BACKGROUND: Fecal incontinence (FI) is a major part of illness and physical discomfort in the general population. Since the 1990s, sacral nerve stimulation has been well established in the treatment of neurogenic FI. The precise placement of the electrode is crucial for the success of the procedure. Eighty percent of the patients benefit from permanent treatment, but in 10-20% of the patients tested electrode placement proves impossible due to anatomical variations of the sacral foramina. In this study, we describe the technical requirements and a new method of electrode placement with referen…

BACKGROUND: Fecal incontinence and constipation are disabling conditions that reduce quality of life. If conservative treatment fails, one option is sacral nerve stimulation (SNS), a minimally invasive technique allowing modulation of the nerves and muscles of the pelvic floor and hindgut.
OBJECTIVES: To assess the effects of SNS for fecal incontinence and constipation in adults.
SEARCH STRATEGY: We searched the Cochrane Incontinence Group Specialized Trials Register (searched 24 April 2007) and the reference lists of relevant articles.
SELECTION CRITERIA: All randomized or quasi-randomized…

OBJECTIVES: At our institution, the use of cecostomy tubes has provided a successful method for managing severe constipation in patients with spina bifida, with good patient and caretaker satisfaction and minimal morbidity. We have developed a modified technique to allow placement of the cecostomy tube under direct vision during laparoscopic appendicovesicostomy. We present our initial experience and technique.
METHODS: Patients with a normal bladder capacity and compliance who were scheduled for creation of an appendicovesicostomy and who also had refractory constipation were offered concurr

PURPOSE: Fecal incontinence may occur in patients who have undergone anterior resection for rectal cancer without presenting sphincter lesions. Chemoradiation may contribute to disrupting continence mechanisms. Treatment is controversial. Assessment of fecal incontinence in patients who agreed to integrate treatment for rectal cancer and treatment with sacral neuromodulation are reported.
METHODS: Fecal incontinence following preoperative chemoradiation and anterior resection for rectal cancer was evaluated in four patients. A good response was observed during the percutaneous sacral nerve ev…

BACKGROUND/PURPOSE: Functional constipation with associated fecal incontinence responds poorly to medical management once megarectum has developed. The authors describe resecting the dilated rectum and inserting a cecostomy button for antegrade enemas in this difficult condition.
METHODS: Four children, ages 9 to 15 years, with a history of unremitting constipation and fecal incontinence were referred for evaluation after not responding to medical management. All patients had exhibited normal lumbosacral magnetic resonance images (MRI) and open rectal biopsies; however, all 4 patients had a m…

Two children with intractable fecal incontinence after correction of high anorectal malformations were successfully managed by the daily administration of a glycerin enema into the cecum via an appendicocecostomy or tubularized cecostomy, according to the method of Malone's antegrade continence enema (ACE). Fluoroscopic defecography performed during this procedure in each patient disclosed that the glycerin enema promptly evoked cecal peristalsis, which was transmitted to the distal colon and rectum, and squeezed out almost all the fecal matter, evacuating it from the anus. However, two enemas…

A 55-year-old hypertensive male and chronic smoker presented with sudden severe central chest pain accompanied by excruciating left lower-limb pain and episodes of non-bloody diarrhea associated with fecal incontinence. Examination revealed a cold, clammy right upper limb and left lower limb with diminished peripheral pulses but no neurological deficits or abdominal tenderness. CT aortography demonstrated a Stanford Type A aortic dissection extending from the aortic annulus to the iliac arteries. The patient was stabilized and then transferred for emergent open surgical repair. Aortic dissecti…

BACKGROUND: Fecal incontinence (FI) is a common condition associated with aging. Asthma and chronic obstructive pulmonary disease (COPD) are obstructive lung diseases associated with both urinary incontinence and increased intraabdominal pressures.
OBJECTIVE: We sought to evaluate whether these obstructive lung diseases increase the risk of developing FI.
METHODS: We prospectively examined the association between asthma, COPD and asthma/COPD overlap and risk of FI among women from the Nurses' Health Study (NHS). We defined incident FI as ≥ 1 liquid or solid FI episode/month during four years…

BACKGROUND: Obstetric anal sphincter injury is the most frequent cause of fecal incontinence (FI) in young women. However, the relationship between the extent of anal sphincter defects and the severity of long-term FI (at least 1 year after delivery) has been poorly studied. The aim of the present study was to determine if, in the long term, the extent of anal sphincter defects graded at anal endosonography was linked with the severity of FI.
METHODS: A retrospective study was conducted on women with a history of vaginal delivery, who presented with FI and had three-dimensional anorectal high…
