Medial transport of the fibula using the Ilizarov device for reconstruction of a massive defect of the tibia in two children.

We present two children with massive defects of the tibia and an associated active infection who were treated by medial transport of the fibula using the Ilizarov device. The first child had chronic discharging osteomyelitis which affected the whole tibial shaft. The second had sustained bilateral grade-IIIB open tibial fractures in a motor-car accident. The first child was followed up for three years and the second for two years. Both achieved solid union between the proximal and distal stumps of the tibia and the fibula, with hypertrophy of the fibula. The first child had a normal range of m

Fibular Rod Osteosynthesis in Ankle Fractures With Compromised Soft Tissue.

BACKGROUND: Complications after plate and screw fixation of ankle fractures are frequently reported in the literature, with a higher rate in patients with advanced age, comorbidities, and poor skin conditions. A reduced complication rate has been reported with intramedullary nailing (IMN) of the fibula; however, the indication has been based on the surgeon's preferences. We report the results after IMN in patients with compromised soft tissue exclusively. METHODS: A total of 71 patients with 72 distal fibula fractures were included in this retrospective study. Information about medical histor…

Cluster analysis demonstrates co-existing sites of fragility fracture and associated comorbidities.

UNLABELLED: We undertook cluster analysis in 11,003 patients who had sustained ≥ 1 fragility fracture, to find associations between fracture … ore - 1.12 SD. FF of the forearm was more frequent (n = 5045), most commonly co-existing with tibia/fibula fractures. Three FF site clusters were identified: ankle and elbow; forearm, tibia/fibula, ribs and spine; and pelvis, femur and humerus. When including comorbidities, four clusters were identified: forearm, tibia/fibula, spine, associated with family history …

Use of Vascularized Fibular Epiphyseal Transfer with Massive Bone Allograft for Proximal Humeral Reconstruction in Children with Bone Sarcoma.

BACKGROUND: The vascularized fibula epiphyseal transfer provides a reconstructive option for longitudinal growth after oncologic resection of the proximal humerus in pediatric patients. However, postoperative fractures and poor shoulder function are common. The purpose of this review was to introduce a composite approach in oncologic reconstruction of the proximal humerus and assess its clinical outcomes. METHODS: We retrospectively investigated five children (3 osteosarcoma and 2 Ewing's sarcoma) who underwent biological reconstruction with combination of vascularized fibula epiphyseal trans

Incidence and risk factors of lower leg fractures in Belgian soccer players.

INTRODUCTION: Soccer is the world's most popular sport and one that is physically demanding and highly competitive. Consequently, the rate o … mmon (37%), followed by foot and tibia fractures (33% and 22%, respectively). The least common were fibula fractures, which accounted for just 9%. A higher incidence of every type of LLF was observed in older and amateur-level soccer players, when compared with their younger and professional counterparts. Male players experienced more tibia and foot fractures, whe …

Long-Term Complications in Patient-Specific Plates for Maxillary Reconstruction.

OBJECTIVES: Virtual surgical planning (VSP) allows for optimal reconstruction of maxillary defects with fibula free flaps. Current data are limited regarding long-term complications of patient-specific plates (PSPs) in this setting. Our objective was to determine long-term complications of PSPs in maxillary reconstruction using fibula free flaps. METHODS: A retrospective chart review was performed on patients undergoing maxillary reconstruction with a fibula free flap and PSP between January 2010 to July 2022. The primary outcomes were plate-related complications, which were defined as plate

[Bilateral fibula graft. Biological reconstruction following resection of malignant bone tumors].

Bilateral vascularized fibula graft (BVFG) is actually not a satisfying method for the replacement of metadiaphyseal defects of the femur and tibia in young patients suffering from malignant bone tumors. This reconstruction was used in five patients (two female, three male, average age 15.2 years, femur n=3, tibia n=2) undergoing metadiaphyseal resection of malignant bone tumors between November 2000 and August 2003. The median length of the defect to be bridged was 16.4 cm (range 11.5-23). In the two cases of tibia reconstruction, the ipsilateral fibula was transposed into the osseous defect

Isolated fractures of the shaft of the tibia.

