Recommendations for Diagnosis and Treatment of Fractures of the Ring of Axis.

In a consensus process with four sessions in 2017, the working group "upper cervical spine" of the German Society for Orthopaedics and Trauma Surgery (DGOU) formulated "Therapeutic Recommendations for the Diagnosis and Treatment of Upper Cervical Fractures", taking their own experience and the current literature into consideration. The following article describes the recommendations for axis ring fractures (traumatic spondylolysis C2). About 19 to 49% of all cervical spine injuries include the axis vertebra. Traumatic spondylolysis of C2 may include potential discoligamentous instability C2/3.

Cervical vertebroplasty for osteolytic metastases as a minimally invasive therapeutic option in oncological surgery: outcome in 14 cases.

OBJECTIVE The treatment of cervical spinal metastases represents a controversial issue regarding the type, extent, and invasiveness of interventions. In the lumbar and thoracic spine, kypho- and vertebroplasties have been established as minimally invasive procedures for patients with metastases to the vertebral bodies and without neurological deficit. These procedures show good results with respect to pain reduction and low complication rates. However, limited data are available for kypho- and vertebroplasties for cervical spinal metastases. In an effort to add to existing data, the authors he

Epidemiology of atlas fractures--a national registry-based cohort study of 1,537 cases.

BACKGROUND CONTEXT: The epidemiology of fractures of the first cervical vertebra-the atlas-has not been well documented. Previous studies concerning atlas fractures focus on treatment and form a weak platform for epidemiologic study. PURPOSE: This study aims to provide reliable epidemiologic data on atlas fractures. STUDY DESIGN: This was a national registry-based cohort study. PATIENT SAMPLE: A total of 1,537 cases of atlas fractures between 1997 and 2011 from the Swedish National Patient Registry (NPR). OUTCOME MEASURES: The outcome measures were annual incidence and mortality. METHODS:

Management challenges of traumatic spondylolisthesis of the Axis with an unusual C2-C3 posterior subluxation in a paediatric patient: case report and literature review.

INTRODUCTION: Paediatric cervical spine injuries are uncommon. Traumatic spondylolisthesis of the axis (TSA) is commonly encountered in the trauma setting. The management of TSA may be surgical or non-surgical. Decision making is quite challenging depending on patient presentation and nature of injury, and even more so in the paediatric age group. OBJECTIVES: To present a case report highlighting the challenges in the management of TSA. METHODS: We present an 8 year old male, who sustained a bilateral C2 pars fracture with associated unusual C2-C3 posterior subluxation. RESULTS: Neuroradiol

Cervical spine trauma. What is new?

To a large extent, what is new in cervical spine trauma relates to evolving indications for the use of various imaging modalities and expanded understanding of conditions that may influence traumatic sequelae. The American College of Radiology Appropriateness Criteria now provide guidelines for decisions concerning imaging in various clinical situations. This article reviews many of the publications of the past 6 to 7 years that may contribute to a radiologist's understanding of cervical spine injuries.

Lateral retrovascular approach to the upper cervical spine.

The authors describe the surgical anatomy of lateral approach to the upper cervical spine. The approach is retrovascular retropharyngeal extraoral allowing for anterolateral exposure of the upper cervical column, and it may be enlarged posteriorly on the homolateral posterior hemiarch of C1 and C2, and/or inferiorly on the lower cervical column. Absolute indications to this approach are anterolateral fusion with transarticular screws C1-C2 and excision of skeletal neoplasms adjacent to the vertebral artery. For other indications such as the treatment of anterior neoplasms and spinal cord decom

Traumatic spondylolisthesis of axis: clinical and imaging experience at a level one trauma center.

PURPOSE: Traumatic spondylolisthesis of the axis (TSA) with bilateral pars interarticularis fracture (a pattern also known as Hangman's fractures) accounts for 4-5% of all cervical fractures. Various classification systems have been described to assist therapeutic decision-making. The goal is to reassess the utility of these classifications for treatment strategy and evaluate additional imaging associations. METHODS: This is an IRB approved, retrospective analysis of patients with imaging diagnosis of TSA from 2016 to 2019. Consensus reads were performed classifying TSA into various Levine an

An assessment of the relationship between cervical vertebrae maturation index and eruption of permanent maxillary canines.

AIM: The purpose of the present study was to establish the relationship between the eruption of permanent maxillary canines and cervical vertebral maturation stages (CVMs) to assess an alternative method for determining time of canine eruption in the late mixed dentition. MATERIALS AND METHODS: One hundred and twenty subjects with erupting permanent maxillary canines were observed both intraorally and radiographically with orthopantamograms. Lateral cephalograms were recorded to divide subjects according to CVM stage into prepeak (CS1 and CS2), peak (CS3 and CS4) and postpeak (CS5 and CS6) gr

[Os odontoideum. Presentation in adult age].

The Os odontoideum is a variable oval or round ossicle with a smooth cortical border, which partially corresponds to the odontoid process, without having continuity with the rest of C2 bone. The multifactorial etiology causes instability and clinically translates into pain and compression data into neural structures. The treatment of choice is surgical and techniques have been developed that focus on preserving the stability of the segment. We present the case of a 23-year-old female patient, who begins to suffer at 8 years of age, refers to moderate to severe cervicalgia, which develops with

[Indications and technic of transoral dens resection in craniocervical dysplasia].

Compressive lesions of the brain stem caused by ventrally or dorsally located bony malformations of the posterior skull base and the upper cervical spine show diffuse neurological signs and symptoms. They can imitate multiple sclerosis, cerebellar tumours, Ménière's disease and "Ménière-like" otological symptoms. Therefore, the otologist is involved in diagnosis and therapy of cranio-cervical dysplasia. At present dysplasia is differentiated into an anterior and posterior form of the disease. The most common lesion in the anterior form is brain stem compression by the high located dens axis. T

Traumatic spondylolisthesis of the axis: Hangman's fracture.

Traumatic spondylolisthesis of the axis is classified either as Type 1, an undisplaced fracture involving the posterior elements only, or Type 2, a displaced fracture involving the posterior elements plus the anterior structures. This classification is the basis of a rational treatment program. Neurologically, not one in a series of 42 patients was made worse by the proposed program of treatment. All fractures healed per primam without operative intervention. There was no evidence of late instability. The incidence of spinal cord and nerve root involvement was low although the incidence of ass
