Pancreatic Heterotopia in the Small Bowel: A Case Report and Literature Review.

Diagnosing pancreatic heterotopia (PH) is challenging, because it is a rare congenital anomaly. We report the case of a 66-year-old man who presented with ileus and jejunal PH diagnosed using single-balloon enteroscopy. Reviewing 178 cases of small intestinal PH in the literature showed that (1) PH mostly occurred at the duodenum (61.8%), followed by the jejunum (22.5%) and ileum (14.6%); (2) patients with ileal PH were younger (mean, 40.7 vs 54.6 years; P < 0.001), while those with jejunal PH were older (mean, 59.5 vs 51.7 years; P = 0.006); (3) most patients with jejunal PH were females, …

Small bowel metastasis from endometrial cancer presenting as a bowel obstruction: A case report with literature review.

This report describes a rare case of small bowel metastasis from endometrial cancer, diagnosed six years after initial treatment. A 62-year-old woman with a history of grade 2 stage IA endometrial cancer, previously treated with hysterectomy and bilateral salpingo-oophorectomy, presented with intermittent abdominal pain and nausea. Imaging studies revealed small bowel obstruction and balloon-assisted enteroscopy identified an annular ulcer with luminal narrowing in the jejunum. Histopathological examination of the biopsy specimen suggested carcinoma; however, its primary origin remained unclea

A case of obscure gastrointestinal bleeding in a 18-year-old woman.

In this report, we discuss a case of an 18 year-old woman with obscure gastrointestinal bleeding (OGIB) revealed to be originated from a Meckel's diverticulum. OGIB is defined as persistent or recurrent bleeding from a difficult to identify source and accounts for approximately 5% of all cases of GI bleeding. Lesions in the small bowel most commonly cause it. OGIB represents one of the most challenging disorders faced by gastroenterologists due to its evasive nature and difficulty in identifying the exact source of the bleeding. Recent technological advances such as capsule endoscopy, balloon-

Complications in Patients with Surgically Altered Gastrointestinal Anatomy Undergoing Endoscopic Retrograde Cholangiopancreatography: 15-Year Experience at a Tertiary Care Center in Latin America.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a common procedure, but it poses challenges in patients with surgically altered gastrointestinal anatomy (SAGA). Alternative techniques like single-balloon enteroscopy (SBE), double-balloon enteroscopy (DBE), or push enteroscopy (PE) have been used, albeit with potential complications. Limited Latin American data exists on ERCP complications in SAGA patients. Our goal is to describe complications of ERCP in SAGA at a national referral institution. METHODS: Retrospective, single-center cohort study. All SAGA ERCP procedures p…

Management of small bowel polyps: from small to big.

PURPOSE OF REVIEW: Benign small bowel polyps or mass are clinically poorly distinguishable from malignant small bowel masses, and the diagnostic conditions are almost the same. The important point for clinicians is first to take advantage of the different available diagnostic tools to optimize the diagnostic algorithm of a small bowel polyp or mass. Next, according to the clinical situation, associated disease or sporadic situation, the difficulty is to adapt the treatment decision to the patient situation. RECENT FINDINGS: The last 20 years have been marked by the development of major diagno

Meckel's diverticulum in an adult: an obscure presentation of gastrointestinal bleeding.

A 17-year-old boy, with a background of haemophilia A, presented to hospital with an episode of brisk and painless per rectal (PR) bleeding. Initial investigation with gastroscopy and colonoscopy revealed no source of bleeding. Examination of the small bowel including MRI, CT angiogram and capsule endoscopy revealed no abnormalities. Double balloon enteroscopy (DBE) from an antegrade position was attempted, with no abnormalities noted. The symptoms persisted and DBE was attempted again from a retrograde approach and a diverticulum was subsequently noted. Laparoscopic surgery was performed iden…

Migration of biliary stent resulting in ureteral obstruction: A case report.

This case report describes a rare complication following endoscopic retrograde cholangiopancreatography (ERCP) involving biliary stent migration, leading to ureteric obstruction. Initial management of choledocholithiasis with ERCP and stenting was followed by laparoscopic cholecystectomy. Unexpectedly, a month later, the patient presented with back pain due to the migrated stent compressing the ureter. Intervention with single balloon enteroscopy successfully resolved the issue, highlighting the importance of timely diagnosis and multidisciplinary management.

Clinical approach to the diagnosis and management of small bowel bleeding.

The diagnosis and management of small bowel bleeding (SBB) can be a clinical challenge. Advances in video capsule endoscopy, balloon-assisted enteroscopy, and multiphasic computed tomography allow for localization and therapeutic intervention. Etiologies of SBB including vascular, neoplastic, and inflammatory conditions are associated with age and comorbidities. The present review highlights terminologies that describe SBB, provides a differential diagnosis for bleeding etiologies, and summarizes a clinical approach to managing this condition.

Cavernous Hemangioma of the Small Bowel: A Case Report and Literature Review.

Hemangiomas of the small intestine are rare and very difficult to diagnose preoperatively. Clinical presentations may include occult or massive gastrointestinal (GI) bleeding, obstruction, intussusception, and perforation. We report a 66-year-old Caucasian male patient with severe anemia secondary to occult GI bleeding from a cavernous hemangioma in the jejunum. A double balloon enteroscopy following capsule endoscopy was performed to obtain biopsy samples, which established the final diagnosis.

A case report of gastrointestinal hemorrhage caused by small intestinal hemangioma.

RATIONALE: Small intestinal hemangioma is a rare condition and very difficult to diagnose preoperatively. It can occur in all segments of the small intestine, but jejunum is common. Its common symptoms are gastrointestinal bleeding and chronic anemia, while intussusception, intestinal obstruction and perforation are rare. In recent years, the popularization and application of capsule endoscopy, computed tomographic enterography and double-balloon enteroscopy play vital roles in the diagnosis and management of small bowel bleeding. We report a case of gastrointestinal hemorrhage caused by of th…

Safety of Deep Enteroscopy and Capsule Endoscopy in LVAD Patients: Case Report and Literature Review.

Patients with a left ventricular assist device (LVAD) have increased risk of gastrointestinal (GI) bleeding. They are prone to develop angiodysplasia of the small intestine, and have a higher risk of bleeding as these patients are all required to be on permanent therapeutic anticoagulation. Here we report a case of a critically ill 55-year-old male on pressors and inotropes with an LVAD, who successfully underwent an antegrade double balloon enteroscopy (DBE).

Biliary drainage in patients with altered anatomy: Literature review of different endoscopic approaches.

Endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy remains a challenging field in therapeutic endoscopy due to the complex anatomical reconstructions that limit access to the biliary tree. Over the past two decades, device-assisted enteroscopy (DAE), including single-balloon, double-balloon, and motorized spiral enteroscopy, has expanded the feasibility of ERCP in this population, with overall technical success rates generally reported between 70% and 90%. Nevertheless, these techniques are technically demanding, time-consuming, and frequently aff
