Mitral regurgitation (MR) is the most prevalent valvular heart disease in the Western world. Surgical repair is the gold standard for the treatment of degenerative MR in low-risk patients. Given the rising number of patients with functional MR and higher operative risk due to relevant comorbidities and increasing age, interventional approaches to repair or replace diseased mitral valves are on the rise. However, the complex anatomy and physiology of the mitral valve and its adjacent valve apparatus bear major challenges. To date, only the MitraClip device has been used in a large number of pat

A 20-month-old girl presented with severe dilated cardiomyopathy and decompensated congestive cardiac failure. Despite escalating inotropic and mechanical ventilation support, she required placement on extracorporeal membrane oxygenation and transfer to the transplant centre in Newcastle, England. She was placed on biventricular assist device and then Berlin Heart but failed to show any recovery of ventricular function. She underwent orthotopic heart transplantation at 2 years of age. She developed bacterial endocarditis with Enterococcus faecalis resulting in severe aortic valve regurgitation

Modified button technique for reattachment of coronary arteries in the aortic root replacement is reported. Anastomosis of the coronary buttons is performed from the inside of the composite valve graft previously including the coronary buttons in the composite valve graft. Reduced tension is present between coronary arteries and the composite valve graft once the heart is beating and the systemic pressure is increasing. In this way coronary buttons are reinforced directly by the composite aortic wall graft prosthesis. The coronary ostia are perfused with lower tension at the site of the corona

Prosthetic valve thrombosis (PVT) is a life-threatening disease, for which treatment strategies have been controversial. Herein, existing data on management options are reviewed, and conclusions drawn as to the choice and use of treatment strategies for PVT. The use of transesophageal echocardiography (TEE) allows distinction to be made between obstructive and non-obstructive PVT by the presence or absence of occluder motion limitation. The differentiation of PVT from pannus and vegetation is, however, still limited by TEE. The incidence of PVT has been underestimated by not taking into accoun

BACKGROUND: There are several high-risk treatment options for valve failure of a biological full root replacement. When tailoring the best treatment option for the patient, implantation of a rapid deployment valve (RDV) should be considered.
CASE PRESENTATION: Six patients presented with aortic regurgitation in a full root Freestyle bioprosthesis. Three had a history of valve endocarditis, while the remaining had non-infectious structural valve deterioration with leaflet and commissural tears. All patients were treated with a rapid deployment EDWARDS INTUITY Elite valve. Follow-up was complet

This paper reports the early results of combined aortic and mitral valve replacement in 39 patients, who underwent operation with the convexo-concave Björk-Shiley prosthesis. There were no intra-operative deaths. Two patients (5%) died while in hospital of myocardial failure and left ventricular rupture, respectively. One late death (3%) was due to a thrombosis of the mitral prosthesis (N.Y.H.A. class IV at operation). Cumulative follow-up (2-31 months) was 100%. One patient had radial artery embolization while converting to atrial fibrillation. Another patient required repair of an aortic par

BACKGROUND: Extensive evidence relating to transcatheter aortic valve replacement (TAVR) has accumulated in recent years, but mid-term outcomes are less reported. We investigated 996 patients after implantation of the CoreValve prosthesis for severe aortic stenosis in a real-world setting.
OBJECTIVE: To report clinical and echocardiographic 3-year results from the ADVANCE study.
METHODS: ADVANCE is a prospective, multicenter, fully monitored, nonrandomized clinical study. This analysis assessed valve-related events, predictors of early and mid-term mortality after TAVR, and systolic and dias

Review of 1,684 cases of isolated aortic or mitral valve replacement with a Starr-Edwards prosthesis demonstrated that the procedure provides improved life expectancy over that found in the natural history of valvular heart disease. Further improvement in results depends on continued reduction in operative and late mortality and in the incidence of thromboembolism. Advanced preoperative functional class, atrial or ventricular enlargement, a history of prior heart surgery, advanced age at operation and untreated valvular disease were among the factors related to increased early or late mortalit

