Recent preclinical studies have demonstrated that convection-enhanced delivery (CED) can be used to perfuse the brain and brainstem with therapeutic agents while simultaneously tracking their distribution using coinfusion of a surrogate magnetic resonance (MR) imaging tracer. The authors describe a technique for the successful clinical application of this drug delivery and monitoring paradigm to the brainstem. Two patients with progressive intrinsic brainstem lesions (one with Type 2 Gaucher disease and one with a diffuse pontine glioma) were treated with CED of putative therapeutic agents mix

OBJECT: The author conducted a study to assess the long-term results obtained in patients who underwent GKS for large vestibular schwannomas (> 3 cm in diameter). Facial and cochlear nerve functions were evaluated.
METHODS: Twenty consecutive large tumors in 18 patients (including two cases of neurofibromatosus Type 2 [NF2]) were followed for more than 6 years. There were eight tumors that were more than 4 cm in maximum diameter. Microsurgery had already been performed prior to GKS in 11 patients (nine recurrent and two residual tumors). Four patients (including one with NF2) died during the

OBJECTIVE: To evaluate the prevalence as well as the management of primary facial nerve schwannoma (PFNS) presenting as vestibular schwannoma.
METHODS: A retrospective review was carried out on 146 patients referred to and operated upon in our institution over a 5-year period for tumors of the internal auditory canal extending or not into the cerebello-pontine angle (TICA). A review of the literature included an extensive MEDLINE search of the literature since 1966 and careful inspection of the bibliography of each article. Articles pertaining to facial nerve schwannomas were reviewed to iden

The development of systemic sarcoidosis in patients with malignancies is a well-known phenomenon. However, cutaneous sarcoidlike lesions are a rare finding. We report 2 patients with B-cell chronic lymphocytic leukemia and hypogammaglobulinemia who presented isolated cutaneous sarcoidlike lesions 9 years after the diagnosis of the hematologic malignancy in one case and after the second course of chemotherapy regimen in the other case. The role of cytokines released from malignant cells and of the hypogammaglobulinemia facilitating an unidentified infectious agent is questionable.

Radiosurgery is a management approach used to treat patients with vestibular schwannomas. The goals are long-term tumour growth control, maintenance of cranial nerve function and prevention of new deficiencies. We sought to determine long-term outcomes measuring the potential benefits against the neurological risks of primary radiosurgery. Gamma Knife radiosurgery was applied as a treatment modality for 289 patients with vestibular schwannomas from April 1992 to April 2002. The long-term results of 100 patients who underwent radiosurgery were evaluated. 60 patients received a primary treatment

OBJECTIVE: This retrospective study focuses on 2 outcome results after surgical intervention for acoustic neuroma: (1) facial nerve status, and (2) hearing preservation.
STUDY DESIGN: A total of 484 patients with an acoustic neuroma.
RESULTS: Postoperative facial nerve outcomes were significantly different (P < 0.001) according to the size of the tumors. Tumor size had even more influence on the immediate postoperative results. In addition, statistical significance (P < 0.05) was demonstrated in comparing facial nerve outcomes with the surgeon's surgical experience. We also noted that as the

The demographic profile and complications are compared and contrasted for 150 consecutive parotidectomies. All patients were under the care of one surgeon (JDL) over a twenty-year period (1977-1997). The case records and contemporaneous database were analysed retrospectively. 111 (74%) procedures were performed for tumours and 39 (26%) for inflammatory disease. The incidence of unexpected permanent facial nerve palsy was 1.8% in the tumour group and zero in the inflammatory group. The overall unexpected palsy rate was 1.3%. Transient paralysis was more common in the inflammatory group than the

OBJECTIVE: This study aimed to illustrate some recent advances in intraoperative monitoring of the seventh and eighth cranial nerves (i.e., direct recording of cochlear nerve action potentials [CNAPs] and facial nerve action potentials [FNAPs]).
STUDY DESIGN: Reports of representative cases and average postoperative results obtained in subjects submitted to cerebellopontine angle surgical procedures are examined. Results obtained in patients with direct recording techniques are compared with those obtained with the classical monitoring techniques (auditory brain stem response [ABR], electroco

Functional reconstruction of the oral cavity after tumor ablation is challenging. A variety of methods are available for reconstruction including primary closure, skin grafts, local cutaneous flaps, musculocutaneous flaps, and free flaps. Appropriate reconstruction addresses the size, location, and tissue needs of the defect. The melolabial flap allows for reliable and effective intraoral reconstruction with minimal donor morbidity, making it ideal for select defects. From 1989 to 1993 16 patients aged 51 to 81 years underwent reconstruction of intraoral defects with 24 melolabial flaps. Twent

This paper represents a general review of basic age-related changes that take place in the craniomandibular apparatus and the most frequently presenting conditions associated with craniomandibular disorders (CMD) in the elderly. The evaluation of geriatric patients with signs or symptoms of CMD must consider (1) normal age-related changes in the craniomandibular apparatus and their impact on both normal function and responses to stress; (2) the role of dentition status and dental prosthesis in CMD; and (3) the contribution of malignant disease, psoriasis, arthritic conditions, pseudogout, gran

The maxillofacial region, extending from the base of the skull to the hyoid bone, is one of the most anatomically complex regions of the body. This area contains elements and organs belonging to a number of different systems that can be affected by a variety of local and systemic pathologic processes. Diagnostic imaging has assumed a central role in the evaluation of this region. In the following monograph, we present the reader with an overview of contemporary maxillofacial imaging, focusing on three areas: recent technical advances that impact on the diagnosis of the maxillofacial region, th

Renal cell carcinoma (RCC) associated with tuberous sclerosis (TSC) has been reported in adult patients; however, this association is rare in children. We have encountered 2 different types of RCCs in infants who developed TSC later in childhood. The pathologic diagnoses were papillary RCC in one infant and a clear cell-type RCC in the other. Gene analysis showed that both the patients had a mutation in TSC2 gene on chromosome 16.

PURPOSE: The management of odontogenic cysts and tumors typically requires a biopsy, which may present significant challenges and prompt an additional visit to the operating room before definitive treatment. The aim of this study was to determine the validity of frozen-section diagnosis in the management of benign oral and maxillofacial lesions, allowing intraoperative diagnosis followed by definitive treatment under the same general anesthetic.
MATERIALS AND METHODS: A retrospective chart review of patients treated at the University of Michigan Health System was performed. Patients of all ag

Parapharyngeal tumors account for 0.5% of head and neck tumors. They are difficult to diagnose because they have few symptoms and are surgically inaccessible. This retrospective study included 61 patients with parapharyngeal space tumors, treated in the last 20 years. The data, obtained from the medical records, included symptoms and clinical signs, diagnostic procedures, surgical approach, postoperative complications and histopathological findings. The most common symptoms were dysphagia, foreign body sensation, pain, and symptom-free patients. For precise tumor localization and its relation

In patients with large vestibular schwannomas (VS), surgery is the primary treatment despite risks such as facial nerve dysfunction. Intra-operative monitoring with transcranial motor evoked potentials (TcMEP) helps predict facial nerve outcome but may lead to subtotal resections and later recurrence. This study compares TcMEP thresholds, facial nerve outcome, recurrence rate and progression-free survival in Koos grade 4 vestibular schwannomas. Fifty-five surgically treated patients with Koos grade 4 VS (2015-2024) were included in this retrospective study. All underwent facial nerve TcMEP mon
