Chronic Myelomonocytic Leukemia: Focus on Clinical Practice.

Chronic myelomonocytic leukemia (CMML) is a clonal stem cell disorder with features that overlap those of myelodysplastic syndromes (MDSs) and myeloproliferative neoplasms (MPNs). Chronic myelomonocytic leukemia often results in peripheral blood monocytosis and has an inherent tendency to transform to acute myeloid leukemia. Clonal cytogenetic changes are seen in approximately 30% of patients, and molecular abnormalities are seen in more than 90%. Gene mutations involving TET2 (∼60%), SRSF2 (∼50%), ASXL1 (∼40%), and RAS (∼30%) are frequent, with nonsense and frameshift ASXL1 mutations being th

Clinical Pharmacist's Role in Preventing and Managing Drug-Induced Oral Ulceration: A Bisphosphonate Case in an Older Adult Care Setting.

Bisphosphonate-induced oral mucosal ulcers are rare adverse events distinct from medication-related osteonecrosis of the jaw. This case demonstrates successful conservative management through the modification of drug administration in an older adult care facility. A 91-year-old woman with dementia developed bilateral floor of the mouth and sublingual ulceration that persisted for 2 weeks. Histopathological examination excluded malignancy and confirmed a drug-induced etiology. Management involved correcting the minodronic acid administration technique without discontinuing therapy, which result

Balancing the risk of complications in foot and ankle surgical patients taking antithrombotic medication.

UNLABELLED: This review intends to provide guidance regarding perioperative management of anticoagulation and antiplatelet drug therapy as they relate to foot and ankle surgery. Venous and arterial thromboembolism are conditions in which the blood clots inappropriately, causing considerable morbidity and mortality. With an increase in awareness of thromboembolic risk factors and expansion of therapeutic options, more patients are routinely taking antithrombotic medication. When these patients require invasive procedures, a decision needs to be made if antithrombotic medication should be held p

Management of residual volumes in intermittent medication infusions in hospitals: a scoping review protocol.

BACKGROUND: Administration of intermittent medication through intravenous infusions is common practice. However, recent literature questions whether current practices deliver full doses of prescribed medications. OBJECTIVE: This protocol outlines the methodology intended to profile literature describing management of residual volume in medication administration lines. METHODS: The proposed review will use Joanna Briggs Institute scoping review methodology to scope the literature, following a six-step framework. Searches will be conducted in electronic databases, and websites of societies pro

Heart Failure in Older People Part 1: Disease State Review and Lifestyle Interventions.

The care for patients with heart failure (HF) has evolved greatly over the past decade. While new guidelines have provided more clarity on categorization and staging, and novel agents have been approved for use, there are still questions surrounding the optimal strategies as they relate to diet and exercise. Additionally, overall health care costs have increased for patients, driven in part by medication therapy. Given the myriad comorbidities associated with the diagnosis of HF, senior care pharmacists are positioned to positively impact the care for patients with HF, regardless of setting. A

Behavioral management of the triggers of recurrent headache: a randomized controlled trial.

This study was designed to evaluate the traditional advice to headache sufferers to avoid all triggers ('Avoidance'), and a novel approach to trigger management (Learning to Cope with Triggers - 'LCT') that included graduated exposure to selected triggers to promote desensitization. Individuals (84F, 43M) with migraine and/or tension-type headache were assigned randomly to one of four groups: Waiting-list (Waitlist); Avoidance; Avoidance combined with cognitive behavior therapy (Avoid + CBT); and LCT. Changes in headaches and medication consumption (in parentheses) from pre- to post-treatment

Treatment algorithm for oral anticoagulant and antiplatelet therapy in epistaxis patients.

BACKGROUND AND OBJECTIVES: There is currently little published guidance on the management of anticoagulant and antiplatelet medication in patients admitted with epistaxis. The routine practice of withholding such medication in an attempt to control the epistaxis is common in the UK. However, this practice is not evidence-based, is often unnecessary, and can be associated with significant morbidity. This study introduces a treatment algorithm for oral anticoagulant and antiplatelet therapy in epistaxis patients, validated through a completed audit cycle. METHODS: One hundred patients admitted

Prevalence and risk factors for medication-refractory reflux esophagitis in patients with systemic sclerosis in Japan.

