The impact of a pharmacist-driven, collaborative practice on diabetes management in an Urban underserved population: a mixed method assessment.

The objective of this manuscript is to describe the results of a pharmacist-driven, Type 2 diabetes targeted, collaborative practice within an urban, underserved federally qualified health center. Pharmacists within a primary care team managed patients with chronic illnesses utilizing a collaborative practice agreement. Pharmacists, pharmacy residents, and supervised students provided care for patients with Type 2 diabetes. The first visit incorporated past medical history, medication reconciliation, determination of adherence and patient knowledge of diabetes pathophysiology, care plan, inclu

The Harris Center: Expanding Integrated Care with Partnerships Beyond Traditional Clinic Walls.

Integrated behavioral health care has advanced from simple service colocation to a coordinated, system-wide approach that connects behavioral health, primary care, emergency medicine, social services, and community support. Health systems increasingly rely on partnerships to address the complex medical and social needs driving population health outcomes. The Harris Center for Mental Health and IDD (The Harris Center) illustrates this evolution, operating as a comprehensive population health partner that serves nearly 90,000 individuals annually across Harris County. Through deep integration wi

Understanding the Barriers to Effective Bone Health Management in People With Multiple Sclerosis.

BACKGROUND: The prevalence of osteoporosis is higher in people with multiple sclerosis (MS) compared with the general population. Poor bone health, including higher rates of osteopenia and osteoporosis than in the general population, has been established in people with MS. This qualitative study examines how much a group of health care professionals (HCPs) knows about osteoporosis as an MS comorbidity, as well as current practices, barriers to effective bone health management in people with MS, and recommendations for improvement. METHODS: Thirty-two Australian HCPs participated in qualitativ

Large increase in opportunistic testing for chlamydia during a pilot project in a primary health organisation.

INTRODUCTION: The Auckland chlamydia pilot project was one of three funded by the Ministry of Health to trial implementation of the 2008 Chlamydia Management Guidelines. Chlamydia is the most commonly notified sexually transmitted infection in New Zealand. AIM: To increase opportunistic testing in under-25-year-olds and to improve documentation of partner notification in primary care. METHODS: A four-month pilot was initiated in Total Healthcare Otara using a nurse-led approach. Laboratory testing data was analysed to assess whether the pilot had any impact on chlamydia testing volumes in th

Heterogeneity of Ego Depletion and Its Relationship With Health Promotion Behavior Among Older Adults With Type 2 Diabetes Mellitus: Latent Profile Analysis.

PURPOSE: To identify latent profiles of ego depletion among older adults with type 2 diabetes mellitus (T2DM), and analyze how these profiles relate to health promotion behaviors. METHOD: A cross-sectional study surveyed 220 older adults with T2DM on ego depletion and health promotion behaviors. Latent profile analysis identified distinct profiles of ego depletion, and multinomial logistic regression explored their links to health promotion behaviors. RESULTS: Latent profile analysis revealed three distinct profiles: high cognitive depletion, high behavioral depletion, and high emotional dep

Public policy for meningococcal vaccination.

On an individual basis, meningococcal disease is consistently shown to be one of the most feared potential childhood infections. On a population level, any clustering of cases or increase in disease requires proactive health protection management, while epidemics can be devastating. It is therefore no surprise that developing protective meningococcal vaccines and effective strategies for their implementation has been a continuing public health priority for some decades.

Carer-led health interventions to monitor, promote and improve the health of adults with intellectual disabilities in the community: a systematic review.

Using carers to help assess, monitor, or promote health in people with intellectual disabilities (ID) may be one way of improving health outcomes in a population that experiences significant health inequalities. This paper provides a review of carer-led health interventions in various populations and healthcare settings, in order to investigate potential roles for carers in ID health care. We used rapid review methodology, using the Scopus database, citation tracking and input from ID healthcare professionals to identify relevant research. 24 studies were included in the final review. Fetal complications related to minor maternal trauma.

The active lifestyle of pregnant women, in combination with the increased incidence of violence in society, place women at greater risk for accidental injury during pregnancy. This identification of increased risk has altered the health care management of mother and fetus after injury. The health care provider treating this patient population must perform thorough maternal-fetal assessments and be suspicious of fetal compromise, even in the face of maternal stability.

