Autopsy-Confirmed Giant Pleural Hematoma Due to Spontaneous Hemothorax in ALK-Rearranged and TP53-Mutant Lung Adenocarcinoma: A Case Report.

Spontaneous hemothorax, defined as bleeding into the pleural cavity without trauma or coagulopathy, is rare. Especially, it is uncommon in patients with primary epithelial lung cancer. We report a rare autopsy case of lung adenocarcinoma harboring both an anaplastic lymphoma kinase (ALK) rearrangement and a tumor protein p53 (TP53) mutation, in which hemothorax led to the formation of a giant pleural hematoma.A 75-year-old man with stage IVB lung adenocarcinoma presented with mediastinal and cervical lymph node metastases and right pleural effusion at diagnosis. Molecular testing revealed an A…

Delayed Hemothorax Due to Rupture of a Right Inferior Phrenic Artery Pseudoaneurysm 67 Days After Rib Fractures: A Case Report.

Delayed hemothorax after blunt chest trauma usually develops within seven days, and the inferior phrenic artery (IPA) is a rare culprit vessel. We report a case of massive hemothorax that occurred 67 days after rib fractures due to rupture of an IPA pseudoaneurysm. A 73-year-old man was transported in shock with a five-day history of right hypochondrial pain. Non-contrast CT revealed a right pleural effusion with attenuation consistent with blood and multiple fractures of the right 8th-12th ribs, while contrast-enhanced CT demonstrated contrast extravasation adjacent to the right 10th rib. Ang…

Late video-assisted thoracoscopic surgery versus thoracostomy tube reinsertion for retained hemothorax after penetrating trauma, a prospective randomized control study.

BACKGROUND: Early video-assisted thoracoscopic surgery (VATS) is the recommended treatment of choice for retained hemothorax (RH). A prospective single-center randomized control study was conducted to compare outcomes between VATS and thoracostomy tube (TT) reinsertion for patients with RH after penetrating trauma in a resource constrained unit. Our hypothesis was that patients with a RH receiving VATS instead of TT reinsertion would have a shorter hospital stay and lesser complications. MATERIALS AND METHODS: From January 2014 to November 2019, stable patients with thoracic penetrating traum…

Optimal Initial Positioning of Chest Tubes to Prevent Retained Hemothorax Using a Novel Steerable Chest Tube With Extendable Infusion Cannula.

INTRODUCTION: With blunt and penetrating trauma to the chest, warfighters and civilians frequently suffer from punctured lung (pneumothorax) and/or bleeding into the pleural space (hemothorax). Optimal management of this condition requires the rapid placement of a chest tube to evacuate as much of the blood and air as possible. Incomplete drainage of blood leading to retained hemothorax may be the result of the final tube tip position not being in contact with the blood collections. To address this problem, we sought to develop a "steerable" chest tube that could be accurately placed or reposi…

Hemothorax due to injuries of subclavian artery and first intercostal artery after subclavian venous catheterization in a pediatric patient : A case report.

Central venous catheterization is useful to evaluate intravascular volume status, while it has a chance of severe complications such as tension pneumothorax, cardiac tamponade, and so on. Now, we report a case of hemothorax after trying subclavian venous catheterization over again in a 2-year-old patient undergoing encephalo-duro-arterio-synangiosis, that required surgical intervention at the end. Though arterial puncture and massive hemothorax is a possible complication of subclavian venous catheterization, the injury of first intercostal artery might be due to inappropriate introducer needle…

Outcomes of Video-assisted Thoracic Surgery-guided Early Evacuation of Traumatic Hemothorax: A Randomized Pilot Study at Level I Trauma Center.

INTRODUCTION: Traumatic hemothorax is accounted for about 20% of traumatic chest injuries. Although majority can be managed with the timely placement of intercostal tube (ICT) drainage, the remaining pose a challenge owing to high complication rates associated with retained hemothorax. Although various treatment modalities including intrapleural instillation of fibrinolytics, radioimage guided drainage, VATS guided evacuation and thoractomy do exist to address the retained hemothorax, but indications along with timing to employ a specific treatment option is still unclear and ambiguous. METHO…

Spontaneous hemothorax caused by concomitant low-dose rivaroxaban and itraconazole in a 95-year-old patient: case report and literature review.

Although direct-acting oral anticoagulants (DOACs) decrease the bleeding risk compared with vitamin K antagonists (VKAs), DOACs might cause spontaneous hemothorax in very elderly patients, even at a very low dose. Interactions between drugs might increase the risk of bleeding. In this article, we report a case of a 95-year-old man who developed spontaneous hemothorax while taking rivaroxaban 2.5 mg twice daily, 3 days after concomitant use of itraconazole. Rivaroxaban was discontinued, and thoracentesis was performed to drain grossly bloody pleural effusion. To our knowledge, this is the first…

Subclavian vein injury and massive hemothorax requiring thoracotomy following insertion of tunneled dialysis catheter--a case report and review of literature.

Tunneled dialysis catheters are widely used for hemodialysis. Large caliber dilator is used for insertion of such catheter which can result in serious vascular injury leading to haemothorax. Here we report such an inadvertent central venous injury requiring thoracotomy following blind dilator advancement along with review of literature and recommendation to prevent such complication.

