Effect of congestive heart failure on safety and efficacy of left atrial appendage closure in patients with non-valvular atrial fibrillation.

OBJECTIVES: To evaluate the influence of congestive heart failure (CHF) on safety and efficacy of left atrial appendage closure (LAAC) in non-valvular atrial fibrillation (NVAF) patients. METHODS: A total of 401 patients who consecutively underwent LAAC with Watchman and LAmbre devices were divided into CHF (85 cases) and non-CHF (316 cases) groups. Comparisons between groups were performed against data. RESULTS: No significant differences were found in implantation success and periprocedural complication rates between the two groups. During a mean 2.2 years of follow-up, the incidence rate …

Safety of percutaneous left atrial appendage closure: results from the Watchman Left Atrial Appendage System for Embolic Protection in Patients with AF (PROTECT AF) clinical trial and the Continued Access Registry.

BACKGROUND: The Watchman Left Atrial Appendage System for Embolic Protection in Patients With AF (PROTECT AF) randomized trial compared left atrial appendage closure against warfarin in atrial fibrillation (AF) patients with CHADS₂ ≥1. Although the study met the primary efficacy end point of being noninferior to warfarin therapy for the prevention of stroke/systemic embolism/cardiovascular death, there was a significantly higher risk of complications, predominantly pericardial effusion and procedural stroke related to air embolism. Here, we report the influence of experience on the safety of p…

Tunnel-Like Myocardial Structure Formed by Sequential Left Atrial Appendage Closure and Resection as a Substrate for Macroreentrant Atrial Tachycardia.

Left atrial appendage closure (LAAC) is frequently performed during cardiac surgery to reduce thromboembolic risk; however, incomplete closure with residual appendage flow is not uncommon. In selected cases, additional left atrial appendage resection (LAAR) may be undertaken, although the electrophysiological consequences of this sequential surgical strategy remain poorly characterized. We report a 59-year-old man with a history of mitral valve repair, tricuspid annuloplasty, Cox-Maze procedure, and surgical LAAC who presented with sustained atrial tachycardia (AT). Persistent residual appenda…

Left Atrial Appendage Closure Versus Oral Anticoagulation in Atrial Fibrillation: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Left atrial appendage closure (LAAC) is an alternative to oral anticoagulation (OAC) for stroke prevention in patients with atrial fibrillation, but randomized controlled trials have primarily focused on composite end points, potentially obscuring differences in individual clinical outcomes. We compared the efficacy and safety of LAAC versus OAC with an emphasis on individual clinical outcomes. METHODS: A systematic review and meta-analysis of randomized controlled trials comparing LAAC with OAC in adults with atrial fibrillation was conducted in accordance with the Preferred Repo…

"Left Atrial Appendage Closure Versus Medical Therapy in Atrial Fibrillation: A Systematic Review and Meta-Analysis of Randomized Controlled Trials".

We aim to compare the impact of left atrial appendage closure vs. medical therapy on clinical outcomes in patients with atrial fibrillation, focusing on key clinical outcomes relevant to thromboembolic prevention. We conducted a PRISMA-guided systematic review and meta-analysis of studies comparing LAAC with medical therapy in AF adults. PubMed, Embase, Scopus, ClinicalTrial. gov, and Cochrane Library were searched from inception to March 2026. Outcomes included all-cause mortality, any stroke, ischemic stroke, hemorrhagic stroke, major bleeding, systemic embolism and cardiac death. Risk ratio…

Catheter-Based Left Atrial Appendage Closure vs Oral Anticoagulation in Patients With Atrial Fibrillation: A Systematic Review and Meta-Analysis.

IMPORTANCE: In patients with atrial fibrillation (AF), oral anticoagulation (OAC) is the gold standard for preventing ischemic stroke, at the expense of a higher risk of bleeding. Catheter-based left atrial appendage closure (LAAC) has been proposed as an alternative strategy to prevent stroke, but the efficacy and safety compared to OAC are uncertain. OBJECTIVE: To evaluate the efficacy and safety of catheter-based LAAC compared to OAC in patients with AF at high risk of stroke. DATA SOURCES: PubMed, Cochrane Central, and Web of Science were screened up to March 2026. The websites of leadin…

Left atrial appendage closure vs oral anticoagulation for stroke prevention in atrial fibrillation: Long-term outcomes from 4 randomized trials.

