Outcomes of Right Ventricular Outflow Tract Reconstruction in Children: Retrospective Comparison Between Bovine Jugular Vein and Expanded Polytetrafluoroethylene Conduits.

Bovine jugular vein (BJV) and expanded polytetrafluoroethylene (ePTFE) conduits have been described as alternatives to the homograft for right ventricular outflow tract (RVOT) reconstruction. This study compared RVOT reconstructions using BJV and ePTFE conduits performed in a single institution. The valve functions and outcomes of patients aged < 18 years who underwent primary RVOT reconstruction with a BJV or ePTFE conduit between 2013 and 2017 were retrospectively investigated. 44 patients (20 and 24 with BJV and ePTFE conduits, respectively) met the inclusion criteria. The mean follow-up ti

Outcomes of the modified norwood procedure: hypoplastic left heart syndrome versus other single-ventricle malformations.

Recent studies have suggested that survival after the modified Norwood procedure is influenced by anatomy and is worse for patients with hypoplastic left heart syndrome (HLHS) than for patients with other forms of functional single ventricle. This study aimed to compare the outcomes of the modified Norwood procedure for these two groups at a single center. A retrospective chart review compared modified Norwood procedure outcomes between children with HLHS (n = 40) and children with other single-ventricle malformations (n = 19) from January 2002 to August 2012. The HLHS patients had a lower mea

Eight years of pulmonary valve replacement with a suggestion of a promising alternative.

UNLABELLED: In a retrospective study we assessed surgical results following right ventricular to pulmonary artery connection repair or replacement at a medium of 2.4 years (0-8) follow-up. Data were retrieved from hospital charts. RESULTS: Three hundred and sixty five operations were performed in 286 patients in eight years starting in 2000 using different surgical methods. Homografts and Monocusps had a more than 50% significantly lower risk for reoperation than Contegra or bicuspid valves (p < 0.01). Data for infants and older children and grown ups were analysed separately. In the infant g

Model-Based Economic Evaluation of the First-in-Class Myosin Inhibitor Mavacamten Versus Care as Usual in Obstructive Hypertrophic Cardiomyopathy Patients from a Dutch Societal Perspective.

OBJECTIVES: Obstructive hypertrophic cardiomyopathy (oHCM) is a myocardial disease, characterised by left ventricular hypertrophy, hampering the ventricular blood outflow. Standard of care (SoC) includes medications such as beta-blockers (BB) and calcium channel blockers (CCB) and septal reduction therapies. Recently, mavacamten, a first-in-class myosin inhibitor, became available to oHCM patients. The objective was to develop a decision analytic model to evaluate the cost effectiveness of mavacamten compared with SoC in oHCM patients from a Dutch societal perspective. METHODS: A Markov model

Septal myectomy for obstructive hypertrophic cardiomyopathy: Comparison of outcomes of the transapical beating-heart and transaortic on-pump approaches.

BACKGROUND: Transaortic on-pump septal myectomy (SM) is the established standard for surgical septal reduction, but transapical beating-heart SM recently has emerged as a less invasive alternative. This study compares the early outcomes of these 2 approaches to performing SM in patients with obstructive hypertrophic cardiomyopathy (oHCM). METHODS: We analyzed the outcomes of 200 consecutive patients undergoing transapical beating-heart myectomy and 200 patients receiving transaortic on-pump myectomy. The primary outcome was procedural success, defined as a peak left ventricular outflow tract

Feasibility of 4D-flow CMR for haemodynamic characterization in hypertrophic cardiomyopathy after septal myectomy with and without anterior mitral valve leaflet extension.

OBJECTIVES: The common surgical treatment in patients with obstructive hypertrophic cardiomyopathy is septal myectomy. This involves resection of a segment of the myocardial septum and can be performed with and without concomitant anterior mitral valve leaflet extension (AMVLE). While both approaches have satisfying clinical outcomes, there is a lack of data regarding the added value of concomitant AMVLE. In particular, their impact on postoperative haemodynamics remains unexplored. Therefore, we conducted a study to assess the feasibility of utilizing four-dimensional-flow cardiac magnetic re

The importance of bilateral monitoring of cerebral oxygenation (NIRS): Clinical case of asymmetry during cardiopulmonary bypass secondary to previous cerebral infarction.

