Efficacy of heel lifts for insertional Achilles tendinopathy (LIFTIT): A randomised feasibility trial.

OBJECTIVES: Insertional Achilles tendinopathy is a common and disabling condition. This trial aimed to determine the feasibility of conducting a parallel group randomised trial to evaluate the efficacy of heel lifts compared to a sham intervention for reducing pain intensity associated with insertional Achilles tendinopathy. METHODS: Twenty-six people with insertional Achilles tendinopathy were randomised to either the heel lift group or sham intervention group. Outcome measures were obtained at baseline, 4, 8 and 12 weeks. The primary outcome was feasibility, evaluated according to demand (r…

Revisiting Biomarkers of Cardiac Allograft Vasculopathy: Addressing the Achilles Heel of Heart Transplantation.

Nearly half of heart transplant recipients will be diagnosed with cardiac allograft vasculopathy (CAV) within five years after transplantation. Advanced CAV can lead to worsening heart failure as well as arrhythmias and sudden cardiac death. The only curative therapy for end-stage CAV is re-transplantation. Current diagnostic methods are invasive and limited by poor sensitivity in early disease. Despite its high prevalence in the post-transplantpopulation, the underlying pathophysiology of this condition has yet to be fully described. It is thought to be primarily related to endothelial dysfun

Single sensor measurement of heel-height during the push-off phase of gait.

Objective. In healthy gait a forceful push-off is needed to get an efficient leg swing and propulsion, and a high heel lift makes a forceful push-off possible. The power of the push-off is decreased with increased age and in persons with impaired balance and gait. The aim of this study was to evaluate whether a wearable equipment (Striton) and algorithms to estimate vertical heel-height during gait from a single optical distance sensor is reliable and feasible for clinical applications.Approach. To assess heel-height with the Striton system an optical distance sensor was used to measure the di…

Midsole Properties Affect the Amplitude of Soft Tissue Vibrations in Heel-Toe Runners.

INTRODUCTION: Soft tissue vibrations can generate discomfort and may necessitate a greater energy demand to preserve an efficient motion in … RESULTS: The variables apeak and PSD[8-55] decreased for VM and GM in the viscous condition. Before heel strike, low-frequency EMG activity decreased for VM, and high-frequency EMG activity tended to decrease for GM in the viscous condition. The damping D was reduced only for VM, and fpeak was unchanged. CONCLUSIONS: A more viscous midsole substantial …

Lower Extremity Stiffness Predicts Ground Reaction Force Loading Rate in Heel Strike Runners.

PURPOSE: High vertical ground reaction force (vGRF) loading rates are thought to contribute to lower extremity injuries in runners. Given that elevated lower extremity stiffness has been reported to be associated with increased GRFs, the purpose of the current study was to determine if overall lower extremity stiffness, individual joint angular excursions and/or torsional stiffness are predictive of the average vGRF loading rate during running. METHODS: Forty heel strike runners (20 men and 20 women) ran overground at a speed of 3.4 m·s. Average vGRF loading rate, lower extremity stiffness, a…

Does Heel Height Cause Imbalance during Sit-to-Stand Task: Surface EMG Perspective.

The purpose of this study was to determine whether electromyography (EMG) muscle activities around the knee differ during sit-to-stand (STS) and returning task for females wearing shoes with different heel heights. Sixteen healthy young women (age = 25.2 ± 3.9 years, body mass index = 20.8 ± 2.7 kg/m2) participated in this study. Electromyography signals were recorded from the two muscles, vastus medialis (VM) and vastus lateralis (VL) that involve in the extension of knee. The participants wore shoes with five different heights, including 4, 6, 8, 10, and 12 cm. Surface electromyography (sEMG…

Dendritic cells as Achilles' heel and Trojan horse during varicella zoster virus infection.