BACKGROUND: Left atrial appendage closure (LAAC) is primarily indicated for stroke prevention in patients with atrial fibrillation (AF) who have contraindications to long-term oral anticoagulants (OACs). However, the long-term comparative benefits of LAAC vs OACs in the broader AF population remain unclear. OBJECTIVE: To study aimed to assess the long-term efficacy and safety of LAAC compared with OACs in patients with AF, we conducted a meta-analysis of randomized controlled trials (RCTs). METHODS: We systematically searched PubMed, Embase, and Cochrane Library for eligible RCTs. Risk ratio…

Left Atrial Appendage Closure Compared With Oral Anticoagulants for Patients With Atrial Fibrillation: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Percutaneous left atrial appendage closure (LAAC) has been suggested as an alternative to long-term oral anticoagulation for nonvalvular atrial fibrillation, but comparative data remain scarce. We aimed to assess ischemic and bleeding outcomes of LAAC compared with vitamin K antagonists (VKAs) or direct oral anticoagulants (DOACs) for the prevention of cardioembolic events in patients with atrial fibrillation. METHODS AND RESULTS: Embase and MEDLINE were searched for randomized trials comparing LAAC, VKAs, and DOACs. The primary efficacy end point was any stroke or systemic emboli…

Revisiting Left Atrial Appendage Closure Versus Oral Anticoagulants in Recurrent Atrial Fibrillation Management: An Updated Systematic Review and Meta-Analysis.

Atrial fibrillation (AF) is a significant public health problem due to its association with coronary heart disease, stroke, and mortality, especially in the elderly. Therefore, traditional warfarin therapy, direct oral anticoagulants (DOACs), and the recent left atrial appendage closure (LAAC) have been compared as treatment approaches. In this regard, we aimed to synthesize the current evidence regarding the comparison these mentioned modalities in patients with AF. A comprehensive database search for records comparing LAAC and DOACs in patients with AF was conducted until December 15, 2023. …

Catheter closure of atrial septal defect in anatomically corrected malposition with left juxtaposition of the atrial appendages using intracardiac echocardiography.

Anatomically corrected malposition is characterized by abnormally related great vessels and atrioventricular and ventriculoarterial concordance. It is often associated with juxtaposition of the atrial appendages where the plane of the atrial septum is abnormally oriented. We describe the device closure of an atrial septal defect in a patient with such a lesion under intracardiac echocardiography guidance.

Evaluation of Zero-Exchange Transseptal Puncture Devices on Procedural Workflow and Consistency During Left Atrial Appendage Closure.

BACKGROUND: Reducing the number of device exchanges during a left atrial appendage (LAA) closure procedure can minimize the risk of injury, air embolism, and thrombus, and optimize procedural workflow. This study compared zero-exchange transseptal puncture (TSP) workflows facilitated by VersaCross Connect (VCC) that integrate directly into WATCHMAN (WM) therapy sheaths (FXD Curve Access [FXD] and TruSteer), with other TSP devices. METHODS: Retrospective chart analysis was performed comparing a radiofrequency needle TSP device, requiring multiple exchanges (NRG, n = 30), requiring at least 1 e

Outcomes of same-day discharge after left atrial appendage closure with and without pre-discharge transthoracic echocardiography.

BACKGROUND: Patients with nonvalvular atrial fibrillation with contraindication to anticoagulation undergo left atrial appendage closure. SCAI/HRS consensus recommends routine post-procedure transthoracic echocardiography (TTE) before same-day discharge. We studied whether there was a difference in outcomes with and without a TTE after device implantation by measuring 45-day hospitalization for any reason. METHODS: We performed a retrospective observational study using the data from our institutional LAAC registry. We compared patients discharged on the same day after the procedure who underw…

Single versus dual antiplatelet therapy following percutaneous left atrial appendage closure-A systematic review and meta-analysis.

BACKGROUND: In the last few years, percutaneous LAA occlusion (LAAO) has become a plausible alternative in atrial fibrillation (AF) patients with contraindications to anticoagulation therapy. Nevertheless, the optimal antiplatelet strategy following percutaneous LAAO remains to be defined. METHODS: Studies comparing single antiplatelet therapy (SAPT) versus dual antiplatelet therapy (DAPT) following LAAO were systematically searched and screened. The outcomes of interest were ischemic stroke, device-related thrombus (DRT) and major bleeding. A random-effect meta-analysis was performed compari