The Taiwan Precision Medicine Initiative provides a cohort for large-scale studies.

Han Chinese people comprise nearly 20% of the global population but remain under-represented in genetic studies1,2, so there is an urgent need for large-scale cohorts to advance precision medicine. Here we present the Taiwan Precision Medicine Initiative (TPMI), established by Academia Sinica in collaboration with 16 major medical centres around Taiwan, which has recruited 565,390 participants who consent to provide DNA samples for genetic profiling and grant access to their electronic medical records (EMRs) for research. EMR access is both retrospective and prospective, allowing longitudinal

Diabetic foot disease management in the Gulf Cooperation Council countries: a scoping review protocol.

The Gulf Cooperation Council (GCC) countries face a substantial impact from the increasing prevalence of diabetes mellitus, which experts identify as a major public health challenge in the region. Despite the escalating burden of diabetes mellitus and its related complications, including diabetic foot disease (DFD), there are noteworthy knowledge gaps concerning the prevalence and trends of DFD in the GCC countries. Furthermore, there is insufficient understanding of the management of DFD within health care settings in this region. The objective of this scoping review is to comprehensively ass

Management of thyroid dysfunction and thyroid nodules in the ageing patient.

Thyroid dysfunction is a common endocrine disorder in the general population, with a reported prevalence of 10-15%. However, this rate is even higher in older adults, with an estimated prevalence of ≈25% in some populations. Since elderly patients usually present more comorbidities than younger individuals, thyroid dysfunction may carry a synergistic negative health impact, mainly due to increased cardiovascular disease risk. Moreover, thyroid dysfunction in the elderly can be more difficult to diagnose due to its subtle or even asymptomatic clinical presentation, and the interpretation of thy

Separate and Sick: Residential Segregation and the Health of Children and Youth in Metropolitan Statistical Areas.

The purpose of this study was to better understand residential segregation and child/youth health by examining the relationship between a measure of Black-White residential segregation, the index of dissimilarity, and a suite of child and youth health measures in 235 U.S. metropolitan statistical areas (MSAs). MSAs are urban areas with a population of 50,000 or more and adjacent communities that share a high degree of economic and social integration. MSAs are defined by the Office of Management and Budget. Health-related measures included child mortality (CDC WONDER), teen births (NCHS natalit

Nurse and client perceptions of home health wound care effectiveness after a change in Medicare reimbursement.

The purpose of this study was to begin to explore client and nurse perceptions related to how a change in a Medicare reimbursement, Prospective Payment System (PPS), affects home health care. The target population included home health clients with stage III or greater decubitus ulcers, age 65 years or older. Six Virginia home health agencies were randomly selected. From those sites, all 39 charts that met population criteria were accessed and analyzed, and then 16 care recipients were purposely selected. In addition, 26 home health nurses with pre- and post-PPS experience were purposefully sel

Patient self-management in primary care patients with mild COPD - protocol of a randomised controlled trial of telephone health coaching.

BACKGROUND: The prevalence of diagnosed chronic obstructive pulmonary disease (COPD) in the UK is 1.8%, although it is estimated that this represents less than half of the total disease in the population as much remains undiagnosed. Case finding initiatives in primary care will identify people with mild disease and symptoms. The majority of self-management trials have identified patients from secondary care clinics or following a hospital admission for exacerbation of their condition. This trial will recruit a primary care population with mild symptoms of COPD and use telephone health coaching

A cost consequence analysis of outreach strategies for high risk pregnant women.

This retrospective, cost consequence analysis examined three distinct outreach strategies used by a community-based public health agency to enroll uninsured, low-income pregnant women in perinatal health care services. The three strategies included outreach workers, liaison workers, and case managers. Data analysis revealed that costs per enrollee by strategy were: outreach workers: $429; liaison workers: $98; and case managers: $187. Cost per strategy must be considered cautiously, however, because outreach and enrollment strategies at this agency are not mutually exclusive. Data analysis als
