Phacoemulsification was performed in an 84-year-old woman with ocular hypertension and narrow drainage angles, previously treated by neodymium:YAG (Nd:YAG) laser peripheral iridotomy. Despite temporal zonular weakness, the surgery was uneventful. An intraocular lens (IOL) was placed and centered in the capsular bag. Two months later, the patient presented with malignant glaucoma. The IOL optic was anteriorly displaced in the capsular bag, and the IOL shift correlated with a -4.0 diopter myopic refractive error. A 42-year-old woman presents with headache and anisocoria. Gonioscopy suggested narrow angles. Ultrasound biomicroscopy confirmed the diagnosis. The patient underwent bilateral peripheral iridotomies with resolution of her symptoms. The differential diagnosis of headache and anisocoria are reviewed. Plateau iris syndrome and subacute angle closure are discussed.

PURPOSE: To assess the present state of examination procedures and treatments for angle-closure glaucoma (ACG) in Japan.
METHODS: A nationwide questionnaire survey was conducted, targeting 1237 registered facilities to Japanese Ophthalmological Society.

We describe a case of historical iridencleisis surgery in a patient who initially presented to our hospital with leaking blebs, presumed to be secondary to trabeculectomy. The patient was a 70-year-old male with a history of an uncertain glaucoma surgery performed on his left eye at 18 years of age (1973). BACKGROUND: Melanocytoma of the iris is a rare tumor which may sometimes undergo necrosis that could result in elevated intraocular pressure through pigment dispersion. Only nine similar patients have been previously reported.
METHODS: A 27-year-old woman presented with a dark brown iris stromal mass located between the 4 and 8 o'clock positions in the inferior quadrant. Her left visual acuity was 20/60. METHODS: This prospective noncomparative single-surgeon interventional case series comprised 190 consecutive myopic eyes of 115 patients having implantation of a ZSAL-4 IOL (Morcher GmbH) through a 5.5 mm x 3.0 mm sclerocorneal tunnel along the steepest meridian with a surgical iridectomy. PURPOSE: To evaluate the results of ab externo 4-point scleral fixation of posterior chamber intraocular lenses (PC IOLs) as a primary or secondary procedure.

Intraocular pressure (IOP) spike following various ophthalmic laser procedures, such as laser peripheral iridotomy (LPI), Nd:YAG capsulotomy and laser trabeculoplasty, is a known risk of these procedures. Most cases are mild, amenable to medical treatment, with no impact on final vision.
