PURPOSE: Warthin-like mucoepidermoid carcinoma (WL-MEC) is a low-grade variant of mucoepidermoid carcinoma (MEC), characterized by a multicystic growth pattern and dense lymphoid stroma, histologically mimicking Warthin tumor (WT). Few cases have been reported, limiting understanding of this variant. We aimed to provide a comprehensive clinical, histopathologic, and molecular analysis of new WL-MEC cases.
METHODS AND RESULTS: Fourteen WL-MEC cases from Brazil, Guatemala, and Mexico were analyzed. Clinicopathologic features (patient demographics, tumor location, AFIP grading, follow-up, recurr

INTRODUCTION: Xerostomia following radioiodine therapy (RIT) in patients suffering from differentiated thyroid cancer is a common side effect in 2 % to 67 % of patients treated with radioiodine (I-131). In order to evaluate the impact of sialendoscopy on health related quality of life (HRQOL) in patients suffering from therapy induced sialadenitis and xerostomia, we analyzed findings from two dedicated questionnaires (Xerostomy Questionnaire XQ and Xerostomy Inventory XI) in patients before and three months after sialendoscopy.
PROCEDURES: In total, 12 patients suffering from differentiated t

The purpose of this study was to evaluate the two functions: segment weight optimization (SWO) and segment edit (SE) in the latest XiO 4.7 radiation treatment planning system and their effect on the planning of intensity modulated radiation therapy (IMRT) for Nasopharyngeal Carcinoma (NPC). SWO first appeared in XiO 4.5 and SE in XiO 4.7. Twelve patients with NPC were selected and there were three plans for each patient: the common step-and-shoot IMRT plan (C-IMRT); S-IMRT was based on the result of C-IMRT and the plan was further optimized with SWO; F-IMRT was based on S-IMRT and the segments

BACKGROUND: Salivary dysfunction is the most common side effect associated with (131)I therapy in patients with differentiated thyroid cancer. The purpose of this study was to evaluate the effect of parotid gland (PG) massage on radioisotope accumulation in the salivary gland.
METHODS: Sixty patients were included in this study. Using Tc-99m pertechnetate, two salivary scans were performed in all patients. In 30 patients, PG massage was performed between the two salivary gland scans, whereas in the other 30 patients no massage was performed between the two scans. Total counts of both PGs and

PURPOSE: To assess anatomical, clinical and dosimetric pre-treatment parameters, possibly predictors of parotid shrinkage during radiotherapy of head and neck cancer (HNC).
MATERIALS: Data of 174 parotids from four institutions were analysed; patients were treated with IMRT, with radical and adjuvant intent. Parotid shrinkage was evaluated by the volumetric difference (DeltaV) between parotid volumes at the end and those at the start of the therapy, as assessed by CT images (MVCT for 40 patients, KVCT for 47 patients). Correlation between DeltaVcc/% and a number of dosimetric, clinical and ge

The sentinel lymph node (SLN) is the first node in the lymphatic basin to be affected by metastatic tumor cells from a primary lesion. It provides a way to avoid elective neck dissection in a case in which there is no metastasis to the SLN. However, identification and excision of the SLN may be difficult due to the shine-through phenomenon, as the SLN in the parotid gland is either located close to the primary lesion or, in many cases, the lymph node of the parotid gland is small and covered by fibrous capsula. When we conducted an SLN biopsy on a 68-year-old male patient with malignant melano

Epithelioid sarcoma is a rare soft-tissue tumor that usually occurs in young adults, with a median age of 26 years. This malignancy has been divided into distal and proximal types. The latter is found in proximal body sites including the head and neck region. We present a rare case of parotid proximal-type epithelioid sarcoma in a 1-year-old male child; this is the 4th reported case in the literature and the youngest in a pediatric patient to date. The tumor showed prominent rhabdoid features by light microscopy. Immunohistochemical studies revealed positive staining to cytokeratin (AE1/AE3),