Varicella zoster virus (VZV), a human alphaherpesvirus, causes varicella and subsequently establishes latency within sensory nerve ganglia. Later in life VZV can reactivate to cause herpes zoster. A reduced frequency of VZV-specific T cells is strongly associated with herpes zoster illustrating that these immune cells are central to control latency. Dendritic cells (DCs) are required for the generation of VZV-specific T cells. However, DCs can also be infected in vitro and in vivo allowing VZV to evade the antiviral immune response. Thus, DCs represent the immune systems' Achilles heel. Unique…

Fast, hungry and unstable: finding the Achilles' heel of small-cell lung cancer.

Over 95% of patients with small-cell lung cancer (SCLC) die within five years of diagnosis. The standard of care and the dismal prognosis for this disease have not changed significantly over the past 25 years. Some of the characteristics of SCLC that have defined it as a particularly virulent form of cancer -- rapid proliferation, excessive metabolic and angiogenic dependence, apoptotic imbalance and genetic instability -- are now being pursued as tumor-specific targets for intervention both in preclinical and early phase clinical studies. Here, we summarize areas of ongoing anti-cancer drug d

Risk Factors for the Development of Pressure Injury in the Heel Area in Critically Ill Patients.

Background/Objectives: Pressure injuries on the heels of critically ill patients occurring during hospitalization are a global problem. Risk factors include comorbidities, distal perfusion disorders, multiple organ failure, pharmacotherapy, and immobilization associated with mechanical ventilation. These factors affect microperfusion quality in the heels. The presence of friction, shear, and compressive forces contributes directly to local tissue hypoxia and secondary tissue destruction in the heels. This study aimed to assess the impact of risk factors on the development of pressure injuries …

Texture differences of microchambers and macrochambers in heel pads between the elderly with and without diabetes.

OBJECTIVE: This study aims to use the texture analysis of ultrasound images to distinguish the features of microchambers (a superficial thinner layer) and macrochambers (a deep thicker layer) in heel pads between the elderly with and without diabetes, so as to preliminarily explore whether texture analysis can identify the potential injury characteristics of deep tissue under the influence of diabetes before the obvious injury signs can be detected in clinical management. METHODS: Ultrasound images were obtained from the right heel (dominant leg) of eleven elderly people with diabetes (DM gro…

Staged management of a large ischemic heel ulcer in a diabetes patient: a case report.

Heel ulcer is one of the severe complications of patients with diabetes mellitus, which poses a high risk for foot infection and amputation, especially in patients with peripheral arterial disease and neuropathy. Researchers have searched for new treatments for treating diabetic foot ulcers in recent years. In this case report, we demonstrated the treatment of large ischemic ulcers for the first time in a diabetic patient. The overall treatment goal of this patient was designed to improve blood supply to her diseased lower extremities and close the ulcer. This two-stage reconstruction approach…

Achilles' Heel-The Significance of Maintaining Microenvironmental Homeostasis in the Nucleus Pulposus for Intervertebral Discs.

The dysregulation of intracellular and extracellular environments as well as the aberrant expression of ion channels on the cell membrane are intricately linked to a diverse array of degenerative disorders, including intervertebral disc degeneration. This condition is a significant contributor to low back pain, which poses a substantial burden on both personal quality of life and societal economics. Changes in the number and function of ion channels can disrupt the water and ion balance both inside and outside cells, thereby impacting the physiological functions of tissues and organs. Therefor

The effects of small variations in shoe heel height on gait in people with a transtibial amputation.

BACKGROUND: Shoe heel height is considered to influence prosthetic alignment, walking comfort, and gait symmetry in people with a transtibial amputation (TTA). However, research on the effect of heel height is scarce, and no evidence is available on the effects of variations smaller than 20 mm. These small heel height variations between store-bought shoes are often overlooked by people with an amputation and may cause secondary musculoskeletal problems in the long term. OBJECTIVE: To examine the effects of small increases in heel height on gait symmetry in people with a TTA and healthy indivi…
