[Treatment of patellar fracture with internal fixation combined closed reduction with arthroscope aid].

OBJECTIVE: To explore the surgical method and clinical short-term outcomes of internal fixation combined closed reduction with arthroscope aid in treating patellar fracture. METHODS: From June 2004 to July 2008, 42 patients with fresh simple patellar fracture underwent internal fixation combined closed reduction with arthroscope aid, included 28 males and 14 females, aged 16-59 years (35 years on average). The locations were 19 left knees and 23 right knees. Injury was by traffic accident in 21 cases, by falling in 11 cases, by sports in 7 cases and by crush in 3 cases. All the patients had f

[Closed reposition of an acute midfoot luxation fracture in Charcot arthropathy with the ring fixator].

Diabetic neuropathic osteo-arthropathy (DNOAP; Charcot arthropathy) is a progressive disease characterized by joint luxation, fractures and excessive destruction of foot architecture. The operative therapy is indicated when conservative therapy fails, in progressive breakdown, in hindfoot Charcot and in spreading infections due to plantar ulcers. The complication rate of 10-20% in open surgery is high. We present a minimally invasive technique of closed reduction and application of a ring fixator for reconstruction of the foot architecture and arthrodesis of the involved joints.

Fractured closed suction catheter: an unusual cause of endobronchial obstruction in a ventilated patient.

BACKGROUND: A 32-year-old HIV-positive man required ventilation for seizures secondary to viral encephalitis. He had a prolonged care unit stay and had percutaneous tracheostomy performed on day 14 of his admission. He subsequently developed persistent right basal infiltrates and atelectasis on chest radiographs that were slow to respond to antibiotic treatment. Fiberoptic bronchoscopy revealed the cause of his infiltrates to be a 14-cm tip section of closed suction catheter tubing that had presumably fractured during suctioning and became lodged in his trachea and right main bronchus. Secondary prevention of fragility fractures: are we following the guidelines? Closing the audit loop.

INTRODUCTION: The aim of this study was to determine whether orthopaedic surgeons follow the British Orthopaedic Association (BOA) guidelines for secondary prevention of fragility fractures. PATIENTS AND METHODS: A retrospective audit was conducted on patients with neck of femur fractures treated in our hospital between October and November 2003. A re-audit was conducted during the period August to October 2004. RESULTS: There were 27 patients in the initial study period. Twenty-six patients (96%)had full blood count measured with LFT and bone-profile measured in 18 patients (66%). Only nine

Closing the osteoporosis management gap in primary care: a secondary prevention of fracture programme.

BACKGROUND: Effective treatments are available to reduce fracture risk in patients with osteoporosis. Prioritisation of assessment and treatment for those patients at highest risk of fracture will and treatment for those patients at highest risk of fracture will will ensure the optimal utilisation of healthcare resources. OBJECTIVES: To confirm prior fracture to be a strong predictor of osteoporosis, evaluate a simple means of identifying patients with osteoporosis, assess the current management gap in this high risk patient group and to enable initiation of treatment where appropriate. Laceration of tibialis anterior tendon complicating a closed tibial fracture: a case report.

Traumatic laceration of the tibialis anterior tendon complicating a closed tibial shaft fracture is a rare injury pattern. Only 3 such cases have been reported to date in the English literature and all were missed on initial examination. A case of a 17-year-old motorcyclist with an acute laceration of the tibialis anterior tendon resulting from a closed oblique tibial shaft fracture is presented. The tendon laceration was suspected preoperatively because of the patient's inability to actively dorsiflex his ankle joint and the existence of a palpable gap in the soft tissues over the anterolater

Closed anatomic reduction of a juvenile tillaux fracture by dorsiflexion of the ankle. A case report.

The juvenile Tillaux fracture is a Salter-Harris Type III intraarticular fracture of the ankle in adolescent children whose distal tibial epiphyses are partially closed. Although there is general agreement that the major force of injury is one of external rotation, when closed reduction is attempted for displaced fractures by reversing the direction of this injuring force, i.e., by internal rotation of the foot on the leg as recommended, there is often failure to achieve satisfactory reduction. Consequently, the fracture is often treated by open reduction. Anatomic reduction was achieved and m

Case Report: Fatal Streptococcus pyogenes infection secondary to closed femoral fracture.

BACKGROUND: Invasive Streptococcus pyogenes infection is exceedingly rare in patients with closed fractures. However, it can precipitate life-threatening necrotizing soft-tissue infection and streptococcal toxic shock syndrome once it occurs. Early recognition and aggressive multidisciplinary management are essential for survival. CASE SUMMARY: We presented a patient who sustained closed fracture of the upper segment of the left femur due to a traffic accident. He had a history of acute pharyngitis 10 days ago. The patient experienced fever on the second day after admission, with his body tem

Closed Flexor Tendon Avulsions Associated With a Distal Radius Fracture: A Case Report.

