MDCT evaluation of distal radius fractures and their association with carpal and distal ulnar fractures.

PURPOSE: The purpose of this study was to evaluate the characteristics of distal radius fractures (DRFs) in patients undergoing multi-detector computed tomography (MDCT) and their association with carpal and distal ulnar fractures. METHODS: This retrospective study analyzed 120 patients, who underwent MDCT for evaluation of DRFs. Two radiologists independently evaluated the data for various fracture characteristics and for associated carpal and distal ulnar fractures. RESULTS: Out of 120 DRFs, 74 were complete articular, 40 were partial articular and only 6 were extra-articular. Displacement…

Development of decision-aid of goal-setting for patients with distal radius fracture: Aid for decision-making in occupation choice for distal radius fracture.

BACKGROUND: Patients with distal radius fractures (DRFs) encounter significant difficulties and challenges in their daily lives due to their medical condition and a lack of strategies for modifying activities. Occupation-based interventions have emerged as promising strategies to improve occupational performance and participation outcomes, addressing these challenges. PURPOSE: This study aims to develop the aid for decision-making in occupation choice for distal radius fracture (ADOC-DRF), a novel decision-aid tool designed to facilitate patient-centered and occupation-based goal-setting by o…

Bone mineral density in the distal radius in a healthy Japanese population and in relation to fractures of the distal radius.

Changes in bone mineral density with age were measured in the distal radius of healthy adults using dual energy X-ray absorptiometry. A total of 2789 healthy women (20-95 years old) and 1255 healthy men (20-87 years old), and 72 women (52-94 years old) and 23 men (51-79 years old) with fractures of the distal radius were assessed. Bone mineral density remains relatively stable in men despite aging, and was significantly higher than in women in every decade. In women aged 70 years and more, the bone mineral density was significantly lower in the fracture group than the non-fracture group. In me

Forecasting fractures: Weather-associated variability in hip fracture and isolated distal radius fracture incidence.

INTRODUCTION: Low-energy hip fractures (HFs) and distal radius fractures (DRFs) are common orthopedic injuries. Although winter increases same-level, low-energy fall rates, the role of specific weather variables, and whether these effects differ by age or fracture type, remains unclear. OBJECTIVE: To evaluate weekly association between weather conditions and HF and DRF incidence over eight-years using age-stratified statistical models. METHODS: Retrospective review of 1882 HF and 444 DRF patients presenting to a single Level 1 trauma center from January 2017-March 2025. Daily local weather d…

Low-Cost, Reusable Fracture Reduction Task Trainer for Distal Radius Fractures.

AUDIENCE: This task trainer is designed to instruct emergency medicine (EM) resident physicians, senior medical students, and advanced practice providers. INTRODUCTION: While orthopedic emergencies are common, exposure to fracture management may vary depending on one's training environment. Given that extremity bony trauma and fracture reduction techniques are listed as core content topics in the American Board of Emergency Medicine's (ABEM) 2022 Model of the Clinical Practice of Emergency Medicine,1 it is beneficial to practice their management in a simulated environment. Fracture reduction

Analysis of fracture healing process by HR-pQCT in patients with distal radius fracture.

INTRODUCTION: High-resolution peripheral quantitative computed tomography (HR-pQCT) has enabled us to observe the changes in bone microarchi … y higher than the external callus area. CONCLUSION: In the healing process of postoperative distal radius fractures, increased BMD at the inner surface of the fracture site was confirmed in all fractures. Bone formation on the endosteal side may be a necessary condition for bone union of distal radius fractures.

Non-dislocation lunate fracture combined with distal radius fracture: A case report.

RATIONALE: Lunate fractures are very rare, accounting for only 0.5% to 6.5% of all carpal fractures. They are mostly caused by high-energy trauma and commonly occur with fractures and dislocations of other carpal bones. It is rarely combined with Colles' fracture and the underlying pathogenesis and prognostic significance still remain controversial. PATIENT CONCERNS: Here we report a case of a fresh lunate fracture combined with Colles' fracture, without dislocations of other carpal bones caused by a low-energy fall injury. DIAGNOSES: Lunate fracture, Colles' fracture. INTERVENTIONS: This p

Concomitant Ulnar Styloid Fracture and Distal Radius Fracture Portend Poorer Outcome.

