Neurotransmitter modulation of sleep-wake States: From molecular mechanisms to therapeutic potential.

Sleep is one of the most fundamental physiological activities in humans and animals, and a normal sleep cycle is crucial for maintaining overall health. However, sleep disorders are increasingly becoming a major mental health issue affecting individuals and society, as well as a contributing factor to the onset of other diseases. Consequently, the development of novel therapeutic strategies for sleep disorders has emerged as a significant scientific challenge garnering widespread attention. Based on current research findings, focusing on neurotransmitters remains a promising approach for devel

Autonomic Dysfunction in Sleep Disorders: From Neurobiological Basis to Potential Therapeutic Approaches.

Sleep disorder has been portrayed as merely a common dissatisfaction with sleep quality and quantity. However, sleep disorder is actually a medical condition characterized by inconsistent sleep patterns that interfere with emotional dynamics, cognitive functioning, and even physical performance. This is consistent with sleep abnormalities being more common in patients with autonomic dysfunction than in the general population. The autonomic nervous system coordinates various visceral functions ranging from respiration to neuroendocrine secretion in order to maintain homeostasis of the body. Bec

Sleep Abnormalities in MultipleSystem Atrophy.

PURPOSE OF REVIEW: The purpose of this review was to explore the different sleep disorders associated with MSA, their mechanisms, and their treatments. RECENT FINDINGS: Stridor is a red flag for the diagnosis of MSA. Recent findings show that its presence in early stage of the disease is associated with a reduction in life expectancy. Its management should be fast and adapted. Its treatment with continuous positive airway pressure or tracheostomy is efficacious. Sleep disorders in MSA are frequent and severe combining insomnia, daytime sleepiness, restless legs syndrome (RLS), REM sleep behav

Further observations on the use of 5-hydroxytryptophan in a child with Lesch-Nyhan syndrome.

The effects of 5-hydroxytryptophan (5-HTP), a serotonin precursor, and carbidopa, a peripheral aromatic amino acid decarboxylase inhibitor, were studied in a boy with the Lesch-Nyhan syndrome. No effects on the self-mutilating behaviors of this disorder were noted. However, sleep patterns were normalized, with a reduction in the proportionate amounts of wakefulness and an augmentation of active sleep. A sharp reduction in waking behavioral dyskinesias was also correlated to increased drug dosage.

Orexin antagonists for neuropsychiatric disease: progress and potential pitfalls.

The tight regulation of sleep/wake states is critical for mental and physiological wellbeing. For example, dysregulation of sleep/wake systems predisposes individuals to metabolic disorders such as obesity and psychiatric problems, including depression. Contributing to this understanding, the last decade has seen significant advances in our appreciation of the complex interactions between brain systems that control the transition between sleep and wake states. Pivotal to our increased understanding of this pathway was the description of a group of neurons in the lateral hypothalamus (LH) that

Sex Differences in Insomnia and Circadian Rhythm Disorders: A Systematic Review.

Insomnia and circadian rhythm disorders are increasingly common in modern society and lead to significant challenges for people's health and well-being. Some studies suggests that men and women differ in neurohormonal secretion, biological processes, and brain morphology. Thus, such differences may affect the etiology, manifestation, and course of sleep disorders, including insomnia and circadian rhythm. This systematic review aims to synthesize the existing literature on sex differences in insomnia and circadian rhythm disorders. PubMed, MEDLINE, Epistemonikos, and Cochrane databases were sea

Genetic and neural mechanisms of sleep disorders in children with autism spectrum disorder: a review.

BACKGROUND: The incidence of sleep disorders in children with autism spectrum disorder (ASD) is very high. Sleep disorders can exacerbate the development of ASD and impose a heavy burden on families and society. The pathological mechanism of sleep disorders in autism is complex, but gene mutations and neural abnormalities may be involved. METHODS: In this review, we examined literature addressing the genetic and neural mechanisms of sleep disorders in children with ASD. The databases PubMed and Scopus were searched for eligible studies published between 2013 and 2023. RESULTS: Prolonged awak

Noradrenergic dysregulation in the locus coeruleus: Implications for neuropsychiatric disease pathophysiology- A systematic review.

