BACKGROUND: Image-guided percutaneous transthoracic lung biopsy has become a widely used and less invasive diagnostic method. Pneumothorax is the most frequent complication after lung biopsy. The aim of the present study is to describe the experience with expectant management of asymptomatic small post-biopsy pneumothorax in order to reduce unnecessary hospital admissions.

Incidental appendectomy is the surgical removal of a macroscopically non-diseased appendix during another primary surgical procedure. The benefits and risks of this surgery give rise to controversy. Sweet syndrome is a rare inflammatory disorder characterized by fever, leukocytosis, and painful erythematous-violaceous plaques associated with a neutrophilic dermal infiltrate. This condition has never been described in the setting of aesthetic surgery. We report the case of a 60-year-old woman who developed Sweet syndrome following a cervicofacial rhytidectomy combined with blepharoplasty and lower eyelid lipofilling. The favorable outcome under systemic corticosteroid therapy highlights the importance of early diagnosis in order to avoid unnecessary surgical procedures.

Dermoscopy is already considered a fairly established method for diagnosing scabies. This is because dermoscopy enables the visualization both of the burrow and the mite itself, forming the so-called "jet with a contrail" structure. In the present report we present an extraordinary case of a patient with scabies lesions on the face and neck, which was misdiagnosed during sequential visits and underwent unnecessary surgical diagnostic procedures. Finally, the diagnostic problem was solved when dermoscopy was applied.

Acute epiploic appendagitis is a relatively rare cause of lower abdominal pain that clinically mimics other acute abdomen conditions that require surgery such as acute diverticulitis or appendicitis. Here, we report a case of a 50-year-old lady who presented with an unusual lower abdominal pain. Awareness of such a clinical condition with its characteristic imaging findings is important to avoid costly hospitalization, unnecessary antibiotic courses, and the morbidity and mortality associated with surgical procedures.

Postoperative pseudogout after total knee arthroplasty is rare. If pseudogout attacks are misdiagnosed as periprosthetic sepsis, patients may undergo unnecessary surgical procedures. We report a case of pseudogout in the early postoperative period. The attack ensued shortly after a nonsteroidal antiinflammatory drug was discontinued. Idiopathic palmoplantar eccrine hidradenitis is characterised by painful erythematous plantar and/or palmar nodules and is triggered by exposure to wet and cold conditions. The course is benign and self-limiting with a tendency for recurrence. We describe two cases in otherwise healthy children after physical activity in a swimming bath. They were hospitalised for one and three days, respectively. Diagnostic considerations are briefly reviewed. Sufficient attention to the characteristic clinical appearance could prevent unnecessary diagnostic procedures and hospitalisation.

Breast disease in the adolescent female encompasses an expansive array of topics. Benign disease overwhelmingly dominates the differential diagnosis and dictates a different protocol for care in the adolescent compared with the adult patient to avoid inappropriately high assessments of risk and unnecessary diagnostic procedures and surgery. There also are emerging issues pertaining to the care of the adolescent breast, such as breast augmentation, nipple piercing, and management of the adolescent patient with a family history of breast cancer.

In 3.25% of 400 consecutive double-contrast upper gastrointestinal examinations, a prominent fold in the prepyloric gastric antrum that extends through the pylorus into the base of the duodenal bulb has been identified. Although the clinical significance of this fold is undetermined, endoscopic evaluation suggests it is a manifestation of chronic gastritis. This fold may simulate other entities, but can be distinguished by its characteristic features. Proper recognition may prevent unnecessary procedures such as endoscopy and biopsy.

Disturbances of the nail apparatus are common and mainly benign. This review aims to investigate the aetiology of these disturbances, which range from more common benign causes to less common melanomas. Melanonychia may be the most prominent concern and is characterised by brown or black nail plate discoloration. Hence, understanding the most common nail changes, their epidemiology, pathophysiology, and clinical features are imperative to diagnosis and may prevent unnecessary surgical procedures in cases where it is not warranted.

Tuberculosis of the female reproductive tract affects usually reproductive age group and is a common cause of infertility with tubal pathology. Diagnosis of the female genital tract tuberculosis is challenging and, very often missed due to the unusual presentations and lack of specific diagnostic tests. This may occasionally lead to unnecessary surgical procedures. We present two case reports of female genital tuberculosis (FGTB) who presented to us with symptoms mimicking malignancy diagnosed TB only on histopalthology.

Optic neuritis is clinically suspected when painful, rapidly progressive central visual loss occurs in a young patient who has a relative afferent pupillary defect. Ophthalmoscopy is normal in the majority of cases but papillitis or optic disc palor can occur. Several other diseases can mimic optic neuritis, such as ophthalmological conditions or other diseases affecting the visual pathways. Their appropriate clinical diagnosis is of paramount importance in order to avoid unnecessary or invasive procedures or treatments.
