The facilitators and barriers of nurse practitioner primary care practice ownership: A qualitative analysis.

BACKGROUND: The United States faces a growing primary care shortage driven by an aging population, rising chronic disease burden and a physician supply that is lagging behind demand. Nurse practitioners (NPs) are playing an increasingly central role in primary care delivery, and a small but growing number are opening independent practices. Despite their potential to strengthen the primary care safety net, empirical research on NP practice ownership remains limited. PURPOSE: Examine the facilitators and barriers to opening and sustaining NP-owned practices, particularly the structural factors

Chronic obstructive pulmonary disease: diagnosis, monitoring and chronic management in nursing practice.

Chronic obstructive pulmonary disease (COPD) is a progressive condition and a leading cause of morbidity and mortality in the UK. Characterised by persistent airflow limitation, breathlessness, cough and frequent exacerbations, it is often linked to smoking and other environmental exposures. This article reviews COPD from a nursing perspective, outlining definition, pathophysiology and evidence-based management. Pharmacological and non-pharmacological strategies are discussed, with emphasis on smoking cessation, inhaler technique, pulmonary rehabilitation and holistic care. The central role of

Beyond Necrosis: Early Recognition of Xylazine-Associated Wounds in Emergency Nursing Practice.

Xylazine has emerged as a common adulterant in the illicit drug supply and is increasingly associated with severe skin and soft tissue complications. Existing literature has largely focused on advanced wound presentations characterized by extensive tissue necrosis, hospitalization, and surgical intervention. Although these presentations are clinically significant, they may not reflect the full spectrum of xylazine-associated wounds encountered in emergency nursing practice. Early-stage lesions may present as subtle findings, including superficial ulcerations, papules, pustules, bumps, or multi

From Classroom to Clinic: A Qualitative Study on Newly Graduated Nurses' Transition to Practice.

BACKGROUND: The global nursing shortage and high turnover rates among newly graduated nurses (NGNs) pose significant challenges to their transition into professional roles. Understanding NGNs' experiences is crucial for identifying the factors contributing to turnover and developing targeted strategies that facilitate their integration into clinical practice, ultimately promoting long-term retention. While extensive research has explored this transition in developed countries, there remains a notable gap in studies focusing on NGNs' experiences in developing nations. This study aims to explore

Management of spinal muscular atrophy: nursing clinical commentary and best practices update.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder that has seen a paradigm shift in its management due to early diagnostic strategies, disease-modifying therapies, and integrated care models. Despite these advances, the nursing role - essential across the care continuum - remains underrepresented in clinical protocols and guidelines. MAIN BODY: This Clinical commentary, authored by Italian nurses with direct expertise in SMA care, outlines the key responsibilities, practices, and procedural frameworks that define specialized nursing contributions. Nurses are involved

Tobacco Treatment Competencies: A Brief Review of Advanced Practice Psychiatric Nursing Education.

OBJECTIVES: Tobacco use increases morbidity and mortality among its users. The rate of tobacco use is declining in the general adult population yet is two to three times higher for those experiencing mental health disorders, specifically those with serious mental illness (SMI). Psychiatric mental health nurse practitioners (PMHNPs) are well-positioned to treat tobacco use in patients with SMI, yet tobacco training during nursing education, both in amount of time spent and content delivered, is not well understood and can leave PMHNPs ill-prepared to intervene. METHODS: A cross-sectional descr

Self-awareness in nurse leadership and its relationship with emotional intelligence and reflective practice.

Self-awareness is widely recognised as a core component of effective nurse leadership, influencing decision-making, communication and patient care. It also underpins emotional intelligence, an essential leadership skill that enables individuals to perceive, understand and manage their own emotions, and respond appropriately to the emotions of others. Self-awareness can be developed through reflection, enabling nurses to recognise how their values, emotions and behaviours affect others, particularly in complex and emotionally demanding healthcare environments. However, self-awareness and reflec

[Confronting Climate Change: The Professional Roles and Practical Challenges of Oncology Nurse Navigators].

