[A rare association: tetralogy of Fallot and Ebstein's anomaly].

The association of Tetralogy of Fallot and Ebstein's anomaly is very rare. The authors report a case in a 22 month old baby. As the Ebstein's anomaly was asymptomatic, the surgical indication for repair of the tetralogy of Fallot was the only planned procedure. During surgery, right ventricular dysfunction made it necessary to perform a cavobipulmonary anastomosis. The postoperative course was simple. Very few cases of this type have been found in the literature.

Left ventricular diastolic dysfunction in Ebstein's anomaly.

This study was performed to evaluate left ventricular (LV) diastolic function in patients with Ebstein's anomaly using Doppler echocardiography. We found that LV abnormal relaxation in this anomaly cannot be explained by right ventricular volume overload alone. Furthermore, LV diastolic dysfunction persists even after intracardiac repair.

[Ebstein's anomaly--experiences with surgical treatment in adulthood].

INTRODUCTION: Long time results with operative treatment of Ebstein anomaly were examined. PATIENTS AND METHODS: From January 1985 to March 2001 16 patients with Ebstein anomaly were operated on. Ages ranged from 16 to 49 years at the time of operation. In 7 cases tricuspid valve repair was possible, and in 9 cases prosthetic valve was inserted. In all but one biological prosthesis has been used. In 15 cases atrial septal defect occurred as a concomitant anomaly, which was closed by direct suture in 9 cases and with patch (2 Dacron, 4 pericardium) in 6 cases. RESULTS: There was no early deat…

Ebstein's anomaly: repair based on functional analysis.

OBJECTIVE: 'Classical' repair of Ebstein's anomaly is usually performed with transverse plication of the atrialized chamber. However, the anterior leaflet has restricted motion which is an important factor of the tricuspid valve insufficiency. We studied the long term results of mobilization of the anterior leaflet associated with longitudinal plication of the right ventricule. METHODS: From 1980 to July 2002, 191 patients (mean age 24.4+/-15 years (1-65)) were operated on. Anterior leaflet function was assessed on pre-op echocardiography and on surgical examination. Conservative surgery was

Paradoxically normal septal motion in Ebstein's anomaly.

Paradoxical septal motion has been reported as characteristic of Ebstein's anomaly. The patient reported here has the characteristic apical displacement of the tricuspid value, but septal motion is uncharacteristically normal. Because there is only mild tricuspid regurgitation, it is likely that the absence of right ventricular volume overload accounts for the normal septal motion in this patient.

Modified Starnes operation for neonatal Ebstein's anomaly.

We report the case of a severely symptomatic neonate with Ebstein's anomaly. A modified Starnes operation was performed, but insufficient drainage of venous blood returning through thebesian veins caused overdistention of the right ventricle and severe left ventricular dysfunction. Urgent reestablishment of right ventricular-right atrial communication successfully resolved these problems.

Ebstein's anomaly of the tricuspid valve complexities and strategies.

Ebstein's anomaly of the tricuspid valve is a fascinating but very complex congenital malformation with a wide spectrum of anatomic and clinical variations. A brief review is given of the clinical presentation and therapeutic options. We need further functional studies to understand the relationship between clinical severity and anatomic derangement. This will lead to better decision-making regarding the indications for and timing of surgery, which are still problematic.

[Ebstein's anomaly: valvular replacement in pediatric patients].

The surgical treatment of Ebstein's anomaly is still controversial. Therefore we have retrospectively studied the results of tricuspid valve replacement (TVR) performed for this anomaly at Sainte Justine Hospital. From October 1977 to December 1997, 9 patients with Ebstein's anomaly, aged from 31 to 248 months (mean 176 +/- 66), have undergone TVR. Eight children were in functional class III or IV (NYHA), while one was in class II. Seven patients underwent plication of the atrialized right ventricular segment. Eight bioprostheses (ranging in diameter from 31 to 35 mm) and one mechanical prosth

Ebstein's anomaly--an autopsy study of 28 cases.

Twenty eight autopsy specimens of Ebstein's anomaly were studied in order to evaluate the morphologic features of the abnormal tricuspid valve. All cases showed marked dilatation of the original tricuspid annulus, a normally positioned anterior leaflet and variable downward displacement of the posterior and septal leaflets. Sixteen cases showed a very large anterior leaflet. All three leaflets showed dysplastic features and a wide range of anatomic abnormalities in the valve and valve apparatus. A thin walled atrialised right ventricle was present in nine cases. Associated cardiac anomalies we

Radiofrequency ablation of tachyarrhythmias in patients with Ebstein's anomaly.

