Leak-point pressures.

The pressure based management of patients with neurogenic vesical dysfunction has led to greatly improved outcomes with respect to upper and lower urinary tract complications. At the heart of this management is detrusor leak-point pressure testing that verifies that a low intravesical pressure is achieved and subsequently maintained. The abdominal (or Valsalva) leak-point pressure that quantifies the degree and type of urethral sphincter dysfunction, is an essential test in selecting the appropriate treatment for stress urinary incontinence. The authors discuss the history, importance, and app…

Intraarticular pressure of the shoulder.

Nine fresh frozen cadaveric shoulders were used to examine the changes in intraarticular pressure (IAP) of the shoulder joint and the position of the humeral head under various loads to determine the relationship between these parameters. All the soft tissues superficial to the rotator cuff were removed. The position of the humerus relative to the scapula was monitored using an electromagnetic tracking system before and after venting the capsule with 0, 0.5, and 1.0 kg of load applied to the humerus inferiorly in addition to the weight of the arm. Simultaneously, the IAP was monitored using a …

Fever and intracranial pressures.

The effect of fever upon intracranial pressures was determined in the rabbit and cat. In the unanesthetized rabbit and cat, cerebrospinal fluid (CSF) pressure was measured via direct cannulation of the lateral cerebral ventricle. Intracranial pressure (ICP) was measured in the rabbit by a subarachnoid screw technique. In all cases, intravenous administration of bacterial pyrogen extracted from Salmonella abortus equi resulted in significant differences from controls in physiological variables measured during the initial "chill" phase of the fever. There was an increase in body temperature, a f…

A Center-of-Pressure Guided Finger-Press Sensor for Cuffless Blood Pressure Estimation.

Cuffless blood pressure estimation using the finger-pressing method remains sensitive to improper finger centering and inconsistent contact force, which degrade the accuracy of the oscillometric envelope and PPG signal morphology. This paper details the development of a research prototype that integrates three force sensors and a photoplethysmograph (PPG) sensor to quantify the finger-device interaction. This system is intended as a pre-clinical research tool rather than a clinically validated medical device. We implement a weighted centroid algorithm for center of pressure (CoP) feedback to g…

Early life factors, blood pressure percentile trajectories, and elevated blood pressure in children.

BACKGROUND: Elevated blood pressure (BP) in childhood is a predictor of adult hypertension. METHODS: We characterized existing blood pressure (BP) percentile trajectories in children aged 3-9 years and examined factors associated with BP trajectory and elevated BP among 1040 participants in a prospective birth cohort study in Durham, North Carolina (2005-2011). We obtained BP measurements from medical records and demographic, social, and behavioral characteristics from questionnaires. We identified systolic blood pressure (SBP) percentile trajectories using group-based trajectory modeling as …

Impact of baseline systolic blood pressure on blood pressure changes following renal denervation.

BACKGROUND: Renal denervation (RDN) is a guideline-recommended treatment to reduce blood pressure (BP) in patients with uncontrolled hypertension. However, it is unclear if there are patient characteristics that are predictive of greater BP reduction. Baseline systolic blood pressure (SBP) has consistently been identified as an indicator of BP reduction after RDN. AIMS: Our study aimed to quantify the expected SBP change after RDN based on baseline SBP. METHODS: Patients undergoing radiofrequency RDN were pooled from multiple clinical studies, including SPYRAL First-In-Human (n=50), SYMPLICI…

Assessment of blood pressure variability: characteristics and comparison of blood pressure measurement methods.

Previous studies have reported that blood pressure variability (BPV) is associated with the risk of cardiovascular events independent of blood pressure (BP) levels. While there is little evidence from intervention trials examining whether suppressing BPV is useful in preventing cardiovascular disease, it is suggested that detection of abnormally elevated BPV may be useful in reducing cardiovascular events adding by complementing management of appropriate BP levels. Cuffless BP devices can assess beat-to-beat BPV. Although cuffless BP monitoring devices have measurement accuracy issues that nee…

Under Pressure: Lamina Cribrosa Pore Path Tortuosity in Response to Acute Pressure Modulation.

PURPOSE: Lamina cribrosa (LC) deformation is hypothesized to play a major role in glaucoma pathogenesis. The purpose of this study was to determine in vivo how varying intraocular pressure (IOP) under fixed intracranial pressure (ICP), and vice versa, deforms the pore paths throughout the LC volume. METHODS: Spectral-domain optical coherence tomography scans of the optic nerve head were acquired from healthy adult rhesus monkeys under different pressures. IOP and ICP were controlled with gravity-based perfusion systems into the anterior chamber and lateral ventricle, respectively. IOP and ICP…

Cumulative Diastolic Blood Pressure Burden in Normal Systolic Blood Pressure and Cardiovascular Disease.

BACKGROUND: The clinical significance of isolated diastolic hypertension defined by the 2017 American College of Cardiology/American Heart Association blood pressure (BP) guidelines remains inconsistent. We examined whether long-term diastolic burden predicts the first major adverse cardiovascular event in participants with sustained and untreated normal systolic BP. METHODS: The Mass General Brigham Biobank is a New England health care-based cohort recruited between 2010 and 2021. A total of 15 979 participants aged 18 to 64 years and without prior cardiovascular disease, antihypertensives, …

Anesthetics affect peripheral venous pressure waveforms and the cross-talk with arterial pressure.

Analysis of peripheral venous pressure (PVP) waveforms is a novel method of monitoring intravascular volume. Two pediatric cohorts were studied to test the effect of anesthetic agents on the PVP waveform and cross-talk between peripheral veins and arteries: (1) dehydration setting in a pyloromyotomy using the infused anesthetic propofol and (2) hemorrhage setting during elective surgery for craniosynostosis with the inhaled anesthetic isoflurane. PVP waveforms were collected from 39 patients that received propofol and 9 that received isoflurane. A multiple analysis of variance test determined …

Lumbar-peritoneal shunt for idiopathic normal pressure hydrocephalus and secondary normal pressure hydrocephalus.

OBJECTIVES: Normal-pressure hydrocephalus is a clinical syndrome consisting of dilated cerebral ventricles with the clinical triad of gait disturbance, cognitive impairment and/or urinary dysfunction. Lumbar-peritoneal (LP) shunt could improve idiopathic normal pressure hydrocephalus (iNPH) while its effectiveness on secondary NPH (sNPH) is elusive. We compared the clinical results of the patients who received LP shunt surgery between iNPH and sNPH. MATERIALS AND METHODS: We retrospectively analyzed the patients who received LP shunt surgery in a single center from January 1, 2017, to June 30…

Characterizing Insertion Pressure Profiles During Cochlear Implantation: Simultaneous Fluoroscopy and Intracochlear Pressure Measurements.

BACKGROUND: Combined electrical-acoustical stimulation (EAS) has gained popularity as patients with residual hearing are increasingly undergoing cochlear implantation. Preservation of residual hearing correlates with hearing outcomes, but loss of hearing occurs in a subset of these patients. Several mechanisms have been proposed as causing this hearing loss; we have previously described high amplitude pressure transients, equivalent to high-level noise exposures, in the inner ear during electrode insertion. The source of these transients has not been identified. METHODS: Cadaveric human heads…
