New strategies to prevent catheter-associated urinary tract infections.

Catheter-associated urinary tract infection (CAUTI) is the most common health-care-associated infection worldwide. Although not all cases of bacteriuria result in clinical infection, several hundred thousand episodes of CAUTI occur each year in the USA alone. The milieu in which the catheter is placed is highly conducive to bacterial colonization, biofilm formation on the catheter surface, and inevitable catheter-associated bacteriuria. A multitude of novel methods of CAUTI prevention have been described, including established approaches that are routinely recommended, such as the use of a sec

An indwelling urinary catheter for the 21st century.

What's known on the subject? and What does the study add? A vast literature has been published on the prevalence, morbidity and microbiology of catheter-associated urinary tract infections. Research and development in recent years has focused on producing antibacterial coatings for the indwelling Foley catheter with insufficient attention to its design. This article provides a critical examination of the design of the indwelling Foley catheter. Design specifications are outlined for a urine collection device that should reduce the vulnerability of catheterised urinary tract to infection. The i

Hand-pumps as reservoirs for microbial contamination of well water.

The retention and release of total coliforms and Escherichia coli was investigated in hand-pumps removed from tubewells tapping a faecally contaminated aquifer in Matlab, Bangladesh, and from a new hand-pump deliberately spiked with E. coli. All hand-pumps were connected to reservoirs of sterile water and flushed. Faecal coliforms were observed in the discharge from all three of the previously used hand-pumps, at concentrations comparable to levels measured in discharge when they were attached to the tubewells. During daily flushing of one of the previously used hand-pumps, the concentration o

Examination of biofilm formation and risk of infection associated with the use of urinary catheters with leg bags.

Urinary catheters and legs bags were simultaneously colonized by Escherichia coli and Proteus vulgaris using a model urinary drainage system. the system was continuously supplied with filter-sterilized artificial urine using a diurnal flow pattern. The extent of colonization was determined by assessment of both planktonic and biofilm formation over time. Contamination of the catheters resulted in rapid colonization of the whole system within a 24 h period. Contamination of the leg bags resulted in an ascending biofilm formation over a four-day period. Results indicated that infection risk coul

Comparison study of home catheter cleaning methods.

Three methods of cleaning urinary catheters for reuse at home by clients on intermittent catheterization programs were studied. Reused catheters were collected from clients, autoclaved, and then incubated in a culture of Escherichia coli broth. Three different isolates of E. coli were used at concentrations ranging from 4.8 x 10(5) to 1.0 x 10(8) colony-forming units per milliliter. The catheters were then rinsed with tap water for 1 minute and soaked in one of three cleaning solutions for 30 minutes. The three cleaning solutions studied were 0.6% hydrogen peroxide, bleach in a 1:4 solution wi

(Bi, Ni)-Based Bimetallic MOF With Light-Assisted Hydrogen Peroxide Generation for Synergistic and Efficient Photocatalytic Antimicrobial.

To effectively control the contamination of antibiotic-resistant bacteria (ARB) and antibiotic-resistant genes (ARGs), (Bi, Ni)-based bimetallic metal-organic framework (MOF) with Z-scheme heterojunction was constructed. The visible light-driven (Bi, Ni)-bimetallic MOF generated H2O2 and other reactive oxygen species, efficiently eliminating 7.22 log antibiotic-resistant Escherichia coli and 7.16 log methicillin-resistant Staphylococcus aureus within 60 min. (Bi, Ni)-bimetallic MOF exhibited strong chemisorption via amino and benzene ring structures, synergistically enhancing the elimination o

Do variations in nasal irrigation recipes and storage effect the risk of bacterial contamination?

OBJECTIVE: Make-at-home nasal irrigation solutions are often recommended for treating chronic rhinosinusitis. Many patients will store pre-made solution for convenient use. This study investigated the microbiological properties of differing recipes and storage temperatures. METHOD: Three irrigation recipes (containing sodium chloride, sodium bicarbonate and sucrose) were stored at 5oC and 22oC. Further samples were inoculated with Staphylococcus aureus and Pseudomonas aeruginosa. Sampling and culturing were conducted at intervals from day 0-12 to examine for bacterial presence or persistence.

Food handlers and foodborne diseases: grounds for safety and public and occupational health actions.

One of the causes of foodborne diseases is contamination during food preparation; food handlers carrying pathogens might be involved in the origin of this condition. This problem is more serious in health care facilities, since consumers of contaminated food are already ill. Therefore, operational procedures should be formulated to prevent incidents. We performed a review of scientific studies, legislation and regulations on this subject. The results indicate that the main causes of food contamination involving food handlers to be considered in strategies to reduce, or even eliminate foodborne

Determination of worst-case cephalosporin residues in laboratory glassware using a validated low-level RP-HPLC method.

