Radial-specific catheters for neuroendovascular procedures: A systematic review and meta-analysis.

Despite a radial-first approach in many neurointerventions, there are no systematic reviews and meta-analysis which comprehensively assess radial-specific catheter for neuroendovascular procedures. A systematic literature search was conducted through four electronic databases based on PRISMA 2020 guideline. Risk of bias was assessed employing Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I) tool. A total of eleven studies with 990 patients using Armadillo, RIST, Zoom RDL, and BMX 81 catheters were included. The Armadillo showed significantly lower failure to catheterize the t

[Brain temperature in patients with central nervous system lesions].

The knowledge of human brain temperature is still very limited. In this report we investigated the relationship between brain and trunk temperature in neurosurgical patients during normothermia and fever. Another problem addressed was that of possible gradients of temperature within the brain. We carried out direct recordings of temperature in 63 operated, neurosurgical patients with a variety of intracranial pathologies. Flexible, teflon-coated thermocouples were placed intracranially during neurosurgical procedures. Oesophageal, rectal and tympanic temperatures were also monitored. An error …

Assessing pediatric neurosurgery capacity in La Paz, Bolivia: an illustrative institutional experience of a lower-middle-income country in South America.

OBJECTIVE: The current pediatric neurosurgery capacity in lower-middle-income countries (LMICs) in South America is poorly understood. Correspondingly, the authors sought to interrogate the neurosurgical inpatient experience of the sole publicly funded pediatric hospital in one of the largest regional departments of Bolivia to better understand this capacity. METHODS: A retrospective review of all neurosurgical procedures performed at the Children's Hospital of La Paz, Bolivia (Hospital del Niño "Dr. Ovidio Aliaga Uria") between 2019 and 2023 was conducted after institutional approval using a…

Utility of neuromuscular blockade reversal in the evaluation of acute neurosurgical patients: A retrospective case-series.

OBJECTIVE: Sugammadex reversal of neuromuscular blocking agents (NMBAs) is usually performed postoperatively. A scarcity of literature exists exploring sugammadex use for timely neurological examination of neurosurgical patients. NMBAs, like rocuronium, are used in the Emergency Department during intubation and their unpredictable duration of action often impedes timely and accurate assessment of patient neurological status. We aim to explore the role of sugammadex in evaluating patients in need of acute neurosurgical care. METHODS: Retrospective assessment of patients presenting with traumat

Steroid-responsive chorea in moyamoya disease.

We report the case of a 13-year-old boy with disabling chorea due to moyamoya disease. His chorea seemed to improve with steroid therapy. We conclude that steroid therapy may ameliorate moyamoya-associated chorea, and perioperative steroids can confound neurosurgical outcome. We are unable to assess the effect of cerebrovascular bypass procedures on the outcome of chorea in this patient.

A case of Creutzfeldt-Jakob disease associated with a dura mater graft in the United States.

We describe a case of Creutzfeldt-Jakob disease associated with a dura mater graft (Lyodura brand) in a 26-year-old man who underwent several neurosurgical procedures as a child. Clinicians and infection control personnel should be aware that recipients of Lyodura brand dura mater grafts processed before May 1987 may remain at increased risk for Creutzfeldt-Jakob disease throughout their lives.

Clinical application of flexible and variable transparent endport technology in minimally invasive neuroendoscopic keyhole surgery: a practical new technology.

With the development of endoscopic technology, neuroendoscopy utilizes its advantages such as good illumination, close range observation, and flexible degrees of freedom. Neuroendoscopic surgery can achieve advantages such as minimally invasive, high clearance rate, low incidence of complications, good brain tissue protection, and fewer surgery-related injuries. However, minimally invasive endoscopic surgery also has inherent limitations, since its narrow surgical channels are prone to collapse and require special instrument support. By summarizing the literature on the use of endoport technol

The lateral orbitotomy approach: Technical nuances and video-illustration.

The lateral orbitotomy approach is a minimally-invasive transorbital technique that was originally developed to address orbital pathologies. With the increased understanding of orbital anatomy and multidisciplinary collaboration between neurosurgeons and oculoplastic surgeons, the approach has been expanded to address various pathologies of the anterior and middle cranial fossae, and intrinsic brain pathologies. Clipping select aneurysms and resecting both intrinsic and extra-axial brain tumors were made possible through a small skin incision around the eye. The lateral orbitotomy offers optim

Role of cranial and spinal virtual and augmented reality simulation using immersive touch modules in neurosurgical training.

