INTRODUCTION: The standard treatment for clavicle fractures non operative but surgical management is essential in certain specific indications. The purpose of this study is to report a not very commonly seen bilateral fracture of a clavicle in a young female patient treated with intramedullary fixation.
CASE REPORT: A 23 year old female patient sustained a bilateral clavicle fracture as a result of direct blow to shoulder and was treated with intramedullary K wires on both sides in single sitting. Postoperatively arm was maintained in a sling for 3 weeks. The implant was removed after a month…

INTRODUCTION: Clavicle fractures can cause pain and functional impairment if not managed appropriately. This article evaluates the prevalence of clavicular fractures, estimates the number of cases requiring operative treatment, evaluates whether removal of implant is a frequent necessity and compares the final functional outcome of the operative and non-operative groups.
PATIENTS AND METHODS: Between November 2005 and November 2007, patients with clavicular fractures were eligible for participation. Patients below 18 years of age and those with pathological fractures were excluded. Demographi…

BACKGROUND: In some short malunion cases, midshaft clavicular fractures are reported to result in unsatisfactory clinical outcomes. Shortening deformity of the clavicle could change the anatomical alignment of the shoulder girdle and is surmised to affect shoulder kinematics on arm movements. Nevertheless, no report has ever referred to documented changes.
HYPOTHESIS: Scapular motion will change with clavicular shortening in cadaveric models.
STUDY DESIGN: Controlled laboratory study.
METHODS: Twelve cadaveric shoulders were used, and sequential clavicular shortening by 0%, 5%, 10%, 15%, an…

Various modalities are available for treatment of a clavicle fracture. Conservative treatment is successful in most cases. Open reduction, when indicated, usually is accompanied by fixation of the fracture. In the case of a jockey with a fractured clavicle, the fracture was complicated by tenting of the skin. The fracture was opened and a bony spike was excised extraperiosteally without doing internal fixation of the fracture to avoid skin necrosis and allow early return to work. The patient returned to his profession 2 weeks after surgery. Radiologic and clinical examinations revealed a satis…

A group of 14 patients underwent 17 consecutive open surgical excisions of the distal clavicle for atraumatic osteolysis after having failed conservative treatment. All patients were men involved in weight training or heavy-lifting occupations. The diagnosis was based on no history of shoulder trauma, positive plain radiographs, and positive joint scintigraphy. All 14 patients were available for followup at an average of 25 months postoperatively. The UCLA Shoulder Rating Scale was used for evaluation. All patients reported full return to sports and employment at a satisfactory level. All resu…

Thirty-six patients with Allman group-2 fractures of the clavicle were treated by ORIF with 2.7-mm ASIF dynamic compression plates. The indications for surgery were an open fracture in one patient, ipsilateral fractures of the arm or the ribs in five patients, bilateral clavicular fractures in one patient, and an inability to reduce the fracture in all other patients. There were no instances of deep infection. One patient suffered a refracture after plate removal; three patients developed pseudarthrosis because plates that were too short were used. The total failure rate was 12%. It is conclud…

Sprengel's deformity is the most common congenital anomaly of the shoulder girdle and is often accompanied by the presence of an omovertebral bone. Preoperative oblique roentgenograms or computed tomography may be necessary to define the abnormal structures in Sprengel's deformity. Sprengel's deformity in a six-year-old girl was corrected as follows. During clavicular exposure to osteotomy, an abnormal osseous bridge extending from the midportion of the clavicle to the spine of the scapula was identified and resected. A literature survey failed to identify other similar anomalies.

