Investigating the interfaces of the epiphyseal plate: An integrated approach of histochemistry, microtomography and SEM.

We investigated the interfaces of the epiphyseal plate with over- and underlying bone segments using an integrated approach of histochemistry, microtomography and scanning electron microscopy (SEM) to overcome the inherent limitations of sections-based techniques. Microtomography was able to provide an unobstructed, frontal view of large expanses of the two bone surfaces facing the growth plate, while SEM observation after removal of the soft matrix granted an equally unhindered access with a higher resolution. The two interfaces appeared widely dissimilar. On the diaphyseal side the hypertrop

Aneurysmal bone cyst-like changes developed in melorheostosis with epiphyseal osteopoikilosis.

Aneurysmal bone cyst (ABC) is a rare and usually painful condition, representing about 1% of all bone tumors. A geographical lytic, expansile, and septated radiological pattern, with fluid-fluid levels on MRI, is classically displayed. ABC can be a primary bone lesion (70% of patients) or can arise in an underlying condition and is subsequently named "ABC-like changes" (30%). ABC-like changes are more frequently encountered in skeletal segments affected by chondroblastoma, fibrous dysplasia, giant cell tumor, osteoblastoma, non-ossifying fibroma, and osteosarcoma. In this article, we describe

Predisposing factors for migration of epiphyseal screws into physis in tension band plating.

The migration of epiphyseal screws into growing physis in tension band plating is a known complication. We investigated the screw migration into physis in 10 patients (18 plates) to study the various technical details, which may have contributed to this complication. The methodology involved retrospective review of radiological records. Among these 10 affected patients, in four patients, there were eight additional tension band plates, which had remained uncomplicated ('controls') at the time when implant failure was detected. We statistically compared the length of epiphyseal screw, proximity

Predicting epiphyseal screw length in anterior plating of distal radial fractures.

The purpose of this study was to determine whether optimal epiphyseal screw length could be predicted with reference to a given diaphyseal screw length when fixating a plate to the anterior surface of the distal radius. Computerized tomography scans of 40 wrists of 28 men and 12 women were semi-automatically segmented. A virtual anterior plate model was fixed to the distal radius. The mean maximal appropriate length of one diaphyseal screw and of the four distal epiphyseal screws were measured and linear regression analyses were performed. We found that the epiphyseal screw lengths were highly

All-Epiphyseal ACL Reconstruction in Children: Review of Safety and Early Complications.

BACKGROUND: All-epiphyseal anterior cruciate ligament (ACL) reconstruction is a well-described technique for skeletally immature patients. The purpose of this study was to elucidate the early complication rate and identify associated risk factors for rerupture after this procedure in children. METHODS: We retrospectively reviewed patients who underwent all-epiphyseal ACL reconstructions performed at a large, tertiary care children's hospital between January 2007 and April 2013. Relevant postoperative data including the development of leg-length discrepancy, angular deformity, rerupture, infec

Long Term Results of Epiphyseal Transplant in Distal Radius Reconstruction in Children.

BACKGROUND: Replacement of osseous defect, restoration of joint function, and restoration of longitudinal growth are the 3 main reconstructive issues that need to be addressed when the physis and epiphysis are damaged in a skeletally immature individual. Failure in achieving these objectives leads to severe deformity and functional impairment, which significantly compromises the quality of life of young patients. Because of its biological and morphological characteristics, the proximal fibula epiphyseal transfer has proven to be an excellent option in limb salvage surgery in pediatric oncologi

Complications after epiphyseal reconstruction of the anterior cruciate ligament in prepubescent children.

PURPOSE: Reconstruction of the anterior cruciate ligament (ACL) remains a major concern in the prepubescent, skeletally immature patient with wide open growth plates. Different surgical techniques have been proposed. This study reports the results and complications of ACL reconstruction in young children using an all epiphyseal technique. METHODS: Between 2006 and 2010, 12 patients (10-13 years, median 12.1 years) underwent epiphyseal primary ACL reconstruction, with a total of 13 knee procedures. Patients were assessed retrospectively with a median follow-up of 54 months (range 39-80 months)

[Growth behavior after epiphyseal plate injury: importance of "watertight" osteosynthesis].

BACKGROUND: The frequency figures for epiphyseal plate injuries of long bones given in the literature are inexact and they probably occur wi … e localization in the skeleton and the localization in the segment because the growth components of epiphyses are asymmetrically distributed in the segment. Stimulatory growth disorders in the actual growth phase < 10 years of age are the obligatory growth disorders which lead to overgrowth of the section of the skeleton affected. In an age over 10 years th …

Polyostotic fibrous dysplasia with epiphyseal involvement in long bones: a case report.

Fibrous dysplasia (FD) is an uncommon, but well-known benign skeletal disorder. In cases affecting long bones, FD is commonly recognized to locate in the diaphyses or the metaphyses and to spare the epiphyses. In this paper, we present a rare case of polyostotic FD in a 13-year-old girl with unilateral multiple epiphyseal lesions arising in the femur, the tibia, and the fibula with the growth plates.

All-epiphyseal anterior cruciate ligament reconstruction using fluoroscopic imaging.

Anterior cruciate ligament injuries in the pediatric population have been increasing in recent years. While reconstruction can often provide the best chance of restoring stability and preventing degenerative joint disease, the skeletally immature patient with open physes can represent a treatment challenge to the orthopaedic surgeon. Here, we present a technique developed by the senior author that uses two-dimensional fluoroscopy, obviating the need for computed tomography imaging. Intraoperative technical details of this procedure are highlighted. This technique allows the orthopaedic surgeon

Multiple epiphyseal dysplasia in an Old Kingdom Egyptian skeleton: A case report.