We performed radiofrequency catheter ablation in five patients associated with Ebstein's anomaly to cure their refractory tachyarrhythmias. The presenting arrhythmias were four cases of orthodromic circus movement tachycardia using accessory pathways as a requisite limb, including one case of a Mahaim fiber and one of atrial flutter of common variety. All accessory pathways, including the Mahaim fiber, were ablated by RF energy delivered through the catheter placed at the AV annulus rather than the displaced anatomical AV groove. Interestingly, the antegrade or retrograde conduction interval o

Results after surgical repair of Ebstein's anomaly.

BACKGROUND: Ebstein's anomaly of the tricuspid valve is a complex malformation. Various operations have been undertaken with varying results. Because valve replacement yielded poor results, surgical treatment has focused on valvuloplasties. METHODS: Between April 1974 and February 1995, 60 patients with Ebstein's anomaly underwent surgical repair. Age ranged from 5 months to 54 years. In 56 patients (93.3%), tricuspid valvuloplasty was feasible, mainly by creating a monocusp valve with the single-stitch technique. The other 4 patients had valve replacement with a bioprosthesis. Six reoperatio

Epidural analgesia for labour in a patient with Ebstein's anomaly.

Ebstein's anomaly is an uncommon congenital cardiac defect which is associated with cyanosis and arrhythmias. There have been very few previous reported cases of successful outcome in pregnancy in women with this disorder. We describe the successful analgesic management of an obstetric patient who had been known to have Ebstein's anomaly since childhood. Her first pregnancy was uneventful and analgesia during labour was provided by an epidural. During her second pregnancy she presented to our hospital as her condition had deteriorated. Symptomatic control was achieved with digoxin. Despite thi

MR imaging of Ebstein's anomaly of the tricuspid valve.

Ebstein's anomaly is an uncommon congenital developmental abnormality of the tricuspid valve that has a wide spectrum of pathologic anatomy. With the evolution of various surgical techniques, through assessment of the morphologic features and type of Ebstein's anomaly assumes greater practical importance. Although the diagnosis of Ebstein's anomaly is usually straightforward and is based on echocardiographic findings, there are limitations in defining pathologic anatomy in some cases. The purpose of this essay is to illustrate the MR findings in various types of Ebstein's anomaly.

A pregnant woman with Ebstein's anomaly. A case report.

Ebstein's anomaly is characterised by downward displacement of the tricuspid valve into the right ventricle. The most deformed leaflets are the septal and mural ones. The abnormally situated tricuspid orifice produces a portion of the right ventricle lying between the atrioventricular ring and the origin of the valve. This proximal segment is atrialized. Commonly associated anatomical defects include a atrial septal second defect, a ventricular septal defect, pulmonary stenosis or atresia, and mitral valve prolapse. An accessory conduction pathway (Wolf-Parkinson White syndrome) was found in t

Ebstein's anomaly: a long-term study of survival.

This study examines data regarding the survival of patients with Ebstein's malformation. Of 61 patients studied at the University of Minnesota, 31 are currently alive, with the average survival being 15 years. Several factors adversely affected survival: NYHA functional classification, hemoglobin level, right atrial pressure, symptoms during the neonatal period, and cardiac surgery. Data from a multi-institutional study collected during the past decade adds information regarding survival. Of 18,281 patients in the study, 71 (0.4%) had Ebstein's malformation. Sixteen of these were among 7235 in

[Anesthesia for cesarean section in a patient with Ebstein's anomaly].

A 30-year-old woman with Ebstein's anomaly was scheduled for Cesarean section at 38 week gestation because of latent fetal distress. After arterial and central venous catheters were inserted, general anesthesia was started. Anesthesia was induced smoothly and rapidly through intravenous route. Following induction, her hemodynamics was stable until post-operative period. The baby weighed 1564g and had an Apgar score of 8 at 1 minute. A patient with Ebstein's anomaly during pregnancy and anesthesia has potential for a variety of hemodynamic disturbances. This case illustrates the importance of c

Ebstein's anomaly: unusual presentation and surgical repair.

Ebstein's anomaly is the most significant congenital anomaly involving the tricuspid valve. This lesion may present in a simple form or may be complicated by other lesions. Patients with this anomaly may vary from mildly symptomatic to severely debilitated. The role of surgical treatment is not uniformly agreed upon, and the optimal procedure for correction or palliation is not clearly defined. Because of the wide spectrum of morphologic abnormalities which may be present in Ebstein's anomaly, no one particular operation may always be satisfactory. We and other groups continue to search for op
