Multiple cannulated screw fixation of femoral neck fractures with comminution in young- and middle-aged patients.

OBJECTIVE: To investigate the distribution and influence of comminution in femoral neck fracture (FNF) patients after cannulated screw fixation (CSF). METHODS: From January 2019 to June 2020, a total of 473 patients aged 23-65 years with FNF treated by CSF were included in the present study. Based on location of the cortical comminution, FNF patients were assigned to two groups: the comminution group (anterior comminution, posterior comminution, superior comminution, inferior comminution, multiple comminutions) or the without comminution group. The incidence of postoperative complications, qu

Comminuted Distal Radius Fracture with Concomitant Median Nerve Transection: A Case Report and Literature Review.

CASE: A 41-year-old woman sustained a distal radius fracture with concomitant median nerve transection. The wound was consistent with an inside-to-outside mechanism with noted metaphyseal bone loss at the level of the median nerve injury. The median nerve was not amenable to primary repair, and given concern for inability to define the zone of the injury, the patient underwent delayed reconstruction of the nerve. CONCLUSION: A review of the literature highlights the rare incidence of this combined injury that can be undetected or misdiagnosed as an acute carpal tunnel syndrome or stretch inju

Role of open cerclage wiring in patients with comminuted fractures of the femoral shaft treated with intramedullary nails.

INTRODUCTION: The role of open cerclage wiring in comminuted femoral shaft fracture treatment with intramedullary nails remains unclear. Here, we analyzed the effect of open cerclage wiring and the risk factors for nonunion after interlocking nailing in comminuted femoral shaft fracture treatment. We hypothesized that open cerclage wiring can be applied in patients with severe comminuted femoral shaft fractures without affecting bone healing. PATIENTS AND METHODS: This retrospective cohort study used data from consecutive patients who underwent interlocking nail fixation of a comminuted femor

Minimally invasive percutaneous plate osteosynthesis for distal radius fractures with long-segment metadiaphyseal comminution.

INTRODUCTION: Distal radius fractures with both metaphyseal and diaphyseal comminution are commonly encountered injuries due to high-energy trauma. However, effectively treating patients with this disease remains challenging for the surgeon. HYPOTHESIS: The goal of this study was to evaluate the outcomes of minimally invasive percutaneous plate osteosynthesis (MIPPO) technique for distal radius fractures with long-segment metadiaphyseal comminution. MATERIAL AND METHODS: Nine patients with distal radius fractures involving long-segment metadiaphyseal comminution were treated with MIPPO from

Primary subtalar joint arthrodesis with internal and external fixation for the repair of a diabetic comminuted calcaneal fracture.

Comminuted, intra-articular calcaneal fractures can cause severe lower extremity impairment and have devastating effects on a patient's well being. Diabetes is a multisystem process that may cause neuropathy and loss of protective sensation further complicating the prognosis. Not all calcaneal fractures are created equal and when considering the patient's overall presentation and extent of injury, the combined approach of internal and external fixation for fracture reduction may be beneficial for restoration of anatomic alignment and function.

Comparison of plate and screw fixation and screw fixation alone in a comminuted talar neck fracture model.

BACKGROUND: Talar neck fracture fixation has been studied in noncomminuted fracture models, but no large clinical series of comminuted fracture patterns have been published and no biomechanical studies have compared plate fixation with screw fixation in comminuted talar neck fractures. METHODS: Nine matched pairs of fresh frozen talar specimens were stripped of soft tissue and mounted in a cylindrical jig. The talar neck was fractured using a dorsally directed shear force at a rate of 200 mm/min, and dorsal comminution was simulated by removing a 2-mm section of bone from the distal fracture

[Resection of the radius head in multiple and comminuted fractures. Clinical results and critical evaluation compared with the literature].

Between 1974 and 1989 39 patients with multi-fragmentary fractures of the head of the radius or radial neck fractures were treated by resection of the radial head at the Department of Trauma and Reconstructive Surgery (UKE-University Hospital Hamburg). Follow-up of 22 patients after 0.5 to 15 years (average 7.8 years) by means of patient documents, actual condition, functionally re-examination of the upper limb and evaluation of the roentgenological changes revealed 22.8% good, 63.6% acceptable and 13.6% poor results according to a modified scoring system. Characteristic complications after ex

Comminuted intra-articular fracture of the distal radius--results of early open reduction and internal fixation.

Comminuted intra-articular fracture of the distal radius remains a challenging problem. This article reviews the results of 15 consecutive cases which were treated with open reduction and internal fixation with T-plate after failing to achieve articular congruency with closed reduction. The follow-up ranges from 20 months to 28 months. In thirteen cases, articular congruency was restored. However, 4 of these 13 cases later lost some of the articular congruency. This suggests the need for supplementary bone grafting. Reduction of the extra-articular portion of distal radius was generally well m

Open reduction and internal fixation of displaced, comminuted intra-articular fractures of the distal end of the radius.