Flexor tendon injuries have been reported in association with distal radius fractures. These are usually chronic in nature, and acute flexor tendon injuries are rare. When these do occur, they tend to be at the level of the fracture and are thought to be caused by direct laceration from the sharp bone edges of the fracture site. There are no reports of injuries more proximal to the fracture site at the musculotendinous junction. We present a patient who sustained a distal radius fracture with associated complete avulsions of the flexor digitorum superficialis tendons to the middle and ring fin

THE OUTCOME OF PULSELESS PINK HAND FOLLOWING CLOSED SUPRACONDYLAR FRACTURE HUMERUS IN PEDIATRICS.

Management of children with supracondylar humeral fractures with pulseless pink hands is still controversial, whether to choose operative or conservative treatment. Proponents of conventional treatment mentioned that most patients can restore the motor and sensory function of the hand shortly after the injury without the need to restore distal pulse by surgery. Opponents of this treatment strategy claim that many patients will develop limb shortening a few years after the injury leading to functional and psychological problems. In this study, we made a comparison of the outcomes of each treatm

Does a closed reduction improve aesthetical and functional outcome after nasal fracture?

BACKGROUND AND PURPOSES: How closed reduction (CR) to repair a nasal fracture affects the patient's quality of life (QoL) has not been investigated. Here, we assessed QoL before and after CR using disease-specific questionnaires and compared the QoL scores of patients with nasal fractures with normative scores from a reference cohort. METHODS: This was a prospective study of 96 patients with nasal fractures undergoing CR. Patients were interviewed about aesthetic, functional, and QoL issues before and after surgery using the Functional Rhinoplasty Outcome Inventory (FROI-17) and the Rhinoplas

Closed total talar dislocation without fracture in a rare college athlete case.

Total talar dislocation without a fracture is an extremely rare injury. It is often the result of high-energy trauma, such as that incurred after a fall, or owing to motor or vehicular accidents. Talar dislocations have poor outcomes owing to the frequent complications of infection, avascular necrosis and osteoarthritis attributed to open dislocations. We report herein a closed total talar dislocation without a fracture in a college athlete who was injured during sports activities. Specifically, a 20-year-old man was injured during a soccer game this led to a closed total talar dislocation. We

Amlodipine accelerates bone healing in a stable closed femoral fracture model in mice.

Calcium channel blockers (CCBs), which are widely used in the treatment of hypertension, have been shown to influence bone metabolism. However, there is little information on whether CCBs also influence the process of fracture healing. Therefore, the effect of the CCB amlodipine on bone healing was studied in a stable closed fracture model in mice using intramedullary screw fixation. Bone healing was investigated by radiology, biomechanics, histomorphometry and Western blot analysis 2 and 5 weeks after fracture healing. Animals were treated daily (post operatively) per os using a gavage with a

Saphenous Nerve Entrapment Neuropathy After Closed Tibial Fracture: A Case Report.

CASE: A 43-year-old man who underwent intramedullary nailing for a closed tibial fracture developed saphenous nerve entrapment neuropathy. He developed severe medial leg pain, which was worse on walking or standing, 2 years postoperatively. Surgical neurolysis resulted in complete pain relief and functional recovery of the limb without recurrence of symptoms. CONCLUSION: Clinicians should consider several etiologies in the diagnostic evaluation of a patient with chronic pain after limb trauma. If a patient complains of lower extremity pain after intramedullary fixation of closed fractures of

Closed reduction of glenohumeral dislocations with associated tuberosity fracture in the emergency department is safe.

AIM: A fracture of the tuberosity is associated with 16% of glenohumeral dislocations. Extension of the fracture into the humeral neck can occur during closed manipulation, leading some to suggest that all such injures should be managed under general anaesthesia in the operating theatre. The purpose of this study was to establish the safety of reduction of glenohumeral dislocations with tuberosity fractures in the emergency department (ED). PATIENTS AND METHODS: We reviewed 188 consecutive glenohumeral dislocations with associated tuberosity fractures identified from a prospective orthopaedic

Comparision of the expandable nail with locked nail in the treatment of closed diaphyseal fractures of femur.

BACKGROUND: Expandable nails achieve stability only by hydraulic expansion; therefore suggest less radiation exposure and operation time. In this study, we aimed to compare the results of expandable femoral nails with locked intramedullary nails in the treatment of diaphyseal fractures of femur. MATERIALS AND METHODS: Isolated closed AO = Arbeitsgemeinschaft für Osteosynthesefragen type 32.A or 32.B unilateral femoral shaft fractures operated with expandable or locked nail were evaluated retrospectively. We match patients who undergone expandable nail fixation with patients of the same-sex, a
