INTRODUCTION: Different factors related to winter are known to influence the fracture incidence, but little is known about the effect of road surface temperature. This study examines the association between road surface temperature and the daily number of fractures in an urban area during two winters.
MATERIAL AND METHODS: Retrospective data collection was conducted on all patients treated at Bispebjerg Hospital, Denmark, for a humeral, ankle, distal radius or hip fracture during the periods October to April 2009/2010 and 2010/2011. Patients were grouped according to age into the following ca

PURPOSE: The aim of our study is to show the functional outcomes and complication rates of humeral complex fractures in adults, using osteosynthesis with two bridging orthogonal submuscular plates.
METHODS: The study consists of a prospective case series of 13 patients with isolated humeral complex fractures treated with two bridging orthogonal submuscular plates. Functional assessment was performed using disabilities of the arm, shoulder, and hand (DASH) score with 30 items. The age ranged from 22 to 68 years, with a mean age of 39 years. Functional assessment with DASH score was performed a

A 28-year-old male patient was referred to the Peripheral Nerve Injury Unit at the Royal National Orthopaedic Hospital for evaluation and treatment of a neuropathic type pain he had developed in his right arm. Some twenty years previously, he had sustained a Gartland type III supracondylar humeral fracture with what was thought clinically to be an anterior interosseous nerve palsy. The fracture was reduced and the nerve palsy subsequently recovered with non operative measures. At his subsequent delayed presentation, surgical exploration revealed that the median nerve epineurium was embedded wi

BACKGROUND: The use of cementless diaphysis-fitting stems has been shown to be an effective treatment option for cases of metadiaphyseal humeral fracture. Complex metadiaphyseal fractures are those that extend below the surgical neck into the diaphysis, which can compromise the metaphyseal fixation of typical arthroplasty designs. The presently described surgical technique circumvents the potential risks associated with the use of cement while also permitting the treatment of common concomitant pathologies, such as arthritis and rotator cuff tendinopathy. Evidence supports the efficacy of this

INTRODUCTION: Fractures of the proximal humerus are the second most common upper extremity fracture and the third most common fracture, after hip and distal radial fractures. The fractures can occur at any age, but the incidence rapidly increases with age. Common complications following non-operative management of proximal humeral are pain, stiffness, and loss of function. The following study was conducted to evaluate the functional and radiological outcome of displaced proximal humeral fractures treated with the proximal humerus internal locking osteosynthesis system (PHILOS).

INTRODUCTION: We describe the surgical technique for open reduction and internal fixation (ORIF) of proximal humeral fractures with a locking plate.
STEP 1 PREOPERATIVE PLANNING: To choose the right candidate, obtain a full understanding of the patient's fracture pattern, activity level and demands, and bone quality; be aware of predictors of complications and poor outcomes.
STEP 2 PATIENT POSITIONING: Place the patient in the beach-chair position with the arm draped free or in a hydraulic device with good access for the image intensifier.
STEP 3 APPROACH: The deltopectoral approach is gene

OBJECTIVE: To investigate whether a humeral intracondylar fissure (HIF) alters bone strain in the French Bulldog humerus, we developed a quantitative CT-based 3-D finite element (FE) model for virtual mechanical testing. We hypothesized that higher strains would be seen in the intracondylar region and lateral epicondylar crest if there was a HIF.
METHODS: Patient CT scans from 3 (n = 3) French Bulldogs were selected. Dog 1 had a closed distal physis and no HIF. Dog 2 had an open distal humeral physis but no HIF. Dog 3 had an open distal physis and a HIF. A 3-D FE model was built for FE analys

Isolated trochlea fracture in adults is a rare surgical entity as compared to its capitellar counterpart. It has been only mentioned sporadically in the literature as case reports. Fracture of the trochlea is accompanied by other elbow injuries like elbow dislocation, capitellum fracture, ulnar fracture and extraarticular condylar fracture. Here we report a unique case of isolated displaced trochlea fracture associated with fractures of the lateral end clavicle and the distal end radius. We propose a unique mechanism for this rare combination of injuries: typical triad of injury, i.e. fracture

Distal humeral intra-articular fractures often result in functional impairment if treated conservatively. These fractures are particularly challenging due to osteoporosis and severe comminution. A 32-year-old female with a grade three open distal humerus intra-articular fracture presented with a 5 cm x 5 cm open wound on her right elbow. The patient underwent open reduction and internal fixation (ORIF) using the BB Joshi external fixator and a local flap for skin coverage, achieving stable fixation and early mobilization. The patient regained a near-normal range of movements and adequate skin

OBJECTIVE: The optimal timing of pin removal following closed reduction and percutaneous pinning (CRPP) for pediatric Gartland type 3 supracondylar humerus fractures remains controversial. Although pins are commonly removed between the third and fifth postoperative weeks, high-level evidence comparing clinical and radiological outcomes between these time points is limited. This study aimed to evaluate whether pin removal at the third vs. the fifth postoperative week influences radiographic healing or functional outcomes.
METHODS: This retrospective observational study included 95 pediatric pa

