Airway scope and gum elastic bougie with Macintosh laryngoscope for tracheal intubation in patients with simulated restricted neck mobility.

BACKGROUND: /st> Airway Scope is a new videolaryngoscope which requires less cervical movement during intubation than direct laryngoscopy. Thus, in patients wearing a rigid cervical collar, we compared the efficacy of the Airway Scope and the gum elastic bougie with Macintosh laryngoscope during tracheal intubation. METHODS: /st> Anaesthesia was induced with propofol, fentanyl, and rocuronium. A rigid cervical collar was applied, and patients were randomly assigned to tracheal intubation with an Airway Scope (n=48) or multiple-use gum elastic bougie with Macintosh laryngoscope (n=48). A Pediatric Case of Esophageal Foreign Body Removal Facilitated by Structured Briefing and Shared Visualization Using a Video Laryngoscope.

Foreign body ingestion is a common pediatric emergency, and coins are among the most frequently ingested objects. When a coin becomes lodged at the pharyngoesophageal junction, prompt removal is required to avoid perforation, mucosal injury, and airway compromise. We report the case of a five-year-old boy with a coin impacted in the upper esophagus at the level of the cricopharyngeus, successfully removed under general anesthesia using a video laryngoscope and Magill forceps without major complications. This case highlights two key educational points. First, the use of a video laryngoscope all

Visibility of video laryngoscope in high-illuminance environment simulating outdoor conditions: effects of screen conditions and polarized sunglasses.

BACKGROUND: Video laryngoscopes generally increase the success rate of tracheal intubation and clinical outcome compared to traditional direct laryngoscopes. However, there is a concern that their effectiveness can be compromised in bright outdoor environments. The impact of polarized sunglasses on the visibility of a video laryngoscope in a high-illumination environment simulating outdoor conditions was assessed. Additionally, the effect of screen smudges on screen visibility was examined. METHODS: A high-illumination environment was created using artificial light equivalent to daylight outd…

Use of a Rigid Curved Laryngoscope for Observation and Debridement of Degenerated Cricoid Cartilage in Nasogastric Tube Syndrome: A Case Report.

Nasogastric tube syndrome (NGTS) is a complication of NGT placement that can cause sore throat, bilateral vocal cord paralysis, and airway constriction. Although rare, this condition should be known by all physicians because it is sometimes fatal. We report a case of NGTS that was successfully diagnosed and debrided with a rigid curved laryngoscope. A 79-year-old man was referred to our department for evaluation of persistent pharyngeal pain and dysphagia after thoracic aortic aneurysm surgery. He had restricted bilateral vocal fold abduction, and the NGT had been placed for nearly 120 days. A

Awake endotracheal intubation using a hyperangulated video laryngoscope with a Total Control Introducer in a patient with a history of difficult intubation.

We report the first use of a fully articulating introducer called the Total Control Introducer (TCI) in combination with a hyperangulated video laryngoscope (VL) to perform an awake intubation in a patient with a history of difficult intubation. After appropriate airway topicalisation, a VL with a hyperangulated blade was inserted to visualise the glottis. A TCI articulating introducer was then used to dynamically navigate through the oropharynx into the trachea. Under indirect visualisation, an endotracheal tube was then passed over the TCI. The TCI was removed and the endotracheal tube was s

Removal of a fish bone endangering the common carotid artery under general anesthesia with video laryngoscope: A case report.

A fish bone penetrating the digestive tract is a common emergency, and its removal often requires endoscopy. We report herein a case in which a fish bone punctured the throat; its front end was close to the common carotid artery, but its back end could not be visualized. Subsequently, we compared the pharyngeal CT and carotid CTA of the patient and found that the fishbone had shifted. So we considered that the end of the fish bone could be rediscovered and successfully removed by a video laryngoscope. Finally, with the patient under deep sedation with maintained spontaneous breathing, the fish

Presbylarynx: How Easy Is It To Recognize The Aging Signs? A Prospective Study With Inter- And Intra-Rater Reliability In Laryngoscopic Evaluation.

PURPOSE: To determine if recognition of presbylarynx is easy and straightforward. METHOD: First, a systematic review regarding the structural features of the geriatric larynx through laryngostroboscopy was performed, and its results were presented to the raters in an educational session. Then, video recordings of rigid laryngostroboscopies were randomly selected and presented to a panel of otorhinolaryngologists. Intra- and inter-rater reliability were determined. RESULTS: Based on the main findings of the systematic review, raters were invited to consider the diagnosis of presbylarynx when

A Comparative Study in Airway Novices Using King Vision Videolaryngoscope and Conventional Macintosh Direct Laryngoscope for Endotracheal Intubation.

BACKGROUND AND OBJECTIVES: Tracheal intubation using laryngoscopy is a fundamental skill, for an anesthesiologist. However, teaching this skill is difficult since Macintosh direct laryngoscope (DL) allows only one individual to view the larynx during the procedure. Hence, this study aimed to determine whether King Vision® videolaryngoscope (KVL) provides any advantage over direct laryngoscopy in teaching this skill to airway novices. MATERIALS AND METHODS: In this prospective randomized crossover study, Ethical Committee clearance was obtained from the institutional review board (MSRMC/EC/201

Comparison of four channelled videolaryngoscopes to Macintosh laryngoscope for simulated intubation of critically ill patients: the randomized MACMAN2 trial.