BACKGROUNDS: Patients with systemic sclerosis (SSc) often have esophageal motility abnormalities and weak esophago-gastric junction (EGJ) barrier function, which causes proton pump inhibitor (PPI)-refractory reflux esophagitis (RE). The aims of this study were to clarify the current management of RE and prevalence and risk factors of medication-refractory RE in patients with SSc in Japan. METHODS: A total of 188 consecutive patients with SSc who underwent both esophageal high-resolution manometry (HRM) and esophagogastroduodenoscopy (EGD) were reviewed. The presence of RE and grades of the ga

Chemical pharmacotherapy options for managing adult acne.

The incidence of adult acne is increasing worldwide. Despite clinical overlap with conventional acne, it has distinct features. Areas covered: A literature search of English-language review articles, randomized control studies and retrospective studies conducted over the past 30 years was performed using PubMed and Google Scholar. Search terms included acne, adult, topical medication, oral medication and skin of color. We highlight important clinical features and treatment modalities pertinent to the evaluation and management of adult acne. Given the relative dearth of literature detailing tre

Optimising Medications in Older Vascular Surgery Patients Through Geriatric Co-management.

BACKGROUND: Prescribing of potentially inappropriate medications and under-prescribing of guideline-recommended medications for cardiovascular risk modification have both been associated with negative outcomes in older adults. Hospitalisation represents an important opportunity to optimise medication use and may be achieved through geriatrician-led interventions. OBJECTIVE: We aimed to evaluate whether implementation of a novel model of care called Geriatric Comanagement of older Vascular (GeriCO-V) surgery patients is associated with improvements in medication prescribing. METHODS: We used

Atropine-Induced Anticholinergic Syndrome Following Off-Label Sublingual Administration for Refractory Sialorrhea: A Case Report.

A sublingual atropine ophthalmic solution is increasingly used as an off-label therapy for the symptomatic management of refractory sialorrhea because of its rapid onset of action, ease of administration, and generally favourable safety profile when used at recommended doses. Although generally well-tolerated at therapeutic doses, systemic anticholinergic toxicity may occur following medication administration errors or unintentional overdose. We report the case of a 76-year-old man with recurrent head and neck squamous cell carcinoma receiving palliative systemic therapy who presented with acu

An Updated Review of Glucagon-Like Peptide-1 Receptor Agonists: Perioperative Implications, Guidance Statements, and Current Literature.

Glucagon-like peptide-1 receptor (GLP-1) agonist medications are becoming a commonly prescribed medication for patients undergoing surgical procedures. Perioperative management of this medication has become an important topic due to the known delayed gastric emptying leading to the potential for retained gastric contents elevating the risk for aspiration during anesthesia. This update will briefly review the history and mechanism of action of these medications. We will then pivot to discuss recent evidence and best practice strategies in managing perioperative patients on GLP-1 therapy.

A medical nutrition therapy primer for childhood asthma: current and emerging perspectives.

Asthma is the most common chronic disease in children. Prevalence has increased in the past 2 decades and has reached a plateau of approximately 9% of children in the United States, affecting about 6.7 million children. The increased prevalence of childhood asthma has paralleled the increased prevalence in childhood obesity. Changes in diet have also been implicated in the increased prevalence of asthma, among other risk factors. The main symptoms of asthma (ie, wheezing, coughing, and chest tightness) require medical evaluation and monitoring. The cornerstone of asthma management is medicatio

Psychotic symptoms in the elderly--assessment and management.

BACKGROUND: Psychotic symptoms in the elderly arise in mood disorders, schizophrenia, dementia, and delirium. OBJECTIVE: This article provides a brief overview of the presentation and management of each condition. DISCUSSION: Management follows logically from a thorough mental state examination, medical work up and accurate diagnosis. Depression and mania with psychotic symptoms respond best to an antidepressant and mood stabiliser respectively, usually in combination with antipsychotic medication. Severe cases require electroconvulsive therapy. Atypical antipsychotics are now the treatment