Recent studies have shown that mental script-based rehearsal and simulation-based training improve the transfer of surgical skills in various medical disciplines. Despite significant advances in technology and intraoperative techniques over the last several decades, surgical skills training on neurosurgical operations still carries significant risk of serious morbidity or mortality. Potentially avoidable technical errors are well recognized as contributing to poor surgical outcome. Surgical education is undergoing overwhelming change, as a result of the reduction of work hours and current tren

Pig as a large animal model for posterior fossa surgery in oto-neurosurgery: A cadaveric study.

This study proposes a practical model for a new approach to the posterior fossa in common domestic pigs. Several surgical procedures can be simulated in the nonliving pig model, including soft tissue dissection, drilling of temporal bone, dural incision, access to the cerebellopontine angle, exposure of cranial nerves and drilling of the internal auditory canal. The pig model perfectly simulates standard otological and neurosurgical procedures, and we highlight the feasibility of our approach for further experiments in a living pig model with the possibility of reproducing the model for resear…

Anesthesia mumps resulting in temporary facial nerve paralysis after the auditory brainstem implantation in a 3-year-old child.

An acute transient sialadenitis of the major salivary glands in the early postoperative period is called 'anesthesia mumps'. It has been reported in different surgical procedures especially in neurosurgical procedures. Anesthesia mumps develops very fast after the extubation period but it usually regresses with no sequelae within a few hours. However, sometimes serious complication can occur such as respiratory distress. In this report, we present a 3-year-old girl with an anesthesia mumps and facial palsy occurring after successful auditory brainstem implantation and we discuss the cause and …

Modified encephaloduroarteriosynangiosis as a surgical treatment of childhood moyamoya disease: report of five cases.

Moyamoya disease is an idiopathic disorder characterized by progressive occlusion of the internal carotid and anterior and middle cerebral arteries, with formation of an extensive abnormal collateral circulation at the base of the brain. Many neurosurgical procedures have been designed to bypass these occluded vessels. The results of one of these procedures, modified encephaloduroarteriosynangiosis was reviewed in five children followed for 3 1/2 to 19 1/2 years. Modified encephaloduroarteriosynangiosis performed unilaterally in one and bilaterally in four of the children, appeared to halt neu…

A basic model for training of microscopic and endoscopic transsphenoidal pituitary surgery: the Egghead.

BACKGROUND: Transsphenoidal endoscopic surgery has gained popularity in the last 2 decades and is becoming a standard technique for resection of pituitary adenomas. In contrast to their ENT colleagues, neurosurgical residents have practically no endoscopic experience when they reach the training stage for transsphenoidal procedures. We have developed an affordable method for repetitive training in endoscopic (and microscopic) work in a narrow channel, allowing training of the basic movements needed for resection of pituitary adenoma. METHODS: In collaboration with colleagues in the ENT Depart

[Building High-Quality Evidence in Endoscopic Neurosurgery : Practical Clinical Research Strategies and Multicenter Collaboration].

Endoscopic neurosurgery has advanced rapidly over the past three decades and now represents a core minimally invasive approach in modern neurosurgical practice. Despite technical progress, high-certainty evidence has often lagged, partly because outcome variability is driven not only by disease characteristics but also by procedure-specific complexity, surgeon proficiency, learning curves, and institution-dependent factors such as multidisciplinary team structure and perioperative workflows. This study provides a practical framework for building reproducible clinical evidence in endoscopic neu

The plated bayonet forceps: technical note and illustrative video clips.

INTRODUCTION: Plated bayonet forceps (PBF) are bayonet forceps with plated tips. PBF have the advantage for longitudinal dissection and tissue spreading with reduced local pressure. The use of PBF has rarely been described. METHODS: We describe the use of PBF in neurosurgical procedures, including representative surgical video clips showing their use. RESULTS: Overall, the main indications we have used PBF include spinal cord splitting during intramedullary tumor resection or syrinx drainage, and intraparenchymal brainstem tumors. PBF may also be applied for any small field tissue spreading,…

[Neck Clipping via Subtemporal Approach].

The subtemporal approach offers the advantages of a wide surgical field in the anteroposterior direction and easy access to the proximal basilar artery. This approach can be adapted to treat low-positioned aneurysms using a tentorial incision, known as the subtemporal transtentorial approach. However, a disadvantage of the subtemporal approach is the risk of injury to the temporal lobe. To mitigate this risk, it is important to ensure proper positioning, adequate cerebrospinal fluid drainage, preservation of the bridging veins, and intermittent retraction of the temporal lobe.
