Nanofibrous nerve conduit-enhanced peripheral nerve regeneration.

Fibre structures represent a potential class of materials for the formation of synthetic nerve conduits due to their biomimicking architecture. Although the advantages of fibres in enhancing nerve regeneration have been demonstrated, in vivo evaluation of fibre size effect on nerve regeneration remains limited. In this study, we analyzed the effects of fibre diameter of electrospun conduits on peripheral nerve regeneration across a 15-mm critical defect gap in a rat sciatic nerve injury model. By using an electrospinning technique, fibrous conduits comprised of aligned electrospun poly (ε-capr

Management of the exposure of e-PTFE membranes in guided bone regeneration.

UNLABELLED: Guided bone regeneration (GBR) has been used extensively since its introduction in the late 1980s. In such treatment, e-PTFE membranes are often preferred because of their inert biological features and the predictability of the barrier effect. One complication of GBR is the premature exposure of the membrane. A consequence of the resulting soft tissue dehiscence is compromised bone regeneration. There are situations in which an exposed membrane can be maintained and obtain satisfactory results. This article discusses suggested guidelines for the treatment of exposed e-PTFE membrane

Efficacy of high-density versus semipermeable PTFE membranes in an elderly experimental model.

OBJECTIVE: The clinical effectiveness of expanded polytetrafluoroethylene (PTFE) membranes for guided bone regeneration has been reported in the literature on several occasions. However, one major drawback of this material is the need for a second surgical operation for removal of the membrane. In addition, most studies involving guided bone regeneration in animal models have used young animals or animals of unspecified age, but in some clinical situations guided bone regeneration may be indicated in elderly jaw bones. The objectives of this investigation were (1) to evaluate the effectiveness

Restoration of mandibular nonunion bone defects. An experimental study in rats using an osteopromotive membrane method.

Standardized through-and-through critical size defects were created in rat mandibles. After 12 weeks, the sites revealed a massive ingrowth of soft connective tissue, forming a transosseous core filling the defects. Upon reentry, the soft tissue inside the remaining bone defects was removed. On one side of the jaw, the defect was covered both buccally and lingually with an expanded polytetrafluoroethylene (e-PTFE) membrane, but on the other side no membrane was placed. Histologic analysis after 6 weeks revealed an essentially complete healing with bone of the membrane-covered defects. No carti

Periodontal repair in dogs: expanded polytetrafluoroethylene barrier membranes support wound stabilization and enhance bone regeneration.

A wound stabilizing effect of expanded polytetrafluoroethylene (ePTFE) membranes was evaluated in supra-alveolar periodontal defects in 5 beagle dogs. The defects, 5 to 6 mm in height, were surgically created around the 2nd, 3rd, and 4th mandibular premolar teeth in contralateral jaw quadrants. The root surfaces were conditioned with heparin, which, in this model, has been demonstrated to compromise periodontal healing and result in formation of a long junctional epithelium. Wound closure included application of ePTFE membranes around each premolar tooth in one jaw quadrant in each dog and fla

Multifunctional MXene/PVDF nanofiber membrane with electroactivity and mild photothermal effect for guided bone regeneration.

Bone defects have become a major challenge restricting the long-term success of dental implant restoration. Conventional barrier membranes often exhibit limited capability in guided bone regeneration, particularly in severe defect cases. To address these issues, a novel multifunctional MXene/PVDF nanofiber membrane was fabricated via electrospinning. First, the membrane possesses favorable mechanical and physical barrier properties, effectively preventing the invasion of soft tissue cells into the bone defect area while providing stable mechanical support. Second, the composite membrane is end

Guided bone regeneration therapy based on plaque control of peri-implantitis with follow-up at 7 years.

Peri-implantitis is a pathologic condition associated with dental plaque that occurs in the implant tissue and is characterized by inflammation of the mucous membrane surrounding the implant, followed by the progressive loss of supporting bone. In this study, a case of guided bone regeneration therapy based on plaque control of peri-implant inflammation was reported. Four years after surgery for the left second premolar implant, the patient presented with "left lower posterior tooth swelling and discomfort for more than 2 years". The X-ray periapical film showed a decrease in distal bone miner

Gelatin hydrogel with basic fibroblast growth factor for tympanic membrane regeneration.

HYPOTHESIS: In this developmental research study that aimed to develop tympanic membrane regeneration therapy, we devised a method of sustained bFGF formulation release using gelatin hydrogel in a guinea pig eardrum perforation model. BACKGROUND: Basic fibroblast growth factor (bFGF) can promote perforation closure. In addition, several studies of bFGF formulations have used gelatin hydrogel-bFGF coupled electrostatically to a gelatin polymer. BFGF is released gradually as a result of degradation of the gelatin polymer, and studies have shown that the long-term pharmacologic effects of bFGF c

Functional recovery of denervated muscle by neurotization using nerve guidance channels.