[Hemothorax disclosing hemorrhagic necrosis of a pheochromocytoma: circumstance for detection of multiple endocrine neoplasia type IIa. Detection of 5 familial cases].

Whilst investigating 26 members of the same family, we discovered 5 cases of multiple endocrine neoplasia type II a. The present report demonstrates the diagnostic value of basal plasma thyrocalcitonin (TCT) assays, before and after stimulation with pentagastrin, and of plasma carcinoembryonic antigen (CEA) assays. Some of the clinical features encountered were novel--e.g. in one patient the phaeochromocytoma was revealed by a haemothorax with cardiovascular collapse--and others were peculiar; thus, in 4 cases the medullary thyroid carcinoma (MTC) was less than 2 cm in diameter and without lym

Percutaneous pigtail catheter versus chest tube for the treatment of pediatric traumatic hemothorax: An Eastern Association for the Surgery of Trauma multicenter study.

BACKGROUND: Small percutaneously placed pigtail catheters (PCs) for traumatic hemothorax (HTX) are safe and effective in adults but have not been evaluated in children. We hypothesized that PC would have similar efficacy and complication rates compared with chest tubes (CTs). METHODS: A retrospective study of hemodynamically stable pediatric trauma patients (younger than 18 years) with HTX or hemopneumothorax was conducted at 41 trauma centers (January 2010 to December 2022). Catheter failure was defined as a requirement for surgery, additional tube placement, or thrombolytics. Multivariable …

Comparing Sensitivity and Specificity of Ultrasonography With Chest Radiography in Detecting Pneumothorax and Hemothorax in Chest Trauma Patients: A Cross-Sectional Diagnostic Study.

Background Thoracic trauma accounts for 20-25% of all traumas and is the third most frequent cause of death, after abdominal injury and head trauma. In the Emergency Department (ED), shifting an unstable patient to the X-ray room for detecting pneumothorax and hemothorax is always risky and bedside X-ray causes radiation exposure not only to the particular patient but also to the surrounding patients in a congested and busy ED. This can be avoided by using bedside ultrasonography (USG) as the initial imaging modality in chest trauma patients. Objective To compare the sensitivity and specificit…

Chest tube output, duration, and length of stay are similar for pneumothorax and hemothorax seen only on computed tomography vs. chest radiograph.

PURPOSE: Whole-body computed tomography (CT) for blunt trauma patients is common. Chest CT (CCT) identifies "occult" pneumo- (PTX) and hemothorax (HTX) not seen on chest radiograph (CXR), one-third of whom get chest tubes, while CXR identifies "non-occult" PTX/HTX. To assess chest tube value for occult injury vs. expectant management, we compared output, duration, and length of stay (LOS) for chest tubes placed for occult vs. non-occult (CXR-visible) injury. METHODS: We compared chest tube output and duration, and patient length of stay for occult vs. non-occult PTX/HTX. This was a retrospect…

Ipsilateral Recurrent Hemothorax Caused by Spontaneous Intercostal Artery Rupture Treated With Transcatheter Arterial Embolization and Video-Assisted Thoracoscopic Surgery: A Case Report.

Hemothorax caused by intercostal artery rupture is a potentially life-threatening disease. A 66-year-old woman presented to our emergency department with left back pain. She had undergone surgery for left hemothorax 14 years ago. Contrast-enhanced chest computed tomography revealed active bleeding from the left dorsal ninth intercostal artery near the descending aorta. Subsequently, she developed hypovolemic shock. Based on her medical history, the patient was suspected of having adhesions in the left thoracic cavity. Surgery was considered a time-consuming approach to control the bleeding. Ou…

Predictors of Blunt Thoracic Aortic Injury Requiring TEVAR in Patients with Left-Sided Hemothorax: Implications for Chest Drainage and Early CTA Assessment.

Background: Blunt thoracic aortic injury (BTAI) is an uncommon but life-threatening consequence of blunt thoracic trauma. Left-sided hemothorax is frequently identified during initial evaluation and typically prompts early chest drainage. However, when an unrecognized BTAI is present, pleural decompression may precipitate hemodynamic instability. This study aimed to identify early predictors of BTAI requiring thoracic endovascular aortic repair (TEVAR) in patients presenting with left-sided hemothorax. Methods: We conducted a single-center retrospective cohort study including consecutive traum…

Comparative effectiveness of suction versus water seal following tube thoracostomy in traumatic pneumothorax and hemothorax: An updated systematic review and meta-analysis.

BACKGROUND: Tube thoracostomy is commonly used in patients with chest trauma. However, optimal management after chest tube insertion remains controversial, particularly regarding the use of suction versus water seal systems. This updated meta-analysis evaluated the comparative effectiveness of suction and water seal management in traumatic pneumothorax and hemothorax patients. METHODS: A systematic search of PubMed, Embase, Web of Science, and Cochrane CENTRAL databases was performed from database inception to May 2026. Randomized controlled trials comparing suction with water seal after tube…
