Perioperative course of FXIII in children undergoing major surgery.

BACKGROUND: Acquired deficiency of FXIII because of perioperative hemodilution has been described several times in adults; however, data in children are scarce. We performed a prospective observational trial to evaluate the intraoperative course of FXIII in children undergoing elective major surgery. METHODS: Blood samples were repeatedly taken from 46 children aged 0.3-16 years undergoing major surgery. Concentrations of FXIII and fibrinogen, thrombelastometry by ROTEM®, and cell count were assessed intraoperatively. RESULTS: A significant decrease in FXIII concentration (median 60%; IQR 49…

Intracavernosal saline infusion decreases intraoperative blood loss during radical retropubic prostatectomy.

OBJECTIVE: Radical retropubic prostatectomy (RRP) is associated with intraoperative blood loss. We studied whether regional hemodilution using intracavernosal saline infusion has an impact on blood loss during RRP. METHODS: This is a prospective, randomized controlled study. There were 16 patients treated with intracavernosal saline infusion, and 17 did not receive infusion and served as the control group. Patients who received neoadjuvant therapy and those who underwent pelvic lymph node dissection or salvage RRP were excluded. Both corpora were infused in a pressure of 80-cm water. Clinical…

[Pathophysiologic studies in acute blood loss].

The authors induced acute blood loss in dogs and examined the changes in hemodynamics, hemodilution, blood coagulation acid-base balance and tissue respiration. They found that the most favourable hours for initial renimation were the first two hours. They used the Bulgarian plasma substitute reoisodex with very good effect on the indicated parameters and microcirculation. As a result of this the time for the appearence of irreversible stage in grave hypovolemia was prolonged with hours.

[Autotransfusion technic in surgery of craniostenoses in children].

The extensive blood loss during the surgical correction of craniosynostosis in infancy needs usually homologous transfusion. The Authors report a program of preoperative and intraoperative hemodilution and their experience in six infants, the very first in pediatric neurosurgery. Prevention of homologous blood transfusion achieved in 4 out 6 cases can be considered a success that could be further improved introducing minor changes in our protocol.

[Neonatal hyponatremia from maternal origin. Three cases (author's transl)].

The effect of the treatment was controlled by measurements of plasma renin activity and aldosterone concentration. These observations concern maternal toxemia treated by diuretic and dietary sodium restriction two weeks before delivery. In two cases, labor was augmented with oxytocin infusion. These cases illustrate the respective role of sodium depletion and hemodilution in the occurrence of maternal and neonatal hyponatremia. They emphasize the amount of sodium supplementation in these newborn infants.

Association between Functional Parameters of Coagulation and Conventional Coagulation Tests in the Setting of Fluid Resuscitation with Balanced Crystalloid or Gelatine: A Secondary Analysis of an In Vivo Prospective Randomized Crossover Study.

Functional point-of-care tests (POCTs) have evolved into useful tools for diagnosing disorders of blood coagulation and fibrinolysis. We aimed to describe the in vivo association between standard and functional parameters of coagulation and fibrinolysis in the setting of acute hemodilution induced by an infusion of balanced crystalloid or synthetic gelatine solutions. This prospective randomized crossover in vivo study included healthy male volunteers aged 18-30 years. Enrolled participants were randomly assigned to receive either the Optilyte® or Geloplasma® infusion. Laboratory analysis incl…

Liposome-encapsulated hemoglobin attenuates cardiac dysfunction and sympathetic activity during hypohemoglobinemic shock.

The use of liposome-encapsulated hemoglobin (LHb), which is a cellular Hb, has been demonstrated to be beneficial in the treatment of hypohemoglobinemic shock. As a molecule of appropriate size (220 nm) that can carry oxygen, LHb may ameliorate cardiac dysfunction during lethal hemodilation. The purpose of this study was to determine the efficacy of LHb transfusion in relieving cardiovascular dysfunction in a rat model of lethal progressive hemodilution. Over the course of 150 min, rats were subjected to blood withdrawal (0.2 mL/min) and simultaneously transfused with LHb, washed rat red blood…

[Influencing hemostasis in substitute solution treatment].

Hemostasis is an important intricately regulated homeostatic process. During the hospitalization a critically ill patient is often subjected to various external and internal stimuli which have abilities to influence the hemostasis. Administration of substitute solutions could be related to such an adverse effect. This paper tries to describe main mechanisms leading to impair the hemostatic balance during the fluid therapy and outline possibilities of their monitoring. The work also deals with every basic arteficial substitute solutions individually in term of their influence on coagulation. Th

Autologous blood transfusion: evaluation of an alternative strategy in reducing exposure to allogeneic blood transfusion.