Tissue-engineered muscle has been proposed as a means of repairing volumetric muscle defects to restore anatomical and functional recovery. We have previously demonstrated that denervated muscle, which is analogous to engineered muscle construct, can be reinnervated by direct transplantation of host nerve (neurotization) in a rat model. However, the use of this approach is not possible if the length of host nerve is inadequate and cannot be mobilized to the insertion site of the engineered muscle. In this study we investigated whether neurotization coupled with nerve guidance channels would in

Microsurgical reconstruction of the trigeminal nerve.

Head and neck tumor surgery or traumatic injuries in the maxillofacial region often result in discontinuity defects of peripheral branches of the trigeminal (fifth cranial) nerve, causing loss of sensation to those areas of the face, mouth, or jaws supplied by this important nerve. Injuries to the peripheral branches of the trigeminal nerve can be repaired by microsurgical techniques, either at the time of the original injury or ablative operation if conditions are favorable, or at a later date. Repair of a peripheral nerve injury has a good chance of a satisfactory outcome if done in a timely

Collagen nerve protector in rat sciatic nerve repair: A morphometric and histological analysis.

Peripheral nerve repair is often complicated by fibroblastic scar formation, nerve dysfunction, and traumatic neuroma formation. Use of bio-absorbable protective wraps may improve outcomes of these repairs. This study histologically compared the incidence of neuroma formation, connective tissue proliferation, and axonal regrowth in transected rat sciatic nerves repaired with and without tubular collagen nerve sleeves. Twenty Sprague-Dawley rats underwent unilateral sharp sciatic nerve transection and microscopic nerve repair with four epineural sutures and were randomly treated with or without

Effect of different microstructures of e-PTFE membranes on bone regeneration and soft tissue response: a histologic study in canine mandible.

The purpose of the present study was to compare the effect of 3 types of titanium reinforced e-PTFE membranes (GTRM #1, #2, and #3) on bone regeneration and tissue integration. GTRM #1 was a standard titanium reinforced membrane, GTRM #2 was a prototype characterized by a more open total surface microstructure and GTRM #3 was a prototype characterized by two extremely porous external layers separated by a totally occlusive internal layer. The membranes were used to cover saddle type ridge defects surgically created in the mandible of 6 dogs. After a healing period of 6 months, all the membrane

An unusual case report of peri-implant cystic lesion.

A new entity called "peri-implant cyst" has recently been described. This lesion is an inflammatory odontogenic cyst on an osseointegrated implant. It arises from inflammatory stimulation and proliferation of the rests of Malassez. Although a relatively rare condition but potentially the cause of implant failure. Peri- implant cystic lesion exhibits the same radiographic and histopathological features as the radicular cyst. The diagnosis rests on the histology, radiology, and on their correlation with clinical features, in particular the location of the lesion. Herein, we report a case of a 58

Hard and soft tissue augmentation in implant surgery: a case report.

Correct pre- and postimplant hard and soft tissue management is prerequisite to achieving optimal esthetic and functional outcomes after implant-prosthesis treatment. Various different methods for preimplant hard tissue augmentation in alveolus maxillary atrophy are described. In some cases, non-resorbable titanium-reinforced membranes are employed for vertical augmentation of the edentulous crest. Also well documented is soft tissue management through mucus-gingival surgery techniques that increase the thickness of peri-implant keratinized gingiva, enhancing soft tissue esthetic and health. T

Regeneration of injured skin and peripheral nerves requires control of wound contraction, not scar formation.

We review the mounting evidence that regeneration is induced in wounds in skin and peripheral nerves by a simple modification of the wound healing process. Here, the process of induced regeneration is compared to the other two well-known processes by which wounds close, i.e., contraction and scar formation. Direct evidence supports the hypothesis that the mechanical force of contraction (planar in skin wounds, circumferential in nerve wounds) is the driver guiding the orientation of assemblies of myofibroblasts (MFB) and collagen fibers during scar formation in untreated wounds. We conclude th

Biostable scaffolds of polyacrylate polymers implanted in the articular cartilage induce hyaline-like cartilage regeneration in rabbits.

PURPOSE: To study the influence of scaffold properties on the organization of in vivo cartilage regeneration. Our hypothesis was that stress transmission to the cells seeded inside the pores of the scaffold or surrounding it, which is highly dependent on the scaffold properties, determines the differentiation of both mesenchymal cells and dedifferentiated autologous chondrocytes. METHODS: 4 series of porous scaffolds made of different polyacrylate polymers, previously seeded with cultured rabbit chondrocytes or without cells, were implanted in cartilage defects in rabbits.