We have reviewed 106 fractures of the tibia with an intact fibula. Most patients were treated in above-knee plaster casts and allowed to walk. Only 2 fractures failed to unite and there were 4 refractures. In 27 patients (25 per cent) the fracture healed with more than 5 degrees of varus angulation. The cause of this deformity and its prevention are discussed.

Quantification by dual photonabsorptiometry of local bone loss after fracture.

Total bone mineral and bone mineral density modifications were evaluated during fracture healing of long bones in patients with traumatic fractures of the lower limbs, using an experimental model based on the dual photonabsorptiometry technique. Seven patients (five males and two females; mean age, 19.5 years; range, 17-23 years) with tibia and fibula midshaft open fractures treated with osteosynthesis were studied. Dual energy gadolinium-153 photonabsorptiometry was used to measure distal leg bone loss at a skeletal segment distant from the fracture to exclude callus formation. Total bone min

Single cortical positional screw for fixation of fibular head instability.

Fibular head instability is a rare, isolated injury in veterinary medicine, with no previous case reports in the veterinary literature. The fibular head is secured proximo-distally by the lateral collateral ligament and craniocaudally by the cranial and caudal fibular head ligaments. A 1-year-old, 26-kg, spayed female border collie mix dog was referred for chronic bilateral Salter Harris-II tibial tuberosity avulsion fractures. The dog was non-weight-bearing on the right pelvic limb, with pain and mobility localized to the right fibular head. Preoperative radiographs showed no fibular patholog

Early posterior column internal fixation: A staged treatment of type C3 tibial pilon fracture.

Pilon fractures represent a challenging subset of tibial fractures. The management of AO/OTA Type C3 fractures remains complex due to associated complications and lack of clear guidelines for surgical timing and methods. A prospective cohort study was conducted to evaluate two staged treatment strategies for AO/OTA Type C3 tibial pilon fractures. The study focused on assessing surgical difficulty, complications, and patient prognosis. One group of patients received early internal fixation of the fibula and tibial posterior column combined with external fixation, while the other group received

A Novel Method of Fibula Flap In Situ Distraction Osteogenesis Prior to Flap Transfer.

BACKGROUND: The fibula flap is the workhorse for mandibular reconstruction, but fibula bone width is not ideal to match mandibular height. In this study, in situ widening of the fibula with distraction osteogenesis before transfer is evaluated as a solution. The authors present a proof of concept of this technique with a patient series, including one patient who has undergone subsequent orthognathic surgery of the reconstructed mandible. METHODS: A retrospective review of patients undergoing the authors' technique was performed. A longitudinal fibula osteotomy was made in situ and distraction

Restoration of the patient-specific anatomy of the distal fibula based on a novel three-dimensional contralateral registration method.

PURPOSE: Posttraumatic fibular malunion alters ankle joint biomechanics and may lead to pain, stiffness, and premature osteoarthritis. The accurate restoration is key for success of reconstructive surgeries. The aim of this study was to analyze the accuracy of a novel three-dimensional (3D) registration algorithm using different segments of the contralateral anatomy to restore the distal fibula. METHODS: Triangular 3D surface models were reconstructed from computed tomographic data of 96 paired lower legs. Four segments were defined: 25% tibia, 50% tibia, 75% fibula, and 75% fibula and tibia.

Use of a posterior antiglide plate in a pediatric ankle fracture: a case report.

The authors describe the case of a 13-year-old male with a Salter-Harris II fracture-subluxation of the fibula. Open reduction and internal fixation with a posterior antiglide plate achieved mortise stability and avoided penetration of the fibular physis by fixation devices. After searching the Medline, EMBASE, CINAHL, and the Cochrane Library databases, to our knowledge, there have been no published reports describing the advantages associated with the use of a posterior antiglide plate for fixation of the immature fibula.

Compression neuropathy of the peroneal nerve caused by a ganglion.

Peroneal nerve injury usually results from fracture of the fibula neck or direct pressure from an ill-fitting plaster cast. Clinically, these patients present with "drop foot." Compression neuropathy caused by a ganglion is rarely encountered. This report concerns the clinical and radiologic evaluation of a 57-year-old man admitted to our clinic with left drop foot. A mass compressing the peroneal nerve at the level of the fibula neck was excised totally and diagnosed as a ganglion.