BACKGROUND: Videolaryngoscopes with an operating channel may improve the intubation success rate in critically ill patients. We aimed to compare four channelled videolaryngoscopes to the Macintosh laryngoscope used for intubation of a high-fidelity simulation mannikin, in a scenario that simulated critical illness due to acute respiratory failure. RESULTS: Of the 79 residents who participated, 54 were considered inexperienced with orotracheal intubation. Each participant used all five devices in random order. The first-pass success rate was 97.5% [95% CI 91.1-99.7] for Airtraq™, KingVision™, …

Videolaryngoscopic versus direct laryngoscopic paraglossal intubation for cleft lip/palate reconstructive surgeries: A randomised controlled trial.

BACKGROUND AND AIMS: In the cleft lip and palate, the laryngoscope blade often tends to lodge inside midline clefts, causing reduced manoeuvrability and tissue trauma. The paraglossal technique avoids the midline and offers better Cormack Lehane (CL) grades. We aimed to assess the first-pass intubation rate in performing the left paraglossal laryngoscopy with a curved-blade videolaryngoscope (VLS) versus direct laryngoscope (DLS) in children with cleft palate and evaluate the time taken for successful endotracheal intubation (TTI) and Intubation Difficulty Score (IDS) with both devices. Improvement of laryngoscopic view by hand-assisted elevation and caudad traction of the shoulder during tracheal intubation in pediatric patients.

Pediatric patients have large heads and relatively small bodies, making it difficult to perform intubation even in the sniffing position. Therefore, this study was planned on the assumption that hand-assisted elevation and caudad traction of the shoulder (HA-ECTS) would compensate for the laryngoscopic view. In this observational study, 45 pediatric patients aged 0-36 months with an ASA physical status of I-III and scheduled for elective surgery under general anesthesia were enrolled. HA-ECTS was defined as hand-assisted personalized traction in the upper and caudad directions with both hands

Usefulness of Airway Scope for intubation of infants with cleft lip and palate-comparison with Macintosh laryngoscope: a randomized controlled trial.

BACKGROUND: Airway Scope (AWS) with its plastic blade does not require a head-tilt or separate laryngoscopy to guide intubations. Therefore, we hypothesized that its use would reduce the intubation time (IT) and the frequency of airway complication events when compared with the use of Macintosh Laryngoscope (ML) for infants with cleft lip and palate (CLP). METHODS: The parents of all patients provided written consents; we enrolled 40 infants with CLP (ASA-PS 1). After inducing general anesthesia using sevoflurane and rocuronium, we performed orotracheal intubations using either AWS (n = 20) o

Detection of passive movement of the arytenoid cartilage in unilateral vocal-fold paralysis by laryngoscopic observation: useful diagnostic findings.

In a previous study of patients with unilateral vocal-fold paralysis (UVFP), three-dimensional computed tomography (3DCT) revealed passive movement during phonation, with the arytenoid cartilage on the paralyzed side pushed to the unaffected side and deviated upwards. The present work compares the 3DCT findings with those obtained by 2-dimensional endoscopy to visualize the vertical passive movement of the arytenoid cartilage. The study population consisted of 23 patients with UVFP and two with laryngeal deviation but normal movement of the vocal folds. Two endoscopic findings represented cran

[Double lumen tube insertion in awake patients through the AirTraq laryngoscope in 2 cases of expected difficult airway].

The likelihood of difficult airway in thoracic surgery increases in the presence of associated cancer of the pharynx or larynx. The difficulty is greater when a double lumen tube must be inserted in these conditions, and various newly developed optical devices offer solutions for managing such cases. We report on 2 patients with expected difficult airway who were scheduled for lung resection. In both cases, intubation was accomplished through the AirTraq laryngoscope while the patient remained awake. Awake patient tolerance is facilitated by this laryngoscope, because the tube can be inserted

Efficacy of the Airtraq(®) laryngoscope with a fiberoptic bronchoscope compared with that of Airtraq(®) alone for tracheal intubation: a manikin study.

PURPOSE: Successful intubation using the Airtraq(®) requires optimal positioning of the glottis in the middle of the viewfinder. If the glottic view cannot be optimized, some glottic manipulation is essential for the Airtraq-assisted successful intubation. We evaluated the efficacy of the combined use of the Airtraq and a fiberoptic bronchoscope (FOB) compared with that of the Airtraq alone for tracheal intubation in simulated airway scenarios. METHODS: Eight anesthesia providers (four staff and four residents) were enrolled in this study. The participants intubated the trachea of the ALS Sim

A comparison of tracheal intubation using the Airtraq or the Macintosh laryngoscope in routine airway management: A randomised, controlled clinical trial.

The Airtraq laryngoscope is a novel single use tracheal intubation device. We compared the Airtraq with the Macintosh laryngoscope in patients deemed at low risk for difficult intubation in a randomised, controlled clinical trial. Sixty consenting patients presenting for surgery requiring tracheal intubation were randomly allocated to undergo intubation using a Macintosh (n = 30) or Airtraq (n = 30) laryngoscope. All patients were intubated by one of four anaesthetists experienced in the use of both laryngoscopes. No significant differences in demographic or airway variables were observed betw…
