Fatal accidents in neonatal pterosaurs and selective sampling in the Solnhofen fossil assemblage.

The Upper Jurassic Solnhofen Archipelago of Germany has yielded a pterosaur assemblage that has long underpinned and continues to dominate m … individuals, we document two highly immature specimens of Pterodactylus exhibiting similar oblique humeral fractures. These fractures are consistent with excessive wing loading during flight, providing compelling evidence of super-precocial flight capabilities in immature pterosaurs. By contrast, background "attritional" sampling under normal environmental conditi …

Retrospective analysis of ultrasound-guided hydrodistension in patients with post-traumatic adhesive capsulitis of the shoulder.

BACKGROUND: Adhesive capsulitis (AC) is a painful shoulder condition characterized by limited motion due to fibrosis and contracture of the joint capsule. Secondary AC frequently occurs after upper limb fractures and immobilization, delaying recovery. METHODS: This retrospective study analyzed 11 patients with post-traumatic AC (< 3 months) following non-displaced humeral fractures treated conservatively with a median immobilization period of 25 days. The cohort included seven men and four women (mean age 52.6 years), five of whom also had rotator cuff injuries. Treatments involved hydrodiste…

A Retrospective Study of Postoperative Development of Implant-Induced Osteoporosis in Radial-Ulnar Fractures in Toy Breed Dogs Treated with Plate Fixation.

OBJECTIVE: This study aimed to evaluate implant-induced osteoporosis (IIO) development in toy breed dogs treated using internal fixation with digital radiographs as the index of pixel values. STUDY DESIGN: There were 226 cases (236 limbs) of toy breed dogs with radial-ulnar fractures. Pixel values were measured on radiographs immediately, 2 weeks and 1 to 12 months after surgery. The ratio of pixel values (PVR) represented the bone mineral areal density based on the humeral condyle in the same image. The dogs were grouped based on the fixation methods, age and status of destabilization. RESU

Primary reverse shoulder arthroplasty for acute proximal humerus fractures: A 5-year long term retrospective study of elderly patients.

BACKGROUND: Reverse shoulder arthroplasty (RSA) in complex shoulder fractures is ever more frequently. This study compares clinical and radiologic results of patients with comminuted proximal humeral fractures (PHFs) treated with RSA, with and without tuberosities grafting. METHODS: Between January 2009 and June 2014, 55 patients aged ≥65 years with 3- and 4-part PHFs were treated surgically. Patients' files and the hospital's digital database were reviewed retrospectively with at least 5-year of follow up. We constituted three groups according to the tuberosity consolidation: patients in who…

Adverse events after recombinant human BMP2 in nonspinal orthopaedic procedures.

BACKGROUND: The FDA has approved recombinant human bone morphogenetic protein 2 (rhBMP-2) for treating acute, open tibial shaft fractures. However, the nature and frequency of complications after the use of rhBMP-2 in nonspinal orthopaedic surgery have not been well characterized. QUESTIONS/PURPOSES: To determine what types of adverse events have been reported after the use of rhBMP-2, whether they were severe enough to require additional surgery, and after what types of operations these adverse events occurred. METHODS: Adverse events reported to the FDA's Manufacturer and User Facility Dev

Time-elapsed microstructural imaging of failure of the reverse shoulder implant.

BACKGROUND: Reverse Shoulder Arthroplasties (RSA) have become a primary choice for improving shoulder function and pain. However, the biomechanical failure mechanism of the humeral component is still unclear. The present study reports a novel protocol for microstructural imaging of the entire humerus implant under load before and after fracture. METHODS: A humerus specimen was obtained from a 75-year-old male donor. An expert surgeon implanted the specimen with a commonly used RSA implant (Aequalis reversed II, Stryker Orthopaedics, USA) and surgical procedure. The physiological glenohumeral

The medial periosteal hinge, a key structure in fractures of the proximal humerus: a biomechanical cadaver study of its mechanical properties.

The medial periosteal hinge plays a key role in fractures of the head of the humerus, offering mechanical support during and after reduction and maintaining perfusion of the head by the vessels in the posteromedial periosteum. We have investigated the biomechanical properties of the medial periosteum in fractures of the proximal humerus using a standard model in 20 fresh-frozen cadaver specimens comparable in age, gender and bone mineral density. After creating the fracture, we displaced the humeral head medial or lateral to the shaft with controlled force until complete disruption of the post

Scapholunate Dissociation due to an Atypical Biomechanical Mechanism in Arm Wrestling: A Case Report.

BACKGROUND: Scapholunate ligament injuries are not uncommon in hand surgery, but they are not described in the sport of arm wrestling. CASE PRESENTATION: The patient was a 28-year-old man who experienced wrist pain when he was about to lose an arm wrestling match. Since then, he has had persistent clicking in his right wrist, which was found to be due to scapholunate dissociation. The reason for scapholunate dissociation rather than a humeral shaft fracture in our patient may be a reduction in torque on the humerus due to forced elbow extension, which could be seen in a losing player at the e

Thorough debridement of the humeral head for osteonecrosis.