In this paper, the impacts of climate change on cancer-related health risks and oncology care systems are examined, with a particular focus on the evolving professional role of oncology nurse navigators within climate-adaptive care. Although not a direct cause of disease, climate change reshapes the distribution of cancer risks and increases the vulnerability of treatment safety and continuity through multiple environmental pathways. Beyond the acute disruptions triggered by extreme weather events, climate change produces chronic and recurrent instability in cancer care, resulting in treatment

Understanding Filicide Risk: A Scoping Review to Inform Mental Health Nursing Practice.

Filicide is usually defined as the killing of a child by a parent(s), stepparent, or equivalent guardian and is a deeply distressing form of family violence that intersects public health, mental health, legal, and social systems. Filicide is often associated with parental mental health disorders, family dysfunction, and complex psychosocial stressors, yet healthcare responses, particularly within mental health nursing, remain underexplored. This scoping review aims to identify the characteristics of individuals who commit filicide and examine the tools, indicators, or clinical practices used w

Training and Practice Gaps in Nursing Home Palliative Care: A Cross-Sectional Study.

BACKGROUND: Palliative care needs are prevalent among nursing home (NH) residents. However, access to and integration of palliative care services remain limited. NHs often rely on a workforce with varying levels of training and exposure to palliative care, which may influence care quality and consistency. Understanding the perspectives of NH personnel across disciplines about palliative care is essential to developing sustainable models of care. METHODS: We conducted a cross-sectional survey of clinical employees (N = 398) at seven NHs within a single integrated health system in New York Stat

Nurse Practitioner Practice Regulations and Psychotropic Medication Dispensing for Medicaid-Insured Adults.

This study assessed the impact of Maryland's Nurse Practitioner Full Practice Authority Act of 2015 on psychotropic medication dispensing for Medicaid-insured adults treated by nurse practitioners (NPs) and physicians. A piecewise regression analysis of 168,509 Medicaid enrollees found that after this policy implementation, individuals were more likely to receive newly dispensed psychotropic medications from psychiatric-mental health NPs and from other NPs without a psychiatric specialty and were less likely to receive these medications from psychiatrists and from other medical doctors. The Practices of Public Hospital Registered Nurses Regarding Post-Stroke Dysphagia, in China.

BACKGROUND: Dysphagia is one of the most common complications in post-stroke patients, post-stroke medical disease management (eg, thrombectomy procedures) do have a huge influence on the long-term outcomes, with frontline clinical nurses serving as pivotal gatekeepers for early identification and management in post-stroke dysphagia (PSD). However, nurses, especially those in low- and middle-income countries, generally lack the ability to manage PSD, and the current state of Chinese public hospital registered nurses' relevant clinical practice remains unclear. PURPOSE: This study aims to eval

Tiny Infants, Positional Head Deformity, Developmental Positioning and Neonatal Nursing Practice.

BACKGROUND: Low-birth weight infants are at risk for neurodevelopmental complications. Prolonged mechanical ventilation and endotracheal tube stability can compromise developmentally supportive positioning and result in positional head deformity (PHD). Nationally, PHD prevalence is 22%-66%, yet an internal audit of 93 neonatal intensive care unit (NICU) discharge summaries found normal head assessments. PURPOSE: To evaluate PHD prevalence among NICU preterm infants and neurodevelopmental positioning practice. METHODS: A convenience sample of 50 NICU preterm infants >72 hours of life discharg

Nursing home geriatric rehabilitation care and interprofessional collaboration; a practice-based study.

BACKGROUND: Frailty and multimorbidity are common among patients in geriatric rehabilitation care (GRC). Proper care of these patients involves multiple professionals which requires optimal interprofessional collaboration to provide the best possible support. Interprofessional collaboration (IPC) goes beyond multi-professional collaboration. It requires professionals to think beyond the expertise of their own discipline and work on joint outcomes in which the patient is actively involved. This study describes the development of the GRC teams of an elderly care organization towards the IPC. ME

Nurse practitioner scope of practice and the prevention of foot complications in rural diabetes patients.

PURPOSE: Diabetic foot ulcers afflict a quarter of type-2 diabetes mellitus patients and are associated with higher mortality rates among people with diabetes. Routine primary and preventive care is essential to both prevent and treat foot ulcers before they can contribute to further adverse outcomes. One approach for expanding this care to people with diabetes in rural communities is increasing the practice authority of nurse practitioners. This study examines whether the presence of nurse practitioner practice authority is associated with fewer foot ulcer complications in rural populations a