Five children with atlanto-axial rotatory fixation (AARF) in association with fractures of the clavicle are described. It is postulated that the rotary fixation is a direct result of the trauma which produces the fracture. The importance of early diagnosis is stressed, since delayed diagnosis may lead to chronic deformity. Early diagnosis depends on awareness of the possibility of AARF, and either fluoroscoping the patient in order to take appropriate spot films or imaging the atlanto-axial joint by CT. A simple classification of AARF is proposed based on distinct radiological features which d…

Condensing osteitis of the clavicle, better defined as aseptic enlarging osteosclerosis of the clavicle, is a rare and benign idiopathic lesion. It is probably of degenerative or mechanical origin, and is most commonly seen in middle-aged women as a tender swelling over the medial one-third of the clavicle. Although the clinical features may be confusing and nonspecific, the typical radiographic and histopathological findings will mostly lead to a correct diagnosis of this disorder. The differential diagnosis is quite extensive. Most difficult to differentiate are: avascular necrosis of the me…

Congenital multiple osteomas of the skin associated with disordered ossification of the clavicle occurred in a four-year-old boy. There were no clinical and laboratory findings of pseudohypoparathyroidism, pseudopseudohypoparathyroidism, or fibrodysplasia ossificans progressiva in this case. The clavicular bony mass was excised in segments. Samples of the cutaneous osteomas were removed for histologic study. On microscopic examination, the specimen from the clavicle consisted of trabecular bone in hyaline cartilage, resembling enchondral ossification, but the specimen from the osteoma cutis re…

A patient with ossification of the ligaments of the entire vertebral column has shoulder difficulties due to ossification of the right costoclavicular ligament. The speculation is that the sclerotic changes in the clavicle were caused by the same inflammatory process that has involved the ligaments of the vertebral column and of the sacroiliac joints. In some cases reported in the literature, although not in ours, vascular structures in the costoclavicular space were involved, resulting in venous obstruction or phlebitis. The patient's therapy currently consists of rehabilitation exercises. No…

Bilateral clavicle fractures are rare, with an overall incidence of 0.43%. Various treatment methods exist, but no consensus has been reached. We report a case of a 51-year-old male patient with bilateral midshaft clavicle fractures treated with minimally invasive Osteosynthesis. After 14 months of follow-up, bone healing and functional recovery were excellent. In conclusion, minimally invasive fixation is a viable alternative treatment for this fracture type, complementing established methods. The selection of a suitable minimally invasive fixation device should be individualized for each fra…

Finite element analysis has become indispensable for biomechanical research on clavicle fractures. This review summarized evidence regarding configurations and applications of finite element analysis in clavicle fracture fixation. Seventeen articles involving 22 clavicles were synthesized from CINAHL, Embase, IEEE Xplore, PubMed, Scopus, and Web of Science databases. Most studies investigated midshaft transverse closed fractures by reconstructing intact models from CT scans and simulating fractures through gap creation. Common loading schemes included axial compression, distal torsion, and inf…

BACKGROUND: Nonunion is a significant complication following nonoperative treatment of displaced midshaft clavicle fractures, potentially leading to impaired shoulder function, pain, and decreased quality of life. This study aims to identify predictors of nonunion in adults treated nonoperatively to optimize treatment decisions and improve outcomes.
METHODS: A retrospective cohort study was conducted using data from 374 patients treated nonoperatively between 2012 and 2024. Patient and fracture characteristics, including age, sex, smoking, diabetes mellitus, and fracture comminution, were ass…

BACKGROUND: Regional anaesthesia can be an attractive alternative anaesthetic approach in clavicle surgery, but it requires the peripheral block of multiple cervical and brachial nerves that transmit nociceptive information. Deep cervical plexus blocks, as paravertebral nerve block, can lead to severe side effects, such as unilateral diaphragmatic paralysis.
CASE REPORT: A 66-year-old male patient, American Society of Anesthesiologists physical status III, was scheduled for open reduction and internal fixation of the right clavicle with plates and screws after a high-energy trauma. Pre-anaest…

BACKGROUND: Neer type V distal clavicle fractures are considered the most unstable fracture type and are characterized by the disruption of continuity between the coracoclavicular (C-C) ligaments and proximal or distal bone fragments. However, owing to the rarity of such fractures, there is currently no universally accepted surgical procedure for their treatment. Recently, the scorpion plate, an anatomical, non-locking, pre-contoured plate with two grasping arms to fix the distal or inferior clavicular fragments, was introduced. This study aimed to investigate the postoperative functional and …
