Design and optimization of auditory prostheses using the finite element method: a narrative review.

BACKGROUND AND OBJECTIVE: An auditory prosthesis refers to a device designed to restore hearing. Some parameters of the auditory prosthesis, such as mass, implanted position, and degree, need to be repeatedly designed and optimized based on the realistic geometry of the ear. Numerous auditory prostheses designs were based on animal or specimen experiments involving many complex instruments, and the experimental specimens had low repeatability. The finite element method (FEM) can overcome these disadvantages and be carried out on the computer with substantial flexibility in modifying the prosth

Role of Neurotrophins in the Generation of Spiral Ganglion Neuron-Like Cells from Embryonic Stem Cells.

Spiral ganglion neurons (SGNs) are crucial for transferring auditory signals from cochlear sensory hair cells to the brainstem. However, SGNs are usually damaged in sensorineural hearing loss. Embryonic stem cells (ESCs) have been used to regenerate SGNs, but it is obscure whether ESC-derived neurons can fully resemble SGN subtype features. This study aimed to understand the effect of neurotrophins on the generation of SGN-like cells from ESCs and their subsequent subtype specification. This study utilized a stepwise neuronal generation approach to direct DsRed ESCs toward neural progenitors a

Effects of Intralabyrinthine Hemorrhage on the Cochlear Elements: A Human Temporal Bone Study.

HYPOTHESIS: To compare histopathologic findings in the cochlea of human temporal bones with versus without intralabyrinthine hemorrhage. BACKGROUND: Hemorrhagic labyrinthitis can cause sensorineural damage, sudden hearing loss, and vertigo. Yet, to our knowledge, no studies have quantitatively described histopathologic effects of intralabyrinthine hemorrhage on the elements of the cochlea. METHODS: We analyzed 46 human temporal bone samples from 23 patients with unilateral intralabyrinthine hemorrhage (23 samples from ears with intralabyrinthine hemorrhage and 23 samples from contralateral e

An Src-protein tyrosine kinase inhibitor to reduce cisplatin ototoxicity while preserving its antitumor effect.

Ototoxicity remains a major dose-limiting side effect of cisplatin. The current studies were carried out to evaluate the effectiveness of a novel Src-protein tyrosine kinase inhibitor in protecting the ear from cisplatin ototoxicity without compromising cisplatin's antitumor effects. The Src inhibitor has been shown to be effective in protecting the ear from noise-induced hearing loss. Three studies were carried out to determine whether this compound has otoprotective activity in rats treated with cisplatin. The first two studies used the Src inhibitor as a cotreatment with single doses of cis

Reduced spiral ganglion neuronal loss by adjunctive neurotrophin-3 in experimental pneumococcal meningitis.

BACKGROUND: Hearing loss is a frequent long-term complication of pneumococcal meningitis (PM). Its main pathological correlate is damage to the organ of Corti and loss of spiral ganglion neurons. The only current treatment option is cochlear implants which require surviving neurons. Here, we investigated the impact of systemically applied neurotrophin-3 (NT-3) on long-term hearing loss and the survival of neurons. METHODS: Eighteen hours after infection with S. pneumoniae, C57BL/6 mice were treated with a combination of ceftriaxone with NT-3 or dexamethasone or placebo. Hearing, cochlear dama

Effects of simulated spectral holes on speech intelligibility and spatial release from masking under binaural and monaural listening.

The possibility that "dead regions" or "spectral holes" can account for some differences in performance between bilateral cochlear implant (CI) users and normal-hearing listeners was explored. Using a 20-band noise-excited vocoder to simulate CI processing, this study examined effects of spectral holes on speech reception thresholds (SRTs) and spatial release from masking (SRM) in difficult listening conditions. Prior to processing, stimuli were convolved through head-related transfer-functions to provide listeners with free-field directional cues. Processed stimuli were presented over headpho

The moral case for sign language education.

Here, a moral case is presented as to why sign languages such as Auslan should be made compulsory in general school curricula. Firstly, there are significant benefits that accrue to individuals from learning sign language. Secondly, sign language education is a matter of justice; the normalisation of sign language education and use would particularly benefit marginalised groups, such as those living with a communication disability. Finally, the integration of sign languages into the curricula would enable the flourishing of Deaf culture and go some way to resolving the tensions that have arise

Radiological diagnosis of the inner ear malformations in children with sensorineural hearing loss.

Malformations in either the inner ear, vestibulocochlear nerve (VIIIth) or auditory cortex of the brain can lead to congenital sensorineural hearing loss (SNHL). In most cases the underlying disorders involve the membranous labyrinth at a microscopic level and therefore radiological examinations are entirely normal. In a significant proportion however (up to 20%), there are abnormalities visualized in the inner ear and/or the VIIIth nerve; the type of abnormality is relevant for the surgical planning of a cochlear implant. Imaging and the accurate radiological identification of the affected in

Auditory neuropathy associated with postnatally acquired cytomegalovirus infection in a very preterm infant.

Neurosensory hearing loss is a well-known complication of antenatally acquired cytomegalovirus (CMV) infection. We here report an infant who developed auditory neuropathy after a postnatally acquired CMV infection. Infection probably occurred through ingestion of infected breast milk. Following a cochlear implant, there is normal language perception and a mildly delayed language expression at age 4. We speculate that the long-term effects of perinatal CMV infections are more dependent on the postconceptional age at which infection occurs than on whether the infection occurs antenatally or post

Endolymphatic sac revision for recurrent intractable Meniere's disease.

Meniere's disease can be observed uniquely in revisions of the conservative surgical procedure for endolymphatic sac enhancement, which preserves the labyrinth, unlike destructive procedures. Genetic anatomic abnormalities in patients lead to malabsorption of endolymph and symptoms of Meniere's that are reversed by endolymphatic sac enhancement, but disabling symptoms eventually may recur in a few cases and require revisional surgery. We observe developing pathophysiologic conditions intraoperatively and have modified our techniques to accommodate the redeveloped pathogenesis we observe and av

Pull-Out Strength of Orthodontic Miniscrews in the Temporal Bone.

BACKGROUND: Minimally invasive cochlear implant surgery by using a microstereotactic frame demands solid connection to the bone. We aimed to determine the stability of commercially available orthodontic miniscrews to evaluate their feasibility for frame's fixation. In addition, which substitute material most closely resembles the mechanical properties of the human temporal bone was evaluated. METHODS: Pull-out tests were carried out with five different types of orthodontic miniscrews in human temporal bone specimens. Furthermore, short fiber filled epoxy (SFFE), solid rigid polyurethane (SRPU

The expression pattern and inhibitory influence of Tenascin-C on the growth of spiral ganglion neurons suggest a regulatory role as boundary formation molecule in the postnatal mouse inner ear.

Sensorineural hearing loss, as a consequence of acoustic trauma, aging, genetic defects or ototoxic drugs, is highly associated with irreversible damage of cochlear hair cells (HCs) and secondary degeneration of spiral ganglion (SG) cells. Cochlear implants (CIs), which bypass the lost HC function by direct electrical stimulation of the remaining auditory neurons, offer an effective therapy option. Several studies imply that components of the extracellular matrix (ECM) have a great impact on the adhesion and growth of spiral ganglion neurons (SGNs) during development. Based on these findings,