Cerebral oximetry based on near infrared spectroscopy (NIRS) technology is used to determine cerebral tissue oxygenation. We hereby present the clinical case of a 12-month old child with right hemiparesis secondary to prior left middle cerebral artery stroke 8 months ago. The child underwent surgical enlargement of the right ventricular outflow tract (RVOT) with cardiopulmonary bypass. During cardiopulmonary bypass, asymmetric NIRS results were detected between both hemispheres. The utilization of multimodal neuromonitoring (NIRS-BIS) allowed acting on both perfusion pressure and anesthetic de

Surgical septal myectomy for hypertrophic cardiomyopathy in Greece: a single-center initial experience.

INTRODUCTION: Surgical septal myectomy is thought to be the gold standard of treatment for obstructive hypertrophic cardiomyopathy (HCM) with obstruction symptoms refractory to optimal medical therapy. In Europe, during the last 2 decades, myectomy has been set aside, while alcohol septal ablation has been widely promoted. In this paper, we analyze our first experience of surgical septal myectomy in a small cohort of patients with HCM coming from a single tertiary center. METHODS: Thirty-two patients (16 male, 50%) with a mean age of 58.1 ± 14.4 (range 12-79 years) underwent myectomy for HCM

True aneurysmal dilatation of a contegra conduit after right ventricular outflow tract reconstruction: a novel mechanism of conduit failure.

Valved conduits are frequently used in congenital heart surgery to establish continuity between the right ventricle and the pulmonary arteries. The Contegra bovine jugular vein (Medtronic Inc, Minneapolis, MN) is a conduit that incorporates a tri-leaflet valve and affords off-the-shelf availability, good handling characteristics, and excellent hemodynamics. However, complications related to the use of this device have been reported, with conduit failure occurring mainly as a consequence of stenosis, conduit thrombosis, and valve regurgitation. We present a case of aneurysmal conduit failure of

Decellularized pulmonary homograft (SynerGraft) for reconstruction of the right ventricular outflow tract: first clinical experience.

INTRODUCTION: Cryopreserved homograft valve conduits have been used to reconstruct the right and left ventricular outflow tract. Long-term studies have shown homograft degeneration and calcification, and it has been postulated that immunological mediated phenomena in a manner similar to that seen in chronic rejection may contribute to the degeneration process. The development of a decellularized, non-glutaraldehyde-fixed valve conduit creates a non-immunogenic connective tissue matrix for autologous recellularization by host cells. The aim of the study was to characterize the clinical and hemo

Multi-omics analysis of genetic drivers linking aortic stenosis and left ventricular diastolic dysfunction in heart failure.

BACKGROUND: Aortic stenosis (AS) and left ventricular diastolic dysfunction (LVDD) often coexist in heart failure (HF), but the mechanisms linking them remain unclear. While AS increases afterload and promotes myocardial stiffening, emerging AI-based evidence suggests LVDD can precede the development of AS or progress simultaneously, indicating shared upstream mechanobiological and inflammatory drivers. This study explores the genetic contributors connecting AS and LVDD to identify early molecular markers and convergent pathways in HF. METHOD: We analyzed Whole Genome Sequence (WGS) and RNA-s

Pacing for drug-refractory or drug-intolerant hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a genetic disease with an autosomal-dominant inheritance for which negative inotropes are the most widely used initial therapies. Observational studies and small randomised trials have suggested symptomatic and functional benefits using pacing and several theories have been put forward to explain why. Pacing, although not the primary treatment for HCM, could be beneficial to patients with relative or absolute contraindications to surgery or alcohol ablation. Several randomised controlled trials comparing pacing to other therapeutic modalities ha
