Isolated mitral valve replacement with the Starr-Edwards prosthesis has been performed on 657 patients at the Mayo Clinic during the 11 year period ending January, 1972. The most recent subset of that series comprised patients who received the Model 6120 prosthesis. In this group, the operative mortality rate was 9 per cent and the actuarial late death rate at 5 years was 20 per cent. This survival rate is a significant improvement over the natural history of severe mitral valve disease. Factors associated with operative deaths are large left artrial size, advanced functional class, and previo

Bioprosthetic mitral structural valve degeneration and failed mitral valve repair (MVr) have traditionally been treated with reoperative mitral valve surgery. Transcatheter mitral valve-in-valve (MVIV) and valve-in-ring (MVIR) replacement are now feasible, but data comparing these approaches are lacking. We sought to compare the outcomes of (1) reoperative mitral valve replacement (redo-MVR) and MVIV for structural valve degeneration, and (2) reoperative mitral valve repair (redo-MVr) or MVR and MVIR for failed MVr. A literature search of PubMed, Embase, and the Cochrane Library was conducted

Background: Aortic stenosis (AS) is one of the most common valvular heart diseases, particularly in the elderly, with a prevalence of approximately 3% in individuals over 75 years of age. Aortic valve replacement (AVR) remains the standard treatment, yet postoperative hemodynamic assessment is often complicated by variations in prosthetic valve size, left ventricular ejection fraction (LVEF), effective orifice area (EOA), and body surface area (BSA). These factors significantly influence prosthetic valve function and contribute to patient-prosthesis mismatch (PPM), which has been associated wi

The formation of thrombotic deposits affects the functionality of mechanical prosthetic heart valves; as a consequence, mechanical valves thrombosis needs early diagnosis to prevent thromboembolic events. This paper compares the acoustic signals produced by two commercial bileaflet mechanical heart valves in the closing phase to detect the presence of thrombi. The closing sounds were recorded in vitro by means of a phonocardiographic device under different hydrodynamic conditions. Thrombotic deposits of different weight and shape were applied onto the valve leaflet and the annular housing, unt

Insertion of a mechanical artificial heart valve is an absolute indication for the use of lifelong oral anticoagulation, irrespective the presence or absence of atrial fibrillation. Depending on the type and position of the artificial valve the optimal International Normalized Ratio (INR) for these patients is between 2.5 and 4.5, although more prospective studies on the optimal range of oral anticoagulation are necessary. Addition of low dose antiplatelet therapy may further decrease the rate of thromboembolism, but also here more studies are needed. Patients with a bioprosthesis need only or

Infective endocarditis is a serious heart disease that can affect native and prosthetic valves. Staphylococci and Streptococci are the etiological agents in almost 80% of cases. Human brucellosis is a multiple organ disease often transmitted via contaminated, unpasteurized goat milk and cheese. Brucella endocarditis is a rare complication associated with elevated morbidity and mortality rates; it can affect native or, even more rarely, prosthetic valves. In this report, we present the successful management of an infected aortic biological prosthesis in a man of 68 years.

In cases of severe atherosclerosis of the ascending aorta, alterations in the standard surgical technique are mandatory. We report mitral valve replacement and coronary artery bypass grafting in a patient with a severely atherosclerotic aorta. Cardiopulmonary bypass was conducted via an arterial cannula in the femoral artery and two single venous cannulas. Coronary artery bypass grafting was performed using bilateral internal thoracic arteries with beating heart in normothermia. The mitral valve was replaced with a mechanical prosthesis during hypothermic fibrillatory arrest.

A patient aged 42 with the aortic valve prosthesis was examined with the help of coronography and ultrasonic scanning of the heart in the period of ischemic injury of the anterior myocardium: occlusion of the unchanged anterior descending artery by an embolus formed on the artificial cardiac valve was revealed. Despite rapid recovery of arterial permeability (3 h 47 min, after the onset of disease) by intracoronary administration of streptase with subsequent anticoagulant and antiplatelet drug therapy the patient developed anterior myocardial infarction with left ventricular aneurysm and intra

BACKGROUND: Patients with mechanical valvular prostheses are becoming more numerous in our country. Their medical care poses a number of problems in a particular socio-economic context of our geographical regions.
AIM: The purpose of this study was to identify the complications of this heart valve replacement surgery by mechanical prostheses and try to compare our results with those of literature.
METHODS: From January 1998 to June 2003, 861 patients were operated for acquired valvular heart disease (413 men and 448 women), average age 39±11 years. It was 495 mitral valve replacement, 205 ao

Mitral and tricuspid valve replacement options have traditionally been limited to mechanical valves, bioprostheses, and the mitral Ross procedure. In recent years, the Melody valve (Medtronic, Minneapolis, MN) has emerged as a viable alternative for infants and young children. We present four patients under two years of age who underwent surgical implantation of a modified Melody valve 22 mm in the atrioventricular position. The valve was placed in the mitral position in three patients and the tricuspid position in one. The procedure was performed with palliative intent in all patients, as no

A 77-year-old female patient was referred to our department with dyspnoea, peripheral oedema and a history of right heart failure. Echocardiography revealed torrential tricuspid regurgitation. Since tricuspid-transcatheter edge-to-edge repair was not feasible and the patient refused open heart surgery, heart team decision was made to perform transcatheter tricuspid valve implantation in a borderline annulus dimension. During the procedure, migration of the valve in the right ventricle occurred. After patients' consent was obtained surgical explantation of the valve with subsequent surgical tri

Immobilisation of mechanical valve leaflets can be a life-threatening complication. In the acute setting, medical therapy can be attempted but is not always successful. We present the first described case of a patient with a mechanical tricuspid valve with recurrent leaflet immobilisation that was able to be mobilised using a transcatheter knocking technique.

Aortic stenosis (AS) is a serious condition in the aging US and European populations. Management of a stenotic valve is crucial as it can become symptomatic quickly leading to ventricular deterioration and overall poor quality of life. Considering that AS is a disease of the elderly patient population, surgical intervention may not be well tolerated by some patients. Transcatheter aortic valve implantation (TAVI) has emerged as an alternative approach for patients who are unsuitable surgical candidates. Since the first balloon-expandable Edwards SAPIEN valve (Edwards Lifesciences, Irvine, CA)

Valvular heart disease is cardiovascular pathology that affects over 100 million people worldwide and is associated with significant morbidity and mortality. Although tricuspid valve disease is the least common primary valve pathology, it is linked to a significantly increased mortality rate (up to 42% within 3 years). Valve thrombosis is a rare complication following mechanical valve replacement, characterized by thrombus accumulation on the valve leaflets, leading to limited valve movement, often in patients not achieving anticoagulation targets. Mechanical valve thrombosis is suspected in p