Salivary gland tumors are uncommon and most of them are of epithelial origin. Mesenchymal tumors affecting the parotid are extremely rare, and we present a series of 19 cases. All parotid tumors (600 cases) treated at the Department of Head and Neck Surgery from A.C. Camargo Hospital, Brazil from 1953 to 2003 were reviewed and 19 cases of nonlymphoid mesenchymal origin were selected. The histological characteristics were reviewed and clinical features were obtained from the medical charts. 15 out of 19 were benign tumors, including 5 lymphangiomas, 5 neurofibromas, and one case each of schwann

OBJECTIVE/HYPOTHESIS: Fine needle aspiration (FNA) is an accurate, cost-effective tool for the study of salivary gland lesions. Although complications are extremely rare, salivary gland FNA can lead to hemorrhage, facial nerve injury, and cellulitis at the needle puncture site. Some studies suggest that FNA can cause infarction or metaplastic transformation of benign Warthin's tumors. We review our recent experience with FNA of the parotid gland, focusing on possible complications and pitfalls.
STUDY DESIGN AND SETTING: The medical records of all patients who underwent FNA of the parotid glan

OBJECTIVE: Pleomorphic adenoma (PA) is the most common benign parotid tumour. Magnetic resonance imaging (MRI) and fine-needle aspiration cytology (FNAC) are routinely used during preoperative work up. However, there is insufficient evidence to support the use of MRI alone to diagnose a PA. This paper aims to evaluate the diagnostic performance of MRI and FNAC in the diagnosis of PA and develop an MRI assessment tool to potentially reduce an invasive biopsy.
METHODS: A retrospective cohort study was conducted in 155 patients with histopathologically confirmed parotid tumours with preoperative

Nuclear protein in testis (NUT) carcinoma (NC) is a rare aggressive tumor with a typical NUTM1 gene rearrangement. Herein, we aimed to investigate the morphological and genetic features of head and neck NC. Immunohistochemistry staining for NUT (C52B1) was performed for 118 samples of head and neck poorly differentiated/undifferentiated carcinoma. Diffuse NUT staining was further confirmed via fluorescence in situ hybridization and next-generation sequencing. Two parotid gland NC cases, one in a 22-year-old man and one in a 52-year-old woman, were confirmed (2/118, 1.6%). Typical morphological

BACKGROUND & PURPOSE: Intensity-modulated proton therapy (IMPT) of superficial lesions requires pre-absorbing range shifter (RS) to deliver the more shallow spots. RS air gap minimization is important to avoid spot size degradation, but remains challenging in complex geometries such as in head-and-neck cancer (HNC). In this study, clinical endpoints were investigated for patient-specific bolus and for conventional RS solutions, making use of a Monte Carlo (MC) dose engine for IMPT optimization.
METHODS AND MATERIALS: For 5 oropharyngeal cancer patients, IMPT spot maps were generated using bea

IMPORTANCE: The clinical implications of extracapsular dissection over superficial parotidectomy are controversial and limited in data on cost-effectiveness.
OBJECTIVE: To compare extracapsular dissection with superficial parotidectomy for benign parotid tumors with respect to surgical outcomes and cost-effectiveness.
DESIGN, SETTING, AND PARTICIPANTS: This was a retrospective medical record review and cost-effectiveness analysis performed from August 2012 to November 2015 at a tertiary care institution. Adult patients (age ≥18 years) who underwent parotidectomy for benign parotid lesions we

The Jacobian of the deformation field of the registration between images taken during Radiotherapy is a measure of compression/expansion of the voxels within an organ. The Jacobian mean value was applied to investigate possible correlations between parotid deformation and anatomical, clinical and dosimetric parameters. Data of 84 patients were analyzed. Parotid deformation was evaluated through Jacobian maps of images taken at the start and at the end of the treatment. Several clinical, geometrical and dosimetric factors were considered. Correlation between Jacobian mean value and these parame
