at 16 years of age with a Nucleus cochlear implant (Cochlear) in his left ear. Twenty two years later, he developed a cholesteatoma in relation to the mastoid portion of the facial nerve and in contact with the electrodes array. The cholesteatoma was removed and the ear canal reconstructed. There was no post-operative facial palsy and the cochlear implant was preserved. Although cholesteatoma is a rare complication of the cochlear implant surgery, it still can occur and can be a source of potential damage to the implant. The origin of