BACKGROUND: Children and adolescents are vulnerable to the ongoing opioid overdose epidemic in the United States. To minimize pediatric patients' exposure to opioids, efforts are underway to reduce opioid prescriptions after closed reduction and percutaneous pinning (CRPP) for supracondylar humerus fractures. Standardized pain management protocols preferentially using opioid-free analgesics are reported to be effective in managing postoperative pain and limiting the utilization of opioids. This study compares retrospective data collected before and after the implementation of a standardized po

BACKGROUND: Extra-articular distal humerus fractures account for approximately 16% of all humerus fractures and 3% of adult fractures. These injuries pose significant treatment challenges due to the complex three-dimensional anatomy and the biomechanical demands placed on the distal humerus.
OBJECTIVE: The objective of this study was to evaluate the clinical and functional outcomes of extra-articular distal humerus fractures treated with locking compression plates.
MATERIALS AND METHODS: A prospective study was conducted on 90 patients with extra-articular distal humerus fractures who underw

INTRODUCTION: Radial nerve palsy is a recognized complication of humeral shaft fractures, often associated with distal-third spiral patterns described by Holstein and Lewis. However, authentic mechanical entrapment of the nerve at the mid-shaft level is rather rare. These events test the limits of normal anatomy and show how important it is to look into things right once when fracture geometry suggests possible nerve entrapment.
CASE REPORT: A 28-year-old right-handed female sustained a humeral fracture at the mid-shaft (AO 12-A3) following a collision with a two-wheeled vehicle. She showed e

BACKGROUND: The aim of the study was to design a convenient technique for dorsal minimally invasive plate osteosynthesis (MIPO) of extra-articular fractures of both distal thirds of the humeral shaft as well as to characterise the course and proximity of the radial nerve (RN) and the axillary nerve (AN).
METHODS: The collective consisted of 20 upper extremities of human adult body donors. A 3.5 mm Locking Compression Plate (LCP), an extra-articular distal humerus plate was inserted through a MIPO approach including two incisions. The primary incision was performed 5 cm in lenght on the dorsal

BACKGROUND: Forearm compartment syndrome is an uncommon sequela of distal radial fractures. This investigation tested the hypothesis that the risk of forearm compartment syndrome associated with an unstable, operatively treated fracture of the distal end of the radius is higher with a concomitant injury of the ipsilateral elbow.
METHODS: All patients who sustained an unstable fracture of the distal end of the radius and/or injury to the elbow (a fracture of the proximal end of the radius and/or ulna, simple elbow dislocation, elbow fracture-dislocation, or distal humeral fracture) and were op

A traumatic birth can cause significant upper limb injury; the presenting features are non-specific and the differential diagnosis long. Transphyseal fractures of the distal humerus are a rare but clinically important birth injury. This injury has typical radiographic findings, which due to the un-ossified nature of the distal humeral epiphysis can easily be misinterpreted. This article presents the radiographic appearance correlated with arthrography, ultrasound and MRI obtained from four cases of neonatal transphyseal fracture of the distal humerus. We hope that by demonstrating the appearan

BACKGROUND: A number of surgical approaches have been described for open reduction and internal fixation (ORIF) of intra-articular distal humerus (IDH) fractures in adults. However, there is no consensus as to which approach is better in terms of functional outcomes and complications. The purpose of this study was to determine whether the functional outcomes and types and rates of complications are influenced by the choice of surgical approach for ORIF of IDH fractures (AO/OTA types 13 B & C).
METHODS: A systematic review of literature was performed using the PubMed, EMBASE and Cochrane Datab

Open fractures, transverse, short oblique and spiroid fractures of the humeral shaft, as well as comminuted fractures with radial palsy or vascular injury, mostly lead to bad end-results if treated conservatively. The same is valid in the case of bilateral humeral shaft fractures, multiple injuries, polytrauma, pathologic fractures and pseudarthrosis. Good end-results and a low rate of complications in the operative procedure require an adequate approach to the fractured limb as well as a meticulous care of the soft tissues. In plate osteosynthesis, the anterolateral approach for the proximal

PURPOSE: To determine the feasibility of using ultrasonography (US) for evaluation of a radial nerve injury associated with humeral shaft fracture.
MATERIALS AND METHODS: In a prospective study, 11 consecutive patients with sensorimotor radial deficiency after distal humeral fracture were evaluated with conventional radiography, US, electroneurography, and electromyography. Surgical repair of the fracture and nerve inspection were performed in five patients. The remaining six patients were successfully treated conservatively. The US appearance of the radial nerve was studied in 10 healthy vol