The perceived risk of transfusion-transmitted disease led to the rejuvenation of autologous blood transfusion (ABT). ABT, a process in which the blood donor and recipient are the same, is increasingly becoming an integral component of the elective surgical protocol in many institutions. Various methods of ABT are being utilized. These include: preoperative blood donation, in which the patient donates blood prior to surgery and the blood is stored for an expected need during or after surgery; acute normovolemic hemodilution, in which blood is collected immediately prior to surgery and replaced …

[Comparative evaluation of different variants of transfusion tactics during spinal surgery in children with scoliosis].

The results of a comparative estimation of two variants of transfusion methods used in 95 children and juveniles with scoliosis in operations on the vertebral column are described. Artificial hemodilution against the background of ganglionic blockade was shown to have certain advantages in order to prevent great blood loss. Hypervolemia was found not to result in overloading the cardio-vascular system even in children with the most pronounced deformity of the vertebral column and the accompanying pathology of organs and systems.

PEG-albumin supraplasma expansion is due to increased vessel wall shear stress induced by blood viscosity shear thinning.

We studied the extreme hemodilution to a hematocrit of 11% induced by three plasma expanders: polyethylene glycol (PEG)-conjugated albumin (PEG-Alb), 6% 70-kDa dextran, and 6% 500-kDa dextran. The experimental component of our study relied on microelectrodes and cardiac output to measure both the rheological properties of plasma-expander blood mixtures and nitric oxide (NO) bioavailability in vessel walls. The modeling component consisted of an analysis of the distribution of wall shear stress (WSS) in the microvessels. Our experiments demonstrated that plasma expansion with PEG-Alb caused a s…

Proteolytic enzyme levels during cardiopulmonary bypass.

Pulmonary damage resembling "shock lung" is frequently seen following cardiopulmonary bypass. In many of these cases a rise in the plasma level of proteolytic enzymes has been observed. In order to establish whether changes in plasma proteinase activity occur as a result of cardiopulmonary bypass per se, or whether hemodilution and surgical tissue destruction may be responsible, three groups of animals were studied. Control dogs were anesthetized, heparinized, and transfused rapidly with Ringer's lactate to a hematocrit of less than 30%. In the second group hemodilution was combined with a sha…

Pharmacokinetics of Methadone in Adult Patients Undergoing Cardiac Surgery With Cardiopulmonary Bypass.

BACKGROUND: Cardiopulmonary bypass (CPB) induces profound physiological changes that may alter the pharmacokinetics of methadone. We aimed t … methadone concentrations. The influence of CPB on methadone pharmacokinetics was best described by hemodilution in a fixed volume of 1.5 L (CPB circuit volume) and by sequestration from the CPB components (CL SEQ = 93.4 L/h, 95%CI 59-124, P < .01). The observed effect of CPB in methadone pharmacokinetics can be compensated by giving a supplementary bolus dose …

Open-heart surgery in Jehovah's Witnesses: experience in a Canadian hospital.

Jehovah's Witnesses have religious belief precluding the use of blood. Few centers have attempted open-heart surgery bound by such strictures; as a result, availability of therapy for such patients has been limited. Many groups that have extensive experience with hemodilution for cardiopulmonary bypass have noted that these procedures can often be done with little or no use of blood. Our experience with 21 adult patients is presented in this paper.

Improved Preprocedure Hematocrit in Chronic RBC Exchange Patients Following Splenectomy: A Case Report.

BACKGROUND: Chronic RBC exchange (RCE) is an established therapeutic strategy used to prevent the development of serious complications in patients with sickle cell disease and beta thalassemia. A subset of these patients have an accelerated decline of transfused red cells, leading to suboptimal exchange transfusions since the preprocedure hematocrit (HCT) is too low to allow for isovolemic hemodilution. These patients often have concomitant splenomegaly. METHODS: In our institution, we had 3 patients who had rapid decline in HCT post-RCE and who underwent a splenectomy. We compared the pre- a…

Identification and Recent Approaches for Evaluation and Management of Rehabilitation Concerns for Patients with Freeman-Burian Syndrome: Principles for Global Treatment.

Bloodless pediatric cardiac surgery requiring the use of cardiopulmonary bypass (CPB) remains a challenge for the entire operating room (OR) team. The amount of circulating blood volume to pump prime volume mismatch of small patients results in hemodilution that frequently results in transfusion of allogeneic blood products. Patients of families of the Jehovah's Witness (JW) faith reject the use of these products because of religious beliefs. Our institution is a referral center for children of JW families because we have developed techniques to minimize blood loss with the hope of performing …
