BACKGROUND: Neurological trauma results in a state of hypermetabolism. Existing literature has explored barriers to adequate nutrition following traumatic brain injury. Despite clinical similarities, it remains unclear whether similar nutritional barriers are faced following both planned and emergency neurosurgical procedures. This study aimed to describe nutritional management, the prevalence of barriers to nutrition, and their association with selected patient outcomes among longer-stay patients undergoing neurosurgical procedures.
METHODS: Medical records were reviewed for hospitalised neu…

OBJECTIVE: Modern neurosurgical training is increasingly challenged by declining surgical caseloads and reduced hands-on opportunities. While haptic simulators offer safe and repetitive practice, their adoption remains limited due to high costs, suboptimal realism, and lack of standardization. This study introduces a highly realistic, low-cost, and fully replicable microsurgical simulator for repetitive training in complex cranial procedures, including sylvian fissure and white matter dissections, aneurysm clipping, and resection of anterior skull base, frontal, and temporal lobe tumors.

BACKGROUND AND OBJECTIVES: The electrophysiological process of emergence from general anesthesia (GA) commences from deeper structures of the brain rather than the cortex. The phylogenetically old parts of the brain (hippocampus) are the first to recover during emergence. Study objectives were to evaluate and predict the effect of preoperative hippocampal volume (HV) measured using MRI with emergence from GA and delayed extubation in patients undergoing elective neurosurgical procedures.
METHODS: Patients aged 18-65 years of age scheduled for elective neurosurgical procedures under GA with GC…

BACKGROUND: Bacterial meningitis is a rare but serious complication following neurosurgical procedures. Acinetobacter baumannii (A. baumannii) has emerged as an opportunistic pathogen, causing serious infections worldwide. The increasing prevalence of multidrug-resistant strains presents growing challenge in healthcare settings. The occurrence of multidrug-resistant A. baumannii meningitis after Posterior Lumbar Interbody Fusion (PLIF) surgery is exceptionally uncommon.
CASE PRESENTATION: We report a 59-year-old osteoporotic female who developed A. baumannii meningitis resistant to our availa…

BACKGROUND: To analyze the clinical characteristics of patients with complex skull base lesions involving both intracranial and extracranial regions and summarize the anesthetic management experience for combined neurosurgical and oral-maxillofacial procedures under a single anesthesia session. CASE PRESENTATION: Medical records of patients who underwent combined neurosurgical and oral-maxillofacial surgery at Beijing Tiantan Hospital, Capital Medical University, between April 1, 2020, and April 1, 2025, were retrospectively reviewed. BACKGROUND: There are numerous procedures in which, beyond adequate manipulation of the temporalis muscle and cranial closure, patients may present bone and muscle defects due to atrophy and consequent facial asymmetry, causing psychological discomfort and functional deterioration. The objective of our work is to combine the knowledge of plastic surgery and apply it to cranial reconstructions with fat transfers in post-neurosurgical patients, analyzing its results.
METHODS: During the year 2022, 45 fat transfer procedures were performed for the correction of craniofacial defects, of which 29

BACKGROUND: Unplanned readmission has gained increasing interest as a quality marker for inpatient care, as it is associated with patient mortality and higher economic costs. Spinal neurosurgery is characterized by a lack of epidemiologic readmission data. The aim of this study was to identify causes and predictors for unplanned readmissions related to index diagnoses and surgical procedures.
METHODS: In this study, from 2015 to 2017, spinal neurosurgical procedures were recorded for surgical and non-surgical treated patients. The main reasons for an unplanned readmission within 30 days follo…

INTRODUCTION: Achieving tension-free scalp closure after decompressive craniectomy or cranioplasty can be quite demanding, especially in patients who have developed fibrosis or infection from previous procedures. In such situations, galeal scoring (galeotomies) can improve scalp mobility. Although this technique is well known in plastic surgery, it is not commonly reported in neurosurgical work.
CASE PRESENTATION: A 30-year-old man sustained a high-velocity head injury that produced a large right fronto-parietal acute subdural hematoma with marked midline shift. He underwent an emergency deco

BACKGROUND: Intraoperative ultrasound (iUS) has emerged as a valuable imaging resource in neurosurgical procedures. To improve familiarity with iUS technology and optimize its usage, additional exposure and technical demonstrations are needed. Spinal arachnoid cysts (SACs) are rare spinal cord lesions most often treated with resection, with procedural challenges stemming from poor preoperative lesion visualization and the infrequency of the procedure. The authors present a case series to detail the use of iUS in assisting with the resection of SAC.
OBSERVATIONS: The cases of 2 patients with s…

BACKGROUND: Meyer's loop (ML), the anterior extension of the optic radiation, is critical in neurosurgical planning for temporal lobe procedures. Establishment of accurate localization of ML relative to key anatomical landmarks is crucial to minimize post-surgical visual field deficits.
MATERIALS AND METHODS: A systematic review was conducted using PubMed, Scopus, and Embase databases from September to December 2024, following PRISMA 2020 guidelines. Anatomical and radiological studies focused on distances from Meyer's loop to surgical landmarks were included.
RESULTS: Out of 1,466 identifie

The main goal of brain tumor surgery is to achieve gross total tumor resection without postoperative complications and permanent new deficits. However, when the lesion is located close or within eloquent brain areas, cranial nerves, and/or major brain vessels, it is imperative to balance the extent of resection with the risk of harming the patient, by following a so-called maximal safe resection philosophy. This view implies a shift from an approach-guided attitude, in which few standard surgical approaches are used to treat almost all intracranial tumors, to a pathology-guided one, with surgi

BACKGROUND AND OBJECTIVES: The transorbital approach varies by the extent of bony removal and the target. Orbital rim-sparing transorbital approach with removal of only the orbit's posterior wall provides optimal cosmetic results, without the need for reconstruction. The size of this corridor, limited by the medial globe retraction, has not yet been defined and is difficult to determine in cadavers because of postmortem tissue desiccation. By using patient-specific models in virtual reality, precise areas and degrees of surgical freedom (AOF and DOF, respectively) provided by globe retraction

PURPOSE: Digital 3D exoscopes have been recently introduced as an alternative to a surgical microscope in microneurosurgery. We designed a laboratory training program to facilitate and measure the transition from microscope to exoscope. Our aim was to observe the effect of a one-year active training on microsurgical skills with the exoscope by repeating a standardized test task at several time points during the training program.
METHODS: Two board-certified neurosurgeons with no previous exoscope experience performed the same test tasks in February, July, and November during a 12-month period

Brainstem is one of the most complex structures of the human body, and has the most complex intracranial anatomy, which makes surgery at this level the most difficult. Due to its hidden position, the brainstem became known later by anatomists, and moreover, brainstem surgery cannot be understood without knowing the evolution of ideas in neuroanatomy, neuropathology, and neuroscience. Starting from the first attempts at identifying brainstem anatomy in prehistory and antiquity, the history of brainstem discoveries and approach may be divided into four periods: macroscopic anatomy, microscopic a

A brain abscess is a focal accumulation of pus in the brain parenchyma arising from direct inoculation, contiguous spread from local anatomical structures or haematogenous seeding from a remote source of infection. It can result in significant morbidity and mortality, making early diagnosis and treatment vital. Only one fifth of patients present with the classic triad of headache, fever and focal neurological symptoms. More commonly patients show signs and symptoms of raised intracranial pressure alone, such as confusion or reduced conscious level, headache, nausea and vomiting, which can be a