The locus coeruleus [LC] is a dense cluster of noradrenergic neurons located in the dorsal pons of the brainstem and serves as the brain's principal source of noradrenaline (NA). This neuromodulatory system plays a central role in regulating arousal, attention, stress response, sleep-wake cycles, and emotional behavior. The LC's extensive efferent projections render it highly sensitive to both external environmental stressors and intrinsic factors, including age-related neuronal loss, which can compromise cognitive function, emotional regulation, and psychiatric stability. Emerging evidence im

Future of Sleep Medicine: Novel Approaches and Measures Derived from Physiologic Systems Dynamics (Part I).

We review recent progress in understanding fundamental aspects of physiologic regulation during wake and sleep based on modern data-driven, analytical, and computational approaches with a focus on the complex dynamics of individual physiologic systems. The presented empirical findings indicate that sleep-wake and circadian cycles do not simply modulate basic physiologic functions but influence physiologic systems dynamics simultaneously over a broad range of time scales. The reviewed empirical approaches and derived measures represent novel mechanistic aspects of sleep and wake regulation, and

[Brain damage in sleep-disordered breathing: the role of glia (clinical aspects)].

Sleep-disordered breathing (SDB) is one of the most frequent sleep-wake disorders. SDB is associated with brain damage that manifests as structural (atrophy of amygdala, hippocampus, insula) and functional (cognitive and emotional dysfunction) disorders. In this review, we summarize the results of clinical and experimental studies investigating the approach for the decreasing brain damage in SDB via glial modulation. In conclusion, we suggest the strategies for future research aimed at optimizing diagnostics and treatment of brain damage in SDB.

Pelvic movements as rhythmic motor manifestation associated with restless legs syndrome.

Video-polysomnographic monitoring of a patient with a 4-year history of an unpleasant restless sensation originating in his lower abdomen showed stereotyped, repetitive, rhythmic pelvic body movements resembling coital behaviour at the wake-sleep transition. We discuss the association between restless legs syndrome and rhythmic movement disorder as a particular manifestation of a spectrum of rhythmic sleep-related movement disorders.

Sleep disturbances and excessive daytime sleepiness in Parkinson disease: an overview.

Sleep disturbances are frequent in Parkinson disease. These disorders can be broadly categorized into those that involve nocturnal sleep and excessive daytime sleepiness. The disorders that are often observed during the night in PD include sleep fragmentation that may be due to recurrent PD symptoms, sleep apnea, Restless Leg Syndrome/ periodic limb movements and REM sleep behavior disorder. Excessive daytime sleepiness is also a common occurrence in PD. EDS can arise from several etiologies, and patients may have more than one etiology responsible. The causes of EDS include nocturnal sleep di

Trace Amine-Associated Receptor 1 Agonists as Narcolepsy Therapeutics.

BACKGROUND: Narcolepsy, a disorder of rapid eye movement (REM) sleep, is characterized by excessive daytime sleepiness and cataplexy, a loss of muscle tone triggered by emotional stimulation. Current narcolepsy pharmacotherapeutics include controlled substances with abuse potential or drugs with undesirable side effects. As partial agonists at trace amine-associated receptor 1 (TAAR1) promote wakefulness in mice and rats, we evaluated whether TAAR1 agonism had beneficial effects in two mouse models of narcolepsy. METHODS: In the first experiment, male homozygous B6-Taar1tm1(NLSLacZ)Blt (Taar1

Sleep Regularity Index After Stroke: Change Over Time and Its Association With Recovery: A Longitudinal Observational Study.

BACKGROUND: Sleep-wake disruption is common after stroke and may impede recovery. The Sleep Regularity Index (SRI) quantifies the probability that an individual is in the same sleep or wake state at the same clock time on consecutive days. This study evaluates SRI during poststroke recovery, estimating associations with stroke outcomes and characterizing transitions between SRI states over time. METHODS: We analyzed data from 71 participants who were part of a larger ongoing study exploring the impact of nonobstructive sleep apnea sleep disorders post stroke. Data were collected at 10, 60, an

Current Understanding of Narcolepsy 1 and its Comorbidities: What Clinicians Need to Know.

Narcolepsy is a chronic neurologic disorder associated with the dysregulation of the sleep-wake cycle that often leads to a decreased quality of life and results in a considerable health burden. There is often a delay to diagnosis of narcolepsy, mainly due to the lack of recognition of this disorder. One of the main factors hindering the diagnosis of narcolepsy is the association of comorbidities, which include other sleep disorders, psychiatric disorders, cardiovascular disorders, and metabolic disorders. The signs and symptoms of these comorbidities often overlap with those of narcolepsy, an