Cephalosporin antibiotics are highly allergenic and present serious cross contamination problems in analytical and manufacturing environments. Regulatory agencies require quality control laboratories to establish strict zero-tolerance or health-based carryover exposure limits, making verification of equipment and glassware cleanliness extremely important. The aim of this study was to assess the efficiency of cleaning laboratory glassware procedures by identifying the level of residual contamination of a worst-case cephalosporin antibiotic, cefixime. A validated reverse-phase liquid chromatogra

Bloodborne Viral Pathogen Contamination in the Era of Laboratory Automation.

BACKGROUND: The CDC states that laboratory testing for persons under investigation for Ebola virus disease can be safely performed using automated laboratory instruments by adhering to bloodborne pathogen practices. We therefore sought to investigate the levels of viral contamination of a total laboratory automation (TLA) system to guide risk mitigation strategies for handling infectious agents. METHODS: Environmental swabs followed by PCR for hepatitis B (HBV) and hepatitis C (HCV) viruses were taken from a chemistry TLA system during routine clinical use and after running a small number of

Persistence of the same strain of Legionella pneumophila in the water system of an Italian hospital for 15 years.

In 2004, an outbreak of legionnaires disease occurred in a hospital in northern Italy with a water system that had been disinfected multiple times since 1990 and equipped with a continuous disinfecting system. Molecular typing linked the outbreak to contamination of the hospital water system and demonstrated the persistence of a predominant strain of Legionella pneumophila for 15 years.

AIDS risk and risk reduction in the radiology department.

As the AIDS epidemic progresses, concern about the risk of occupational transmission of the causative organism, human immunodeficiency virus (HIV), is increasing. In this article, we summarize the risk of occupational acquisition of HIV in the health care setting and specify protocol and equipment that can reduce this risk in the radiology department. Accidental needle-stick injury is the most common form of exposure to infected blood, which is the only body fluid implicated to date in the occupational transmission of HIV. Prospective cohort studies demonstrate a 0.3-0.4% risk of infection for

Traditional and low-cost technical approaches for investigating greenhouse gases and particulate matter distribution along an urban-to-rural transect (Greve River Basin, Central Italy).

Human activities, largely tied to fossil fuels and intensive agriculture, emit massive amounts of climate-altering species and harmful pollutants into the atmosphere that affect soil, ecosystems, and water. Air quality monitoring is crucial to minimize harmful effects and protect human and environmental health. The Greve River basin (Tuscany, central Italy) represents an excellent example of an ecosystem affected by various anthropogenic air contaminants. The upstream areas are predominantly rural, while the downstream zones are characterized by urban and industrial development. Air pollutants

Nosocomial infections from contaminated endoscopes: a flawed automated endoscope washer. An investigation using molecular epidemiology.

Approximately 1 year after purchase of one manufacturer's automated endoscope washing machine, we began to detect heavy contamination of upper gastrointestinal (UGI) endoscopes cultured after cleaning and disinfection in the washer. During the first 6 months of 1988, 77% of surveillance cultures (20-mL flush through the biopsy channel) were positive for gram-negative bacilli (median concentration, 10(5) cfu/mL), most frequently Pseudomonas aeruginosa serotype 10. During the first 19 months of use of the washer, nosocomial post-UGI endoscopy colonization or infections with P. aeruginosa increas

Rapid screening of ethylene glycol and diethylene glycol in raw materials and medicinal syrups using low-cost field deployable assays.

There have been hundreds of child deaths due to contamination of medicinal syrups with diethylene glycol (DEG) and ethylene glycol (EG). Detection of DEG and EG is usually performed by gas chromatography, a method that is costly, laborious, time-consuming, and not readily available in many low- and middle-income countries (LMICs). Thin-layer chromatography is relatively lower cost and portable; however, as with gas chromatography, it requires time and trained personnel. Alternative rapid, low-cost and simple methods to determine DEG/EG contamination are needed. We tested the suitability of enz

Noninvasive ventilation in patients with COVID-19 from the perspective of the risk of contamination: a narrative review.

INTRODUCTION: noninvasive ventilation (NIV) can be a useful resource to treat acute respiratory failure (ARF), which occurs in patients with COVID-19. However, it is important to consider that there are still no clinical studies that have verified the safety of its use in increase of contamination. AREAS COVERED: Given the potential benefits and simultaneous concerns over the use of NIV in patients with COVID-19, further inquiry is necessary to reach a clinical consensus and provide recommendations for safe use, avoiding contamination. In this context, this narrative review, which included ar

Listeriosis associated with pre-prepared sandwich consumption in hospital in England, 2017.

A case of listeriosis occurred in a hospitalised patient in England in July 2017. Analysis by whole genome sequencing of the Listeria monocytogenes from the patient's blood culture was identified as clonal complex (CC) 121. This culture was indistinguishable to isolates from sandwiches, salads and the maufacturing environment of Company X which supplied these products widely to the National Health Service. Whilst an inpatient, the case was served sandwiches produced by this company on 12 occasions. No other cases infected by this type were detected in the UK between 2016 and 2020. Between 2016