Osteonecrosis of the shoulder is a devastating disease, particularly in the young patient. Published data is replete with vascularized and nonvascularized bone grafting procedures for joint preservation of the hip; however, these same techniques have not been applied to the shoulder. When treating osteonecrosis of the shoulder, endoscopically-guided thorough debridement of the necrotic bone may promote good outcomes by ensuring that the bone graft, vascularized or nonvascularized, is stabilized against a viable host bed. In addition, this new technique facilitates access to the subchondral pla

Open Reduction and Internal Fixation (ORIF) of Complex 3- and 4-Part Fractures of the Proximal Humerus: Does Age Really Matter?

INTRODUCTION: Treatment of complex fracture patterns of the proximal humerus continues to be a challenging and controversial clinical scenario. The aim of this study was to report on the outcomes of complex displaced 3- and 4-part fractures of the proximal humerus treated with locked plating and compare the functional results of patients on the basis of age at time of injury. METHODS: A retrospective review was completed to identify patients whom had sustained a 3- or 4-part fracture of the proximal humerus (Neer classification), treated surgically with locked compression plating. Patients we

AN ANATOMICAL STUDY OF THE THORACIC LIMB OF THE GIANT ANTEATER (MYRMECOPHAGA TRIDACTYLA) COMPARED WITH THE DOMESTIC DOG TO GUIDE SURGICAL APPROACHES TO THE HUMERUS AND RADIUS.

The giant anteater (Myrmecophaga tridactyla) is a neotropical mammal considered to be vulnerable to extinction. Recent increased interest in veterinary care in the giant anteater has prompted renewed interest in anatomical descriptions in this species. The terrestrial habits and slow movements of the giant anteater contribute to its susceptibility to vehicular trauma on highways, which is a significant cause of mortality in the species. This study describes the muscular anatomy of the thoracic limb and variations of the long bones with emphasis on the structures surrounding the humerus and rad

Variant of the terrible triad of the elbow, a CASE report with a review of the literature.

INTRODUCTION: The terrible triad described by Hotchkiss in 1996 is a complex lesion of the elbow, following a trauma combining forced valgus and external rotation. It is a lesion that puts the elbow at risk of developing complications such as instability, stiffness, or synostosis of the proximal radio-cubital joint. CASE REPORT: We report the case of a patient who suffered a closed trauma to the right elbow following a fall onto the palm of the hand with a valgus lateral rotation mechanism. The lesion assessment showed a B2 fracture of the distal humerus (AO classification) with a line splitt

Dual Infection of an Open Fracture Caused by Mycobacterium setense and Clostridium celerecrescens.

Infections caused by Mycobacterium setense or Clostridium celerecrescens are extremely rare. In this report, for the first time a dual infection with these two pathogens is described. An 18-year-old female suffered multiple injuries, including an open comminuted fracture of the right humeral diaphysis after falling from a fifth-floor balcony in January 2019. Five months after the accident, a fistula appeared in the scar, reaching the bone tissue. M. setense and C. celerecrescens were cultured from sinus swabs and subsequently from perioperative samples. The patient was initially treated with a

Is failure of tuberosity suture repair in hemi-arthroplasty for fracture mechanical?

BACKGROUND: Tuberosity suture repair in hemiarthroplasty for fracture carries risk of malunion giving poor results. Is failure mechanical or biochemical? We investigated the mechanical aspect with repetitive loading in cadaveric repairs. MATERIALS AND METHODS: Tests were carried out in line with U.K. Human Tissue Authority regulations. A 4-part fracture was created in 8 cadaver shoulders by osteotomizing the tuberosities. A standard hemiarthroplasty implant was cemented in at correct height and retroversion, and standardized repairs applied. Initial firmness of repair was confirmed by attempt

Influence of peri-implant bone quality on implant stability.

INTRODUCTION: Insufficient primary stability is still reported for proximal humerus fractures in elderly patients. Fixation stability could be improved by aiming locking screws at bone volumes with better properties. The aims of this study were to investigate the bone regions engaged by the locking screws of a Proximal Humeral Nail (MultiLoc PHN), and to evaluate the influence of peri-screw bone quality on bone-nail construct stability. MATERIALS AND METHODS: Twelve cadaveric humeri were divided into two groups. The distal locking part of the PHN was fixed to the specimens. The nails were rem

Nonunions of the surgical neck of the humerus: surgical treatment with an intramedullary bone peg, internal fixation, and cancellous bone grafting.

Twenty patients with pseudarthrosis of the upper humerus underwent surgery with the intramedullary bone peg technique. A 6 to 10 cm corticocancellous autogenous bone graft (11 iliac crest, 6 anterior tibial crest, 3 middle-third of the fibula) was pegged] into the humerus and bridged the pseudarthrosis. Stability of the fracture site was obtained by plate osteosynthesis; an additional peripheral cancellous graft was performed. Our patient series included 15 women and five men with an average age at operation of 58 years; the dominant side was involved in 12 cases. Eleven had undergone 22 previ
