BACKGROUND: Post-prandial esophageal acid exposure is a key determinant of symptom generation in gastroesophageal reflux disease. When esophageal acid exposure is high, high symptom intensity may mask individual variation in the relationship between acid exposure and perception.

Patients with functional dyspepsia, which involves no organic disease and no reflux, do not benefit from lansoprazole, a proton pump inhibitor. Any improvement seen is probably related to a placebo effect. Prescribing proton pump inhibitors for these patients just for their placebo effect is going to further raise health care costs. Antacids and H2 blockers are less expensive alternatives.

The gastric cytoprotective effects of vitamin A, De-Nol and sucralfate were compared with the effectiveness of pirenzepine in healing ulcer in patients with chronic gastric ulcer. The mechanisms of idiopathic pulmonary fibrosis (IPF), a rare, devastating disease with a median survival of 3-5 years, are not fully understood. Gastroesophageal reflux disease (GERD) is a frequent comorbidity encountered in IPF. We describe the case of a young male presenting with hiccups refractory to antacids. Due to high index of suspicion was evaluated with a chest X-ray which revealed pericardial effusion. The effusion was drained, and the hiccups resolved. It is a teaching moment, to consider simple imaging, i.e., chest X-ray in patients with refractory hiccups to rule out more serious underlying causes.

The occurrence of hypocalcemia in a term infant after the first few days of life is known to result from excessive dietary phosphate, hypomagnesemia, neonatal hypoparathyroidism, hypovitaminosis D, and maternal hyperparathyroidism. Less well recognized is the possibility that maternal ingestion of antacids may cause late neonatal hypocalcemia. Our observations suggest that taking large doses of calcium carbonate during pregnancy may be a potential cause of neonatal hypocalcemia.

60 patients with exacerbated gastric and duodenal ulcer were studied. Summary clinical effect was significantly better (by 12.34%, P < 0.05) in patients who were co-administered actovegin as compared to those who received conventional treatment (cholinolytics, antacids, reparative drugs and preparations of bismuth). The first group demonstrated more vigorous amelioration of pain, endoscopic involution and disappearance of disseminated intravascular coagulation signs as well as shorter hospital time.