The literature on the effect of ulnar styloid fractures (USFs) on concomitant distal radius fractures (DRFs) is mixed. We conducted a study to determine if associated ipsilateral USFs affect outcomes of DRFs. We retrospectively evaluated 315 DRFs treated (184 operatively, 131 nonoperatively) over a 7-year period. Concomitant USFs were identified. Mean follow-up was 12 months. Disabilities of the Arm, Shoulder, and Hand (DASH) and 36-Item Short Form Health Survey (SF-36) outcome scores, and grip strength and wrist range of motion data, were collected. Statistical analysis was performed with Stu…

Distal Radioulnar Joint Instability in Distal Radius Fracture.

Distal radioulnar joint (DRUJ) instability following distal radius fracture (DRF) is an increasingly recognised entity, particularly with recent advances in the management of DRF. This review aims to provide a comprehensive appraisal of DRUJ anatomy, methods of assessing instability and current treatment strategies. Both non-operative and operative modalities are considered. In the acute setting, surgical management primarily involves open reduction and internal fixation (ORIF) of the distal radius, which can restore DRUJ stability through correction of translation and distraction of the dista

A proposed paradigm shift in the management of distal radius fractures.

BACKGROUND AND OBJECTIVE: Distal radius fractures represent a remarkable orthopaedic entity. Most distal radius fractures can be treated conservatively with closed reduction and immobilisation with satisfactory results, while open reduction and internal fixation is reserved for displaced fractures. Our objective was to propose a paradigm shift in the management of distal radius fractures. METHODS: A literature search of management of distal radius fractures was conducted. PubMed and Cochrane databases were used for the search. English articles with open access or institutional subscription av…

Management of ipsilateral trifocal fracture of the radius-a case report.

INTRODUCTION: Diaphyseal forearm fractures are commonly seen in clinical practice. However, segmental forearm fractures are relatively rare. We hereby present a case of surgical management of trifocal fracture of radius (mid-shaft of radius, distal end of radius and neck of radius fracture) with ipsilateral shaft ulna fracture. CASE REPORT: A middle aged patient with ulnar shaft fracture and trifocal radius fracture was managed surgically with open reduction and internal fixation of all four fractures. The ulna was addressed first, to achieve alignment, followed by distal radius and shaft rad

Hand laterality recognition in distal radius and/or ulna fracture.

INTRODUCTION: Hand laterality, an important ability to determine the orientation of a limb is common to get affected after short term immobilization. Distal radius and/or ulna fracture is a commonly encountered fracture resulting from upper-limb trauma. Conservative treatment using closed reduction and plaster cast application to immobilize the joint remains choice of treatment over surgery in the treatment of these fractures. There is a paucity of literature reporting impairment in hand laterality after long term immobilization as commonly performed in patients with distal radius and/or ulna

The demographics and outcomes in patients with bilateral distal radius fractures.

Although distal radius fractures are quite common, bilateral distal radius fractures seldomly occur. Due to this, treatment is primarily based on surgeon experience with unilateral fractures, however bi- lateral fractures add a level of complexity : loss of functional independence. The purpose of this study was to examine a cohort of patients with bilateral distal radius fractures to identify differences in demographics, mechanism of injury, and outcomes to further our understanding of these rare injuries. 23 patients were identified retrospectively over a 5-year period that met inclusion crit…

Arthroscopy in Distal Radius Fractures: Indications and When to Do It.

Wrist arthroscopy in the setting of distal radius fractures allows for direct visualization of the articular surface and treatment of associated soft tissue injuries. Arthroscopic techniques may be used to fine tune reductions with residual articular step-off or gap after an initial reduction attempt and are critical for addressing free articular fragments and die-punch lesions. Surgeon training, experience, and comfort with wrist arthroscopy remains a barrier to widespread adoption of this technique. Level I studies are needed to investigate whether improved articular reduction leads to meani…

[Die-Punch Fracture of the Scaphoid Fossa of the Distal Radius].

Even though the die-punch fragment of the distal radius fracture was described as early as in 1962, its detection may be crucial to select the optimal therapy even nowadays. Typically, the die-punch fragment is located in the lunate fossa. It can, however, also be located in the radial column. The patient with the die-punch fracture of the scaphoid fossa, treated by us with open reduction and plate osteosynthesis from two surgical approaches, is presented in our case report. Key words: die-punch fracture, distal radius, surgical treatment.

Dorsal Plating for Specific Fracture Pattern of the Distal Radius.

Background: Volar locking plates have provided the capability to repair both simple and complex fractures. However, complications related to the inability to fix or to maintain the fixation of some fracture patterns have been reported with volar locking plates. The purpose of this study was to evaluate the results of dorsal plating treatment for specific pattern of fractures. Methods: Patients with distal radius fractures were retrospectively evaluated. Inclusion criteria for this study were those related to the patient and treatment (adult patients, internal fixation with dorsal plating, a mi…