The results in sixteen patients who had a displaced, comminuted intra-articular fracture of the distal end of the radius and who were treated by open reduction and internal fixation were retrospectively reviewed. At a mean follow-up of 4.8 years, 81 per cent of the patients had a rating of good or excellent by the scoring system of Gartland and Werley, but only 56 per cent had such a rating when the modified scoring system of Green and O'Brien was used. All of the fractures healed at an average of nine weeks. A step-off of two millimeters or more in the distal radial articular surface at the t

Intramedullary Threaded Nail Versus Dorsal Plate and Screw Fixation of Comminuted Metacarpal Shaft Fractures: A Cadaveric Biomechanical Study.

PURPOSE: Indications are expanding for the use of intramedullary threaded nail (IMTN) fixation in metacarpal fractures. We hypothesized that IMTN fixation would have superior bending stiffness compared to dorsal plate and screw fixation and provide sufficient stability to withstand cyclic loading comparable with early range of motion. METHODS: Index, long, ring, and small finger metacarpals were harvested from matched pair cadaveric hands. A midshaft 1.75 mm transverse gap was created to simulate a comminuted fracture. Specimens were randomized into two groups for fixation with either a retro

Hybrid fixation with a double-row suture and low-profile plate for comminuted or avulsed greater tuberosity fractures.

BACKGROUND: Isolated greater tuberosity (GT) fractures account for approximately 20% of proximal humeral fractures and are frequently associated with glenohumeral dislocation and rotator cuff injuries. Displacement exceeding 5 mm is recommended for surgery to prevent subacromial impingement and functional impairment. Current fixation methods, such as cannulated screws, suture anchors, and locking plates, each have limitations, including inadequate stability for comminuted fractures, risk of impingement, and suboptimal anatomical adaptation. This underscores the necessity of a hybrid technique

Radiographic outcomes of dorsal spanning plate with and without volar plate fixation for comminuted intraarticular distal radius fractures.

PURPOSE: Dorsal spanning plate fixation for comminuted intra-articular distal radius fractures involves indirect reduction via ligamentotaxis, potentially resulting in suboptimal restoration of native anatomy. Displaced volar fragments may necessitate separate buttress support with a volar plate. This objective of this study was to retrospectively compare radiographic outcomes between distal radius fractures managed with dorsal spanning plate fixation alone versus dorsal spanning plate fixation with concomitant volar plating. METHODS: A retrospective review identified 51 distal radius fractur

Volar locking and angular stability plate for treatment of comminuted scaphoid fractures: a case series of 44 cases.

BACKGROUND: The primary method employed worldwide for the treatment of scaphoid fractures is screw fixation. However, in unstable and comminuted fractures, percutaneous fixation could produce complications due to technical challenges, such as improper axis positioning, inaccurate screw length measurement, intra-articular screw penetration, and impingement. Alternative open approaches for the surgical management of scaphoid fractures have been proposed, and in recent years, a new specific volar locking plate for the treatment of scaphoid fractures has been developed. This study aims to present

Effect of Implant Linkage on Axial and Rotational Stiffness of Nail-Plate Constructs for Comminuted Distal Femoral Fractures.

OBJECTIVES: To determine the effect of linking the intramedullary nail and the laterally applied locking plate to treat comminuted distal femur fractures and allow for immediate weight bearing. METHODS: Comminuted extra-articular distal femur fractures were created in 16 synthetic osteoporotic femurs and split into 2 groups: linked versus unlinked. In the linked construct, in addition to standard plate fixation and proximal locking of the nail, 2 nonthreaded locking bolts (prototypes) were placed through both the plate and the nail. In the unlinked construct, the same number of screws affixed

Comminuted Dorsal Ulnar Fragment in Distal Radius Fractures Treated Using the Integrated Compression Screw With a Mini-Plate.

Stabilization for displaced dorsoulnar fragments in distal radius fractures is challenging to treat with conventional volar locking plates alone. The integrated compression screw combined with a volar locking plate has been introduced as an additional tool to stabilize the dorsoulnar fragment and has been reported to work effectively. However, the compression screw is unable to stabilize a comminuted dorsal ulnar fragment; therefore, it is necessary to consider using an additional dorsal plate. We have developed a modified surgical technique to stabilize a comminuted dorsal intra-articular fra

Management of comminuted inferior patellar pole fractures with cerclage-wire-augmented separate vertical wiring: a retrospective clinical study.

PURPOSE: Comminuted inferior patellar pole fractures are challenging injuries and require effective treatment due to the extension mechanism of the knee. This study aims to evaluate the outcome of above fractures treated with a modified technique of cerclage-wire-augmented separate vertical wiring (SVW) with cerclage wiring passed through the proximal patella. METHODS: Retrospective analysis (1/2017-1/2020) were performed for patients that were treated with three SVWs through the posterior margin of proximal fragment and directly to the anterosuperior border of patella, combined with a cercla

Effect of inherent tibial asymmetry on leg length discrepancy measurements after intramedullary nailing of comminuted femoral shaft fractures.

INTRODUCTION: Locked intramedullary nailing (IMN) is the standard treatment for femoral shaft fractures in adults with high rates of union and relatively low rates of complications. Leg length discrepancy (LLD) after IMN of femoral shaft fractures is common, and is reported in 20-43% of cases. A known surgical challenge when trying to obtain equal leg lengths is comminuted fracture, which results in a loss of bony landmarks that guide reduction. The purpose of this study was to assess the effect of inherent tibial asymmetry on LLD measurements after IMN. METHODS: Postoperative CT scanograms w
