Levorphanol for excruciating pain from oral cancer: a case report.

In the United States the most common cancers of the head and neck are those of the oral cavity and pharynx. In 2007, oral cancer will be diganosed in more than 37,000 Americans and will claim more than 7,500 lives. The report presented here describes a case of extrarodinary pain in a woman with metastatic adenocarcinoma of the hard palate. At the time of her referral for hospice care, she was experiencing excruciating, unremitting, chronic pain graded 10 of 10 on a standard pain scale. Her analgesic treatment consisted of opioids prescribed in increasingly higher doses until they were ineffect

186Re-HEDP for metastatic bone pain in breast cancer patients.

Two-thirds of patients with metastatic cancer suffer from pain. Pain originating from skeletal metastases is the most common form of cancer-related pain. Bone pain, often exacerbated by pressure or movement, limits the patient's autonomy and social life. Pain palliation with bone-seeking radiopharmaceuticals has proven to be an effective treatment modality in patients with metastatic bone pain. These bone-seeking radiopharmaceuticals are extremely powerful in treating scattered painful bone metastases, for which external beam radiotherapy is impossible because of the large field of irradiation

Randomised trial of oral morphine for chronic non-cancer pain.

BACKGROUND: The use of opioid analgesics for chronic non-cancer pain is controversial. Some surveys report good pain relief and improvement in performance while others suggest a poor outcome with a propensity to psychological dependence or addiction. METHODS: We undertook a randomised double-blind crossover study to test the hypothesis that oral morphine relieves pain and improves the quality of life in patients with chronic regional pain of soft tissue or musculoskeletal origin who have not responded to codeine, anti-inflammatory agents, and antidepressants. Morphine was administered as a su…

[Evaluation and treatment of chronic pain after cervicofacial cancer surgery].

When lasting pain occurs after surgery of head and neck cancer, tumoral recurrence should be considered. In addition to curative treatment, relief of pain is often provided by opioid analgesics. Doses vary according to tolerance and patient. Other than digestive routes of administration may be required. Here are two clinical reports: the first case with cervical epidural analgesia by ambulatory autoadministration device (Patient control analgesia), the other one with intrathecal in C7-T1 catheter with port access in which morphine was injected every 24 hours resulting in efficient analgesia, i

Management of cancer pain with oral controlled-release morphine sulfate.

Morphine sulfate Contin (MSC) is an investigational matrix delivery system for oral morphine sulfate that allows for prolonged blood levels of morphine. Twenty-six patients with inadequately controlled cancer-related pain were examined in an open but controlled study using MSC. Initially, all patients were converted from the prestudy analgesic regimen to an equianalgesic amount of immediate-release morphine sulfate (IRMS) on a q4h dose schedule that was in turn titrated to the level of adequate pain relief. Patients then were switched to MSC q8h and eventually to q12h, starting at doses repres

[Shoulder-arm pain and its treatment in breast cancer diseases].

Shoulder-arm pain after mammary carcinoma conditions has four causes which must be distinguished since they require an entirely different therapy. Only one of these causes involves the condition or its progression, requiring invasive intervention. The other causes result from the treatment. The damage to nerve tissue due to irradiation has a late onset and is difficult to treat. The development of a sympathetic reflex dystrophy begins early and is simple to treat if this treatment is carried out in good time and systematically. The treatment of a false muscular posture should be based on a hol

Peripheral neuron circuitry mediating pain in head and neck cancer.

PURPOSE OF REVIEW: Pain is a major source of morbidity in head and neck cancer (HNC), yet its neural mechanisms remain incompletely defined. This review examines how malignant cells, the tumor microenvironment, and cancer treatment remodel peripheral sensory and autonomic circuits, and how neural activity reciprocally influences tumor biology, with particular attention to findings published since 2023. RECENT FINDINGS: Recent studies show that HNC selectively reorganizes tumor-associated nerves rather than merely injuring a pre-existing neural scaffold. Sensory-neuron transcriptional response

Metabolic Modulation for Cancer Pain Relief: A Retrospective Pilot Study.

PURPOSE: Cancer pain, as a common symptom in patients with advanced cancer, seriously affects quality of life. A metabolism-based therapeutic strategy involving glucose metabolic regulation (hereafter pragmatically referred to as Warburg therapy) may offer clinical benefits for cancer pain management. This study aimed to evaluate the analgesic potential of this metabolic modulation strategy in patients with advanced cancer. METHODS: This was a single-institution retrospective pre-post cohort study involving 71 patients with advanced cancer treated with Warburg therapy. The primary endpoint wa…

[Biopsychosocial intervention for chronic non-cancer pain in Primary Care].

BACKGROUND: Non-oncological chronic pain is one of the leading causes of disability and reduced quality of life worldwide. Its management in Primary Care is essential due to its accessibility and continuity. Based on the biopsychosocial model, which integrates physical, psychological, and social factors, this study evaluated the effectiveness of a group intervention for patients with non-oncological chronic pain treated in this setting. METHODS: A randomized, controlled, single-blind clinical trial was conducted with 64 participants divided into an experimental group (n=31) and a control grou

Multimodal Pharmacologic Strategies for Chronic Cancer Pain: A Systematic Review.

Cancer pain remains inadequately controlled in some patients despite opioid therapy, particularly when neuropathic or mixed pain mechanisms are present. This systematic review evaluated the efficacy and safety of non-opioid or adjuvant analgesics used alongside opioids in adults with chronic cancer pain. MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL) were searched for English-language randomized controlled trials published from January 2010 to August 2025. Two reviewers independently selected studies, extracted data, and assessed risk of bias using Cochrane Risk …

Spinal analgesia in cancer pain management-MASCC general practice recommendations.

Patients with cancer pain refractory to conventional medical management may benefit from spinal analgesia, although there are some critical points regarding some aspects which cannot be examined by an evidence-based approach. A group of experts was selected by MASSC to provide clinical practice advice on the use of spinal drug delivery in patients with cancer-related pain. Refractory cancer pain should be considered a condition in which a patient has failed to receive adequate analgesia or has developed uncontrolled side effects after comprehensive pain management. The intrathecal route (IT) w…

Current cannabis use and pain management among US cancer patients.

BACKGROUND: National studies reporting the prevalence of cannabis use have focused on individuals with a history of cancer without distinction by their treatment status, which can impact symptom burden. While pain is a primary motivation to use cannabis in cancer, the magnitude of its association with cannabis use remains understudied. METHODS: We examined cannabis use and pain management among 5523 respondents of the Behavioral Risk Factor Surveillance System with a cancer history. Survey-weighted prevalence proportions of respondents' cannabis use are reported, stratified on cancer treatmen
