Association between continuous glucose monitoring metrics and cardiovascular autonomic neuropathy in diabetic patients: a systematic review.

Cardiovascular autonomic neuropathy (CAN) is a serious and prevalent complication of diabetes, linked to significant morbidity and mortality. Continuous glucose monitoring systems (CGM) provide a comprehensive and continuous glucose profile, enabling precise assessment of glycemic variability, hypoglycemia, and other key glucose metrics. Despite the increasing use of CGM, the relationship between CGM-derived metrics and CAN risk has yet to be rigorously evaluated. A systematic search of PubMed, Cochrane Library, Web of Science, Medline, and Embase was conducted up to 30 September 2024. Eligibl…

Continuous Glucose Monitoring in Older Adults: What We Know and What We Have Yet to Learn.

OBJECTIVE: To assess the growing use of continuous glucose monitoring (CGM) systems by older adults and explore additional areas integration that could benefit adults with frailty. BACKGROUND: The use of CGM devices has expanded rapidly in the last decade. This has been supported by substantial data showing significant benefit in glycemic metrics: hemoglobin A1c improvements, less hypoglycemia, and improved quality of life. However, sub-populations, such as older persons, exist where available data are limited. Furthermore, frail older adults represent a heterogeneous population with their ow…

Correlation Between Hemoglobin Glycation Index Measured by Continuous Glucose Monitoring With Complications in Type 1 Diabetes.

OBJECTIVE: HbA1C is the "gold standard" parameter to evaluate glycemic control in diabetes; however, its correlation with mean glucose is not always perfect. The objective of this study was to correlate continuous glucose monitoring (CGM)-derived hemoglobin glycation index (HGI) with microvascular complications. METHODS: We conducted a cross-sectional study including permanent users of CGM with type 1 diabetes mellitus or latent autoimmune diabetes of the adult. HGI was estimated, and presence of microvascular complications was compared in subgroups with high or low HGI. A logistic regression…

Current status of continuous glucose monitoring among Korean children and adolescents with type 1 diabetes mellitus.

Type 1 diabetes mellitus (T1DM) requires life-long insulin therapy because of diminished insulin-secretion capability. Glycemic control and glucose monitoring are important to prevent T1DM complications. Continuous glucose monitoring (CGM) measures glucose level, every one to five minutes, in the interstitial fluid from a subcutaneous sensor and facilitates better glycemic control, reduces hypoglycemia, and is safely used in the pediatric population. CGM can be categorized as retrospective, real-time, or intermittently scanned CGM, and all forms are available in Korea. The CGM device has 3 com…

Use of microdialysis-based continuous glucose monitoring to drive real-time semi-closed-loop insulin infusion.

Continuous glucose monitoring (CGM) and automated insulin delivery may make diabetes management substantially easier, if the quality of the resulting therapy remains adequate. In this study, a semi-closed-loop control algorithm was used to drive insulin therapy and its quality was compared to that of subject-directed therapy. Twelve subjects stayed at the study site for approximately 70 hours and were provided with the investigational Automated Pancreas System Test Stand (APS-TS), which was used to calculate insulin dosage recommendations automatically. These recommendations were based on micr…

Associations of Hemoglobin Glycation Index with Glucose Variability and Time in Ranges in Patients with Type 2 Diabetes Undergoing Continuous Glucose Monitoring.

BACKGROUND: Discordance between glycated hemoglobin (HbA1c) levels and blood glucose is commonly observed in clinical practice. The hemoglobin glycation index (HGI) has been proposed as a measure to quantify individual variations in glycation. Our study aimed to investigate the association between HGI and glucose variability assessed by continuous glucose monitoring (CGM) in patients with type 2 diabetes (T2D) treated with metformin monotherapy. METHODS: Adults with T2D treated with metformin monotherapy (daily dose ≥ 1500 mg) who had HbA1c levels ≥ 7.0% (n = 100; mean age 54.0 ± 8.2 years; 5…
