Acute Exacerbation of Chronic Obstructive Pulmonary Disease Immediately After Extubation Following Percutaneous Nephrolithotomy: A Case Report.

In patients with moderate-to-severe COPD, short-term (1-day) preoperative respiratory optimization may be insufficient to prevent acute exacerbation after extubation. Prolonged preoperative preparation, meticulous intraoperative lung-protective ventilation, and a post-extubation rapid-response protocol are essential.

Performance of the Quadruple-D Score Versus Other Nephrolithometric Tools in Obese Patients Undergoing Percutaneous Nephrolithotomy.

OBJECTIVE: To evaluate whether the CT-based Quadruple-D score predicts stone-free status after percutaneous nephrolithotomy (PCNL) in obese patients and to compare it with established nephrolithometric scores, including SCR, CROES, Guy's Stone Score, and S.T.O.N.E. METHODS: We retrospectively analyzed obese adults (BMI > 30 kg/m²) who underwent PCNL at a tertiary center between January 2013 and June 2025 and had preoperative non-contrast CT and 1-month postoperative imaging. The Quadruple-D score (0-4) assigned one point each for stone volume <500 mm³, attenuation <1000 HU, skin-to-stone dist

Mean Changes in Estimated Glomerular Filtration Rate in Patients Undergoing Percutaneous Nephrolithotomy Having Renal Stone Disease.

Background Urinary stone disease is associated with renal impairment because of obstruction and infection. Comorbidities include hypertension, dyslipidemia, diabetes, and impaired renal function. Furthermore, as recurrences are common in urolithiasis, such patients undergo many treatments throughout their life. Percutaneous nephrolithotomy (PCNL) is an effective treatment for renal stones with a diameter greater than 2 cm. The primary objective of this study was to observe the mean changes in estimated glomerular filtration rate (GFR) in patients undergoing PCNL having renal stone disease. Met

Acute delayed onset hemodynamic and respiratory compromise due to thoracic translocation of hemourinoma after percutaneous nephrolithotomy.

Percutaneous nephrolithotomy (PCNL) despite its minimally invasive approach has an high complication rate, with the most common complications being extravasation of urine and perioperative bleeding requiring transfusion. While most of these complications are minor, many serious and life-threatening complications do occur. One such complication is the development of hemothorax or hydrothorax which usually develops in the early postoperative period with blood or urine passing from the surgical site through a newly established pleural-peritoneal fistula. Here we describe an unusual presentation a

Autonomic hyperreflexia in spinal cord injury patient during percutaneous nephrolithotomy for renal stone: a case report.

Autonomic hyperreflexia or autonomic dysreflexia is a syndrome seen in approximately 85% of all quadriplegic patients and constitutes the only medical emergency related to spinal cord injury. We report on a paraplegic patient with a C5 to C7 spinal cord injury who suffered autonomic hyperreflexia during percutaneous nephrolithotomy for a right renal stone. The reflex-induced hypertension subsided with termination of the operation, and administration of an alpha and beta-adrenergic blocking agent (labelatol hydrochloride). The stone was removed successfully by pyelolithotomy with the patient un

Suction-Assisted Miniaturized Percutaneous Nephrolithotomy Outcomes in Anomalous Kidneys: A Multicenter Prospective Study-An EAU-Endourology Collaboration.

BACKGROUND: Renal stones in anomalous kidneys pose surgical challenges due to altered anatomy. Miniaturized percutaneous nephrolithotomy (mini-PCNL) reduces morbidity, but concerns remain about stone-free rates (SFRs) and infection. Suction-assisted mini-PCNL (SM-PCNL) enhances fragment removal and controls intrarenal pressure, but its role in anomalous kidneys is unclear. OBJECTIVE: To evaluate perioperative outcomes of SM-PCNL in anomalous kidneys in a multicenter, real-world study and assess variations based on positioning, lithotripsy modality, and renal anomalies. METHODOLOGY: This pros

Long-term outcomes of percutaneous nephrolithotomy in 502 chronic kidney disease patients: insights from a tertiary center.

INTRODUCTION: Urinary stone disease is a significant risk factor for chronic kidney disease. Percutaneous nephrolithotomy is the standard treatment for complex renal stones, yet its long-term effects on renal function in such patients remain underexplored. This study evaluates the outcomes, complications, and factors influencing percutaneous nephrolithotomy success in chronic kidney disease patients. MATERIALS AND METHODS: This single-center study included 502 chronic kidney disease patients undergoing percutaneous nephrolithotomy between March 2019 and April 2024. Outcomes assessed were chan

C-arm-Guided Paravertebral Block for Surgical Anesthesia in a High-Risk Cardiac Patient Undergoing Percutaneous Nephrolithotomy.

Percutaneous nephrolithotomy (PCNL) is an effective, minimally invasive procedure for removing large or complex renal stones. While general anesthesia is commonly used, it may lead to complications such as hemodynamic instability. This procedure typically involves accessing the kidney through a small incision in the back, where a nephroscope and other instruments are inserted to break up and remove the stones. General anesthesia is standard, but it can cause fluctuations in blood pressure, heart rate, and oxygen levels, which may complicate recovery. In this case, a 51-year-old male with dilat