A variety of conditions can lead to left ventricle outflow tract obstruction, but cases of subaortic stenosis decades following a mitral valve replacement are exceedingly rare. Any abnormal positioning of a prosthetic valve can result in continuous turbulence leading to permanent deposition of fibrous tissue. We present a case of a 56-year-old female that underwent mechanical mitral valve replacement, due to severe rheumatic mitral valve disease, with recurrent admissions for dyspnea. Ultimately, she underwent a left heart catheterization and during pullback from the left ventricle to the aort

From March 2002 to August 2005, 53 patients with age between 30 and 86 underwent surgical treatment for aortic valve disease. Preoperative diastolic heart failure was observed in 15 cases (28.3%). Operative procedures consisted of aortic valve replacement (AVR) in 42 cases [AVR and mitral valve replacement (MVR) in 3], aortic valve plasty (AVP) in 2, and aortic root replacement in 4. Concomitant procedures included maze procedure in 2 cases, coronary artery bypass grafting (CABG) in 6, mitral valve surgery in 15, and tricuspid valve annuloplasty (TAP) in 8. There were 7 cases for patient-prost

Management of a pre-existing mechanical mitral valve prosthesis during durable left ventricular assist device (LVAD) implantation remains controversial. Retaining the prosthesis may predispose to leaflet immobility, thrombosis, and inflow obstruction, whereas full prosthetic replacement adds substantial operative risk, particularly in redo cardiac surgery. We describe a novel surgical strategy combining removal of the mobile prosthetic components with preservation of the sewing ring during HeartMate 3 implantation. A 49-year-old woman with advanced heart failure and previous mechanical mitral

Almost half a century ago, cardiac surgeons introduced the concept of prosthesis-patient mismatch (PPM), which was defined by indexing the effective orifice area (EOA) to body surface area. Although PPM has demonstrated strong prognostic value following surgical aortic valve replacement (SAVR), its significance in transcatheter aortic valve replacement (TAVR) remains uncertain. In this review, we revisit the concept of PPM and explore the methodological and clinical challenges in its application. Special focus is given to the limitations in interpreting EOA and indexed EOA (EOAi), including fl

One of the complications of tricuspid valve replacement (TVR) is the complete heart block (CHB). In these patients an epicardial permanent pacemaker is frequently used but its insertion is another major operation and higher thresholds are needed. Two patients are reported, both women, with rheumatic heart disease and TVR who required a permanent pacemaker because they developed CHB. The first patient underwent mitral valve replacement with a disc valve seventeen years before and TVR recently. A single chamber pacemaker was implanted. Left ventricular pacing was achieved through the great cardi

The Omniscience prosthetic heart valve has been clinically evaluated in 44 patients operated on between January and July, 1981. There were 23 aortic valve replacements (AVR), 18 mitral valve replacements (MVR) and 3 double valve replacements (DVR). Mean age was 48 +/- 11 years. Preoperatively, 61% patients were in New York Heart Association (NYHA) functional class IV, 36% in class III and 2% in class II. The hospital mortality rate was 4.5%. Actuarial freedom from thromboembolic events was 97.4 +/- 2.6% (3 1/2 years of follow-up). Postoperatively, 93% of the patients were in NYHA class I and 7

The surgical treatment of mitral regurgitation has been completely revamped in the last years. The development of reproducible and durable plasty techniques has made mitral valve replacement less and less frequent. The virtual disappearance of rheumatic valve disease in developed countries has made degenerative disease the most frequent etiology of mitral insufficiency. Furthermore the outlook of this pathology has been modified by the early surgical treatment. The following article presents the surgical techniques available today and their indications in the different types of mitral insuffic

The On-X valve is a newer generation mechanical bileaflet valve. Its key features include the use of pure pyrolytic carbon (devoid of silicon), a length-to-diameter ratio similar to a native valve, an inlet flared orifice, a leaflet opening up to 90 degrees, a shorter leaflet closing angle, a 2-point leaflet contact, and an actuated pivot. These features have translated into increased strength, improved valve hemodynamics, reduced hemolysis, and thrombogenicity.
