PURPOSE: Percutaneous nephrostomy (PCN) in a nondilated pelvicaliceal system is technically challenging. We describe an effective method to achieve transient dilatation of the pelvicaliceal system via induction of diuresis using infusion of a diuretic agent in normal saline, therefore allowing easier access to the pelvicaliceal system.

We report on a male newborn with urinary ascites and perirenal urinoma due to calyceal rupture caused by bladder outlet obstruction and posterior urethral valves. Despite the presence of left-sided high-grade reflux, the "pop-off" mechanism occurred in the non-refluxing right kidney. Impressive normalization of the upper urinary tract anomalies and renal function occurred after surgical treatment of the valves and removal of obstruction.

Percutaneous placement of nephrostomy catheters in patients with non-dilated pelvicaliceal systems remains problematic. We successfully performed ultrasound-guided percutaneous nephrostomies in 5 such patients after intravenous administration of a diuretic agent. Diuresis resulted in transient dilatation of the calyces, facilitating the procedures, which were performed under real-time sonographic guidance using the Seldinger technique. No complications were encountered.

The authors report an exceptional case of delayed diagnosis of avulsion of the ureteropelvic junction in a 27-year-old man. Organization of the urohaematoma prevented ureteropelvic anastomosis and the kidney could only be preserved by performing ureterocalicostomy. This technique, generally used for tuberculous retracted renal pelvis, or failure of pyelotomy or ureteropelvic junction repair in horseshoe kidneys or kidneys with retracted renal pelvis, allowed renal preservation in this case.

PURPOSE: Fornix rupture is a urological emergency situation. In most cases ureteral stones represent the underlying cause. We present the largest series ever reported.
MATERIAL AND METHODS: Between 1994 bis 2005 a total of 162 patients were diagnosed with a fornix rupture. There were 121 men and 41 women with a median age of 53 years (range: 20-79 years).