The skeletal remains of a young adult male (aged 25-35 years) recovered from an Old Kingdom cemetery at Saqqara, Egypt, displayed multiple skeletal abnormalities in the limbs and vertebral column. These included bilateral shortening and robusticity of the humeri with flared distal metaphyses and exaggerated muscle attachments, early-onset osteoarthritic changes in the glenohumeral joints, thoraco-lumbar scoliosis, deformities of the proximal tibiae characteristic of a genu varum condition, angulation and shortening of the fourth metacarpal and metatarsal typical of brachydactyly type E1, and p

Outcomes of extra-articular, intra-epiphyseal drilling for osteochondritis dissecans of the knee.

BACKGROUND: When conservative management fails to heal femoral condyle osteochondritis dissecans (OCD) lesions in a child, then drilling of the subchondral plate below the lesion to stimulate healing may be beneficial. This study reviews the outcomes of extra-articular, intraepiphyseal drilling of OCD lesions of the knee with intact articular cartilage. METHODS: Over an 8-year period, all children, who failed at least 6 months of nonoperative management, underwent arthroscopic knee surgery and extra-articular, intra-epiphyseal drilling for their symptomatic, nondisplaced femoral condyle OCD l

Immunohistochemical examination of epiphyseal growth plates of Japanese Brown cattle with chondrodysplasia.

A new type of inherited chondrodysplasia is described in Japanese Brown cattle, but the basic defects of the epiphyseal growth plate (EGP) in the limb long bones, and proliferation and differentiation of the chondrocytes in the EGP, are not yet understood. In the present study, the EGPs of the limb long bones in eight cases of chondrodysplasia and four normal (control) cattle were examined histologically and immunohistochemically. In the control cattle, proliferative chondrocytes (PCs) and hypertrophic chondrocytes (HCs) were arranged in columns parallel to the long axis of the bone, and HCs w

Treatment of first metatarsal longitudinal epiphyseal bracket by excision before closure.

The longitudinal metatarsal epiphyseal bracket is a rare ossification abnormality of tubular bones in which an abnormal epiphysis brackets the diaphysis of a phalanx, metacarpal, or metatarsal. This leads to progressive shortening and angular deformity of the involved bone. When it affects the first metatarsal, the longitudinal metatarsal epiphyseal bracket leads to a short, trapezoidal metatarsal. We present the case of a 6-year-old patient with an ossified longitudinal epiphyseal bracket along the medial side of the first metatarsal. The abnormality was surgically treated by bracket excision

Epiphyseal dysplasia and other skeletal anomalies in a patient with the 6p25 microdeletion syndrome.

The 6p25 microdeletion syndrome comprises the Axenfeld-Rieger eye anomaly in association with a characteristic facies, developmental delay, hearing loss, and organ malformations. Skeletal anomalies in the form of hemivertebrae, clubfeet, and other positional joint anomalies have also been described in some patients. We report on a patient with a 2.2-2.4 Mb terminal microdeletion of the short arm of chromosome 6 who in addition had abnormalities of the proximal femoral and humeral epiphyses. We suggest that an epiphyseal dysplasia may be an additional clinical component of the 6p25 microdeletio…

The effect of the epiphyseal growth plate on the length of the first metacarpal in triphalangeal thumb.

PURPOSE: Triphalangeal thumb is characterized as a congenital difference of the thumb with an extra phalanx. Additional thumb length is attr … g a longer first metacarpal in this condition. First metacarpals in triphalangeal thumb with double epiphyses grew disproportionately more than those of a normal population, whereas those with distal epiphyses grew disproportionately less than normal. First metacarpals with proximal epiphyses grew at the same rate as normal first metacarpals. CONCLUSIONS: …

Outcome of physeal and epiphyseal injuries of the distal tibia with intra-articular involvement.

The authors reviewed 83 physeal and epiphyseal injuries of the distal tibia with intra-articular involvement. The children, aged 11 to 14 years, were treated in the authors' department during 1987 to 1999. Treatment was nonoperative for 72.25% (60/83) and surgical for 27.75% (23/83) according to specific indications. This gives the basis for a classification of these injuries. The main purpose of the study was to investigate the long-term results of these injuries according to a radiologic classification. The parameters considered were the patient's age, the mechanism of injury, and the possib

Osteochondrosis and epiphyseal bone abnormalities associated with copper deficiency in bison calves.

Two bison calves were submitted to the Western College of Veterinary Medicine to confirm suspected copper deficiency. In addition to clinical signs, there were pathologic changes in the cartilage and subchondral bone of several joints. Water analysis indicated high levels of sulfate in the drinking water, contributing to a secondary copper deficiency.

Dysplasia epiphysealis hemimelica of the proximal tibia showing epiphyseal osteochondroma in an adult.

A 33-year-old woman was referred to our hospital complaining of pain and a tumorous lesion in her left knee joint in the absence of any history of trauma. Radiological examinations demonstrated an osseous mass originating from the epiphysis of the proximal tibia, with a continuous osteoblastic lesion involving the lateral half of the epiphysis. The pathological diagnosis of these lesions was compatible with that of osteochondroma. The clinical and pathological features of this case were considered to be identical with those of dysplasia epiphysealis hemimelica, although this patient was older
